Chemical Composition and Use of Momordica charantia L. and Hyptis spicigera Lam. Extracts as Mosquito Larvicides and Insect Growth Regulators against Malarial Vector, Anopheles gambiae Giles

Background: Mosquitoes are responsible of numerous vector-borne diseases and among these diseases is malaria which takes away lives of thousands of people, especially children of under five, particularly in Africa. To reduce the mortality and economic burdens of this deadly disease, synthetic insecticide has been of use to control its main vector, Anopheles gambiae. Due to adverse effects caused by these conventional products on non-target organisms and the environment, plants have been of first choice as they proved their effectiveness against mosquitoes and are said to be eco-friendly and relatively safer. Therefore, the aim of this study was to screen the phytochemicals and determine the larvicidal and adult emergence exhibitory activities of Momordica charantia and Hyptis spicigera leaves extracts against Anopheles gambiae larvae. Methods: The WHO standard protocol for larvicidal activity and IGRs was followed up and concentrations from 500 - 2000 ppm for larvicidal and 500 - 1500 ppm for IGRs were set up. Results: There was the presence of alkaloids, flavonoids, tannins, phenols, steroids, terpenoids and oils in Momordica charantia against alkaloids, taponins, tannins, phenols, steroids, terpenoids and oils in Hyptis spicigera. Methanol crude extract registered the lowest LC<sub>50</sub> value of 270.6 ppm followed by ethyl acetate and hexane fractions recorded the LC<sub>50</sub> of 742.1 and 756 ppm, respectively for larvicidal activity of Momordica charantia against LC<sub>50</sub> of 760 and 867.5 ppm for hexane fraction and methanol crude extract, respectively for Hyptis spicigera. Methanol crude extract and hexane fraction of both plants as well as the ethyl acetate fraction of M. charantia registered greater emergence inhibition with IE<sub>50</sub> values of 590.2, 842.3 and 982 ppm for methanol crude extract, hexane and ethyl acetate fractions of M. charantia, respectively against 901.7 and 873.2 for methanol crude extract and hexane fraction of H. spicigera, respectively. Conclusion: The toxicity and adult emergence inhibition may be associated with constituents in both plants that interfere with the normal neuronal, respiratory and endocrine systems functions. 展开更多

Background: Anopheles gambiae is enemy number one of mankind in Africa and particularly in Cameroon due to its ability of transmitting malaria which is the deadliest disease in this part of the world. Synthetic insecticides have been used to control malaria vectors but they have negative effects on non-target organisms and are environmentally unfriendly. Control of mosquitoes at larval stages using phytochemicals is currently the leading tool to reduce the mosquito population and so the reduction of malaria transmission rates. Therefore, the present study was to evaluate the phytochemical contents, larvicidal and adult emergence inhibition activities of Balanites aegyptiaca seed and Aristolochia albida root solvents extracts against Anopheles gambiae larvae. Methods: The World Health Organization standard protocols were followed for the different bioassays. Concentrations ranging from 500 - 2000 ppm for larvicidal and 500 - 1500 ppm for IGRs were used. Results: Both plants showed the presence of alkaloids, flavonoids, saponins, tannins, phenols, terpenoids and oils except the absence of phenols in Aristolochia albida. The larvicidal activity of Aristolochia albida extracts showed that hexane and methanol fractions were the most active killing relatively all exposed larvae with the LC<sub>50</sub> values of 420.1 and 453 ppm, respectively. The same observation was made in hexane fraction from Balanites aegyptiaca (LC<sub>50</sub> = 588 ppm). The insect growth inhibitory activity of Aristolochia albida extracts proved that hexane and methanol fractions relatively caused a 100% inhibition in the mosquito development, recording the EI<sub>50</sub> values of 482.4 and 555.6 ppm, respectively. The same trend was observed with Balanites aegyptiaca hexane fraction registering better EI<sub>50</sub> of 623.9 ppm. Conclusions: Our findings demonstrate that Balanites aegyptiaca seed and Aristolochia albida seed extracts are rich in phytochemicals capable of killing mosquito larvae and disrupting mosquito larval development. This could contribute to the control of mosquito populations and improved management of malaria. 展开更多

