Methods: Twenty-five patients with idiopathic Parkinson disease with unilateral symptoms (IPDUS) and 25 healthy volunteers were enrolled in this study. MRS of the lentiform nucleus in each patient was taken and then concentrations of N-acetylaspartate (NAA), Creatine (Cr) and Choline (Cho) were calculated. Results: Compared to that in the control, NAA/ (Cho+Cr) was significantly lower in the lentiform nucleus contralateral to symptoms and even that in the ipsilateral side in IPDUS patients (all P<0.05); while there was no difference between the two sides in the healthy volunteer (P>0.05). The ratio of NAA/(Cho+Cr) ipsilateral to the sympatomatic side of the patient was also lower than that of the control (P<0.05). Conclusions: there might be some changes with MRS on the lentiform nucleus during the early stage of idiopathic Parkinson抯 disease with unilateral symptom. MRS may be one of the reliable methods for early or even sub-clinical diagnosis.展开更多

Modified titration prot...This study looked into the efficacy of a modified titration protocol of intratympanic gentamicin injection(ITG) in the patients with unilateral intractable Ménière's disease(MD). Modified titration protocol of ITG at a low dose(20 mg/m L) was administered to 10 patients with definite unilateral intractable MD. After initial first two fixed ITGs on weekly basis,the patients might or might not be given any more injections,depending on the appearance of unilateral vestibular loss(UVL). ITG was terminated if the patients satisfied the criteria of UVL. All patients were followed-up for at least two years. The effects of ITG on the vertigo attack,functional level scores and postural balance were evaluated. Of the 10 cases,8 showed the sign of UVL after receiving initial two ITGs and were not given any more intratympanic injections,and the other 2 patients were administered three ITGs. A two-year follow-up revealed that complete and substantial vertigo control was achieved in 9 cases,and limited vertigo control in 1 patient. Hearing level was lowered in 2 patients. The posture stability and functional level scores were improved. Our study showed that the modified titration protocol of ITG at a low dose could effectively control vertigo in patients with unilateral intractable MD.展开更多
AIM:To report on the utilization of eye care services and its associated factors among those with unilateral visual impairment(VI)in a rural South Indian population.METHODS:A population based cross-sectional study...AIM:To report on the utilization of eye care services and its associated factors among those with unilateral visual impairment(VI)in a rural South Indian population.METHODS:A population based cross-sectional study was conducted in three districts(Adilabad,Mahbubnagar and West Godavari)in the state of Andhra Pradesh,India.A detailed interview and a comprehensive eye examination were conducted. Those with unilateral VI were asked questions about noticing any change in vision and on utilization of eye care services. The most important reason reported by the participant for not utilizing the services was used for the analysis. Multiple logistic regression models were used to examine the association between noticing a change in vision and socio-demographic variables such as age,gender,education and area of residence,severity and causes of VI.RESULTS:Among the 4456 participants aged ≥16y who were administered the questionnaire,53.2% were women,and 54.7% had no education. Of the 489(11%; 95% CI:10.1-11.9)people with unilateral VI,399(81.6%)participants reported noticing a change in their vision over the last five years but only 136(34.1%)participants had soughteye care consultation. Those who had any education(OR:1.9; 95% CI:1.1-3.2),had blindness(OR:2.7; 95% CI:1.4-5.2),and cataract(OR:2.1; 95% CI:1.0-4.3)as a cause of unilateral VI were more like to seek eye care consultations. The most commonly reported reasons for not seeking eye care services were "do not have money for eye checkup" in 30.7% of the participants followed by "do not have a serious problem"(30.0%).CONCLUSION:A large proportion of rural population though noticed a change in their vision did not seek eye care due to financial and person-related reasons. Eye care service providers need to address these barriers to enhance the uptake of eye care services among those with unilateral VI.展开更多

And it is named moyamoya disease because of a characteristic "puff-of-smoke" angiographic appearance. The steno-occlusive process is typically seen involving bilateral internal carotid arteries at their clinoid portion and/or the proximal portion of the anterior cerebral arteries (ACAs) and/or the middle cerebral arteries (MCAs). The coexistence of moyamoya disease and arteriovenous malformation (AVM) is thought to be very rare.展开更多
