Lung cancer is associated with a heavy cancer-related burden in terms of patients’physical and mental health worldwide.Two randomized controlled trials,the US-National Lung Screening Trial(NLST)and Nederlands-Leuvens Longkanker Screenings Onderzoek(NELSON),indicated that low-dose CT(LDCT)screening results in a statistically significant decrease in mortality in patients with lung cancer,LDCT has become the standard approach for lung cancer screening.However,many issues in lung cancer screening remain unresolved,such as the screening criteria,high false-positive rate,and radiation exposure.This review first summarizes recent studies on lung cancer screening from the US,Europe,and Asia,and discusses risk-based selection for screening and the related issues.Second,an overview of novel techniques for the differential diagnosis of pulmonary nodules,including artificial intelligence and molecular biomarker-based screening,is presented.Third,current explorations of strategies for suspected malignancy are summarized.Overall,this review aims to help clinicians understand recent progress in lung cancer screening and alleviate the burden of lung cancer. 展开更多

BACKGROUND To investigate the clinicopathological features of endometrial clear cell carcinoma that has invaded the right oviduct with a cooccurring ipsilateral oviduct adenomatoid tumor.CASE SUMMARY A case of endometrial clear cell carcinoma invading the right oviduct with a cooccurring ipsilateral oviduct adenomatoid tumor was collected and analyzed using pathomorphology and immunohistochemistry.Endometrial clear cell carcinoma cells were distributed in a solid nest,papillary,shoe nail-like,and glandular tube-like distribution.There was infiltrative growth,and tumor cells had clear cytoplasm and obvious nuclear heteromorphism.The cancer tissue was necrotic and mitotic.The cancer tissue invaded the right oviduct.The ipsilateral oviduct also had an adenomatoid tumor.The adenomatoid tumor was arranged in microcapsules lined with flat or cubic cells that were surrounded by smooth muscle tissue.The adenomatoid tumor cells were round in shape.CONCLUSION Clear cell carcinoma of the endometrium can invade the oviduct and occur simultaneously with tubal adenomatoid tumors.Upon pathological diagnosis,one should pay close attention to distinguishing whether an endometrial clear cell carcinoma is invading the oviduct or whether it is accompanied by an adenomatoid tumor of the oviduct.Immunohistochemistry is helpful to differentiate these two disease entities.Endometrial clear cell carcinomas express Napsin-A and P16 and are negative for estrogen receptor and progesterone receptor.The presence of endometrial clear cell carcinoma does not affect the expression of CK and calretinin in adenomatoid tumors. 展开更多

Analysis of infection and prognosis of extensively drug resistant Acinetobacter baumannii in ICU

Objective:To investigate the infection of patients in the ICU with extensively drug-resistant Acinetobacter baumannii(XDRAB)and analyse the effect of tigecycline therapy for XDRAB.Methods:Seventy-one patients admitted to the ICU with XDRAB infection from January 2013 to July 2017 were divided into the tigecycline group(combination therapy with 50 mg tigecycline and 0.1 g minocycline every 12 hours)or the control group(no tigecycline treatment).Patient data,including age,sex,history of malignant tumour,body temperature,APACHE Ⅱ score,ALT,AST,GGT,TB,ALB,TBIL,DBIL,IBIL,BUN,Cr,absolute neutrophil count,procalcitonin(PCT),site positive for XDRAB infection,length of stay,and prognosis,were collected and compared between the two groups.Binary logistic regression was used to analyse the prognostic risk factors for XDRAB.Results:A total of 61.6% of the patients infected with XDRAB had multiple sites of XDRAB colonization.The cure rate achieved in the tigecycline group was higher than that of the control group(70.7%vs 41.9%).Multivariate analysis showed that older age,a high level of PCT after treatment and the absence of tigecycline treatment were poor prognostic factors for XDRAB infection.Conclusion:Combination therapy with tigecycline and minocycline is effective for XDRAB infection and reduces the cost of treatment.Combined therapy with tigecycline is a predictor of good prognosis for patients infected with XDRAB,whereas older age and increased PCT levels after treatment are predictors of poor prognosis. 展开更多

Inflammatory cytokine storms can trigger disease exacerbation and even death and have reached a consensus in the clinical treatment of acute organ failure.However,the existing strategies remain a great challenge to efficiently suppress inflammatory cytokine storms for promoting organ repair and regeneration.Herein,fully human umbilical cord(UC)-derived adhesive materials(UCAM)that integrate decellularized extracellular matrix(ECM)nanofiber hydrogel and homologous mesenchymal stem cells(MSCs)are demonstrated to greatly suppress inflammatory cytokine storms,demonstrating high efficacy in treating acute liver failure(ALF)in rats with 90%hepatectomy.The UC-derived adhesive materials have the capacity to secrete a significant quantity of cytokines by MSCs to recruit activated immune cells to migrate into their ECM nanofiber networks,segregating them away from the infection area and thereby greatly suppressing the inflammatory cytokine storms.As expected,the UC-derived adhesive materials can significantly promote hepatocyte proliferation to achieve functional recovery and regeneration of the liver,significantly improving the survival rate in rats.Our fully human UC-derived adhesive materials provide a new avenue in suppressing inflammatory cytokine storms for promoting organ regeneration that would be really utility in clinical organ transplantation-related treatment. 展开更多

To study the therapeutic effect of Chinese herbal medicine (CHM) combined with bronchial arterial chemotherapy (BAC) in treating lung cancer.Methods: Ninety patients with mid-advanced non-small cell lung cancer (NSCLC) were randomly divided into two groups. The 45 cases in Group A were treated with CHM combined with BAC and the 45 cases in Group B treated with BAC alone. The short-term and long-term effect, follow-up survival rate, quality of life, changes of clinical symptoms and peripheral blood figures in the patients were observed.Results: After treatment, the rate of CR+PR+NC in the two groups was 88.89% and 68.89% respectively, the inter-group comparison showed a significant difference (P<0.05). The 0.5-, 1- and 2-year survival rate in Group A was 75.56%, 55.56% and 48.89% respectively and in Group B 71.11%, 46.67% and 24.44% respectively. The 2-year survival rate in the former was better than that in the latter (P<0.05). Moreover, the improvement of clinical symptoms, Karnofsky scoring, body weight and peripheral blood figure in Group A was superior to those in Group B.Conclusion: Therapeutic effect of BAC could be enhanced by combining it with CHM. 展开更多