Introduction: Gout is defined as an arthritic condition resulting from the deposition of monosodium urate crystals in and/or around joints, following long-standing hyperuricemia. This may cause gouty arthritis in joints and tophi in soft tissues. Spinal gout is rare and never mentioned in our context. It can appear as acute back pain, radiculopathy, spinal cord compression, spondylodiscitis or neoplasic/infectious epiduritis. Our aim was to share our surgical experience and proceed of a Literature review. Cases Presentation: Between January and August 2022, two patients male were surgically cared, aged of 42 and 60 years old. The gout was unknown in the youngest and poorly followed in the eldest. There was no past medical history of tuberculosis or immunodeficiency in both. The early diagnosis retained was unspecific lumbar spondylodiscitis due to clinical features: Patients complained both of lower back pain with initial fever. It was of a progressive left L5S1 deficit with erectile defect and dysuria in the first case and a progressive paraplegia without sphincter disorders in the second case. We proceeded with a lumbar laminectomy with a biopsy on both patients. The spinal tophus was ligamentous in one case and arthro-ligamentous in the other. There was a progressive motor recovery from postoperative Day-2 till postoperative Month-1. A probabilistic antituberculosis treatment was promptly initiated postoperatively based on radioclinic features while waiting for histologic proof. The Polymerase Chain Reaction (PCR) of Mycobacterium tuberculosis was negative and the histology was of a chronic calcified osteitis with dense fibrosis in both. The anti-gout treatment was implemented after 15 days with blood test evidence. A rheumatologic follow-up was also initiated and adjuvant physio-therapy. The results were very satisfactory from 4 - 6 months with independent walking. Discussion Conclusion: Spinal Gout may be suggested in 40-male-old faced with any acute rachialgia with neuro deficit with dubious neuro-imaging. 展开更多

Introduction: The health cheque system is a prepayment mechanism aimed at reducing neonatal and maternal mortality through improving the management of pregnant women. The pregnant woman with the health cheque system that she pays at six thousand francs XAF (African financial community) is covered free of charge for all the care provided by the cheque system in the health facilities accredited to the health cheque project. We did a study, with objective to determine the hospital outcome of newborns with a health cheque system (HCS) compared to those without health cheque system. Method: A descriptive cross-sectional study with retrospective data collection was carried out at the Ngaoundere Regional Hospital from January 2018 to September 2021. Results: During our study period, 2985 newborns were received. We saw an increase in admissions over the years, particularly in the group of newborns with the health cheque system. Comparatively, the percentage of newborns cured in the health cheque system group was 76.73% (n = 1643) versus 77.72% (n = 656) those in the non-health cheque system group. Those who died were 8.96% (n = 192) in the health cheque system group compared to 6.27% (n = 53) in the non-health cheque system group. Conclusions and Recommendations: Most patients admitted to our service have the health cheque system. We notice an increase in hospital attendance with the health cheque project. The outcome of the newborn under the health cheque system is not different from that without health cheque system. The health cheque system was successful in getting the larger number of newborns into care. The next step is to put strategies in place to keep these patients in care for the duration of hospitalization. 展开更多

This study aspired to assess the impact of the COVID-19 pandemic on the attendance of patients at the Radiology and Medical Imaging Department of a tertiary hospital in Douala, Cameroon. Materials and Methods: This was an observational, retrospective and descriptive hospital-based study. Data on patient attendance was extracted from the medical records at the Radiology Unit of Douala Gyneco-Obstetric and Pediatric Hospital (DGOPH) between the 1<sup>st</sup> of March 2019 and the 30<sup>th</sup> of August 2021. Results: There was a drop in the number of sonographies and conventional radiographies by 25.73% and 18.26% respectively during the peaks pandemic period of 2020 compared to the preceding pre-pandemic era (2019). There was a recovery in 2021 as the numbers increased. Computed tomography studies increased by 40.65% during the peak pandemic period of 2020. Conclusion: This study enables us to understand the health-seeking behavior during the COVID-19 pandemic. The increase in CT utilization during the peak period of the pandemic nevertheless required caution in order to ensure safety and to promote rational cost-effective utilization. 展开更多

However, in the wake of SARS-CoV-2 virus pandemic, chest computed tomography (CT) is necessary to assess the severity of the disease. The objective of this study was to compare the chest CT findings of pregnant and non-pregnant women with SARS-CoV-2 viral pneumonia at a tertiary hospital setting. <strong>Materials and Methods: </strong>This was a cross-sectional, retrospective, descriptive study of admission records of pregnant and non-pregnant patients aged 15 - 49 years during a three (3) months period (April-July 2020) in DGOPH. Logistic regression was used to search for predictors of lung involvement during COVID-19. <strong>Results:</strong> A total of 31 patient files were studied of which 9 (29%) were those of pregnant women. The pregnant women mostly had a combination of lung ground-glass opacities and consolidation on CT compared to the non-pregnant women (7 out of 9 cases, 77.8%, vs. 6 out of 22 cases, 22.7%;p = 0.01). The group of pregnant women had more than 3 affected lobes (50.0% vs. 22.7%, p = 0.04) with a middle lobe predilection (77.8% vs. 31.8%, p = 0.02). CT predictors of COVID-19 pneumonia in the group of pregnant women after the multivariable logistic regression analysis were the presence of nodules (aOR = 13.9;95% CI: 1.25 - 134.2;p = 0.032) and linear bands of interlobular septal thickening (aOR = 17.8;95% CI: 1.46 - 217.6;p = 0.024). <strong>Conclusion: </strong>In this study, the chest CT of pregnant women with COVID-19 pneumonia revealed mostly a combination of findings compared to non-pregnant women, with more affected lobes. These findings suggest the likelihood of a greater CT severity of COVID-19 pneumonia among pregnant women and therefore the need for timely and appropriate management. 展开更多

The aim of this study was to contribute to the improvement of the management of cervical cancer by describing the MRI aspects of cervical cancer according to the IFOG-MRI classification. <strong>Material and Methods:</strong> This was a cross-sectional study, conducted in patients who came for clinical suspicion, extension assessment of histologically confirmed cervical cancer, from October 2020 to March 2021, in Yaoundé (Cameroon). <strong>Results:</strong> 54 patients were selected, with an average age of 50.4 years. Genital hemorrhage was the most frequent symptom (92%). The mean tumor size was 52.9 mm. According to IFOG staging, stages Ia, Ib, IIa, IIb, IIIa, IIIb, and IV accounted for 9.10%, 10.20%, 12.50%, 29.50%, 5.70%, 12.50%, and 20% respectively. No patient had known metastasis at the time of examination. <strong>Conclusion:</strong> MRI allows non-invasive, satisfactory tumor staging examination by providing information on tumor size, parametrium invasion, adjacent organ involvement, and the presence of lymph node metastasis. 展开更多

The fundamental step to improve this situation is to determine 1) mothers’ knowledge regarding anemia (symptoms/signs and etiologies), and 2) their attitudes towards anemia. The present effort was to determine 1) and 2) on mothers (n</span><span style="font-family:""> </span><span style="font-family:Verdana;">=</span><span style="font-family:""> </span><span style="font-family:Verdana;">152) with children (1</span><span style="font-family:""> </span><span style="font-family:Verdana;">-</span><span style="font-family:""> </span><span style="font-family:Verdana;">5 years) through questionnaire. Data showed that the less educated the mother was, the more she gave the wrong definition of anemia. When these mothers have anemic children, 129 go to the hospital, 34 give grenadine (sweet soda), 14 go to the traditional healer, and 6 give the various decoctions. Our results showed that less-educated mothers had wrong anemia management, such as attachment to a visual logic that assimilates blood to food with red coloring. The actions must directly involve these mothers and the traditional healers to improve this situation. 展开更多
