Aim: To analyze data from patients who underwent hepatectomy for hepatocellular adenoma (HCA) in Rio de Janeiro, Brazil. Methods: From January 2005 to March 2019, sixty-nine patients with HCA underwent hepatectomy at centers in Rio de Janeiro. They were included in the analysis patients undergoing hepatectomy with pathological diagnosis of hepatocellular adenoma and excluded patients with hepatectomy with anatomopathological diagnosis other than hepatocellular adenoma, mainly nodular focal hyperplasia. Data related to patients, tumor and surgery were analyzed retrospectively. Results: Sixty patients (87%) were female and nine were male. Among women, 83% had a history of contraceptive use;among men, only one had an androgen intake history. Overall mean age was 36.4 years (15 - 49), with men older than women (33.9 ± 8.14 years vs. 40.4 ± 6.27 years;P = 0.02). Forty one patients reported abdominal pain, associated or not to other symptoms;32% had an episode of hemorrhage;28 were asymptomatic with an incidental radiological finding. In total, 45 patients presented only one lesion and overall mean size was 8.1 cm (2 - 31);tumors were larger among men (mean size 12.9 ± 9.86 cm vs. 7.7 ± 4.58 cm;P = 0.009). Twenty one surgeries were laparoscopic. Hepatocellular carcinoma (HCC) was identified in three specimens and the incidence was higher among men (22.2% vs. 1.6%;P = 0.042) and in tumors larger than 20 cm (66.6% vs. 0.02%). There was no perioperative mortality. Two of the three patients with HCC died with extrahepatic recurrence;the remaining patient is well after 36 months. Overall mean follow-up time was 14.2 months (2 - 76). Conclusion: Male patients with HCA were older and had larger tumors when compared to females. Incidence of HCC was higher among men and in lesions larger than 20 cm. Male patients with HCA should be treated more aggressively than females.展开更多

A cross-sectional, analytical study was conducted with 40 main dish-type preparations, 20 of which are omnivorous and 20 vegans, served at lunch, to assess energy and nutritional densities, carbon, water and ecological footprints and cost of preparations. We propose a healthy and sustainable preparation index (HSPI) to evaluate from the list, the best preparation options considering the nutritional quality combined with the impact that the food causes on the environment. Preparations with the highest HSPI were considered the best options because they have a good relation between the nutritional profile and the environmental impact. Results: Regarding energy (ED), nutritional (ND) densities and water, carbon and ecological footprints, omnivorous preparations presented much higher values when compared to the vegan ones. The omnivorous preparations had the highest average cost (R$ 3.44). Regarding the HSPI, vegan preparations showed better rates than omnivorous preparations. Food services should promote healthy and sustainable choices by offering menus with low energy density preparations, high nutritional density, and low environmental impact, considering local realities and customer needs. Conclusion: This study was able to evaluate the best preparation options, considering the nutritional profile and the food impact on the environment, using health and sustainable indicators. Obtaining indicators of preparations regarding healthiness and sustainability, in practice, translates environmental aspects in menu planning, which contribute to changes in food consumption patterns in food services, in addition to contributing to the reduction of the environmental impact. In this way, they can be used as tools added to the menu planning process for the analysis of the environmental impact of menus, in addition to nutritional and qualitative aspects.展开更多

The updated program includes dynamic analysis, which incorporates inertial and damping effects, time-dependent load conditions, and a transient solver with multiple time integration schemes. The software assumes small displacements and linear-elastic material behavior. The paper briefly explains the theoretical basis for these developments and the reorganization of the source code using Object-Oriented Programming (OOP). The updated Graphical User Interface (GUI) allows interactive use of dynamic analysis features and displays new results such as animations, envelope diagrams of internal forces, phase portraits, and the response of degrees-of-freedom in time and frequency domain. The new version was used in a structural dynamics course, and new assignments were elaborated to improve students’ understanding of the subject.展开更多

GTN presents different degrees of proliferation, invasion and dissemination, but, if treated in reference centers, has high cure rates, even in multi-metastatic cases. The diagnosis of GTN following a hydatidiform molar pregnancy is made according to the International Federation of Gynecology and Obstetrics(FIGO)2000 criteria: four or more plateaued human chorionic gonadotropin(hCG)concentrations over three weeks; rise in hCG for three consecutive weekly measurements over at least a period of 2 weeks or more; and an elevated but falling hCG concentrations six or more months after molar evacuation. However, the latter reason for treatment is no longer used by many centers. In addition,GTN is diagnosed with a pathological diagnosis of CCA or PSTT/ETT. For staging after a molar pregnancy, FIGO recommends pelvic-transvaginal Doppler ultrasound and chest X-ray. In cases of pulmonary metastases with more than 1cm, the screening should be complemented with chest computed tomography and brain magnetic resonance image. Single agent chemotherapy, usually Methotrexate(MTX) or Actinomycin-D(Act-D), can cure about 70% of patients with FIGO/World Health Organization(WHO) prognosis risk score ≤ 6(low risk), reserving multiple agent chemotherapy, such as EMA/CO(Etoposide,MTX, Act-D, Cyclophosphamide and Oncovin) for cases with FIGO/WHO prognosis risk score ≥ 7(high risk) that is often metastatic. Best overall cure rates for low and high risk disease is close to 100% and > 95%, respectively. The management of PSTT/ETT differs and cure rates tend to be a bit lower. The early diagnosis of this disease and the appropriate treatment avoid maternal death, allow the healing and maintenance of the reproductive potential of these women.展开更多

Animals in the control group received an AIN-93 M diet, and animals in the experimental group received an Atkins-based diet(59.46% protein, 31.77% fat, and 8.77% carbohydrate). After 8 wk, the rats were anesthetized and exsanguinated for transaminases analysis, and their livers were removed for flow cytometry, immunohistochemistry, and light microscopy studies. We expressed the data as mean ± standard deviation(sd) assuming unpaired and parametric data; we analyzed differences using the student's t-test. statistical significance was set at P < 0.05.RESULTS: We found that plasma alanine aminotransferase and aspartate aminotransferase levels were significantly higher in the experimental group than in the control group. According to flow cytometry, the percentages of nonviable cells were 11.67% ± 1.12% for early apoptosis, 12.07% ± 1.11% for late apoptosis, and 7.11% ± 0.44% for non-apoptotic death in the experimental diet group and 3.73% ± 0.50% for early apoptosis, 5.67% ± 0.72% for late apoptosis, and 3.82% ± 0.28% for non-apoptotic death in the control diet group. The mean percentage of early apoptosis was higher in the experimental diet group than in the control diet group. Immunohistochemistry for autophagy was negative in both groups. sinusoidal dilation around the central vein and small hepatocytes was only observed in the experimental diet group, and fibrosis was not identified by hematoxylin-eosin or Trichrome Masson staining in either group.CONCLUSION: Eight weeks of an experimental diet resulted in cellular and histopathological lesions in rat livers. Apoptosis was our principal finding; elevated plasma transaminases demonstrate hepatic lesions.展开更多

The composite material was produced from a mixture of sand, cement, and water. Sisal fibers were added to the mixture in different lengths. Mechanical characterization of both the composite and the plain mortar was carried out using three point bend, compression, and impact tests. Specimens containing notches of different root radii were loaded in three point bending in an effort to determine the effect of the fibers on the fracture toughness of the material. The results obtained indicate that, while fiber reinforcement leads to a decrease in compressive strength, J-integral calculations at maximum load for the different notch root radii have indicated, particularly for the case of long fibers,a significant superiority of the reinforced material in comparison with the plain cement mortar, in consistence with the impact test data.展开更多

Unlike other wavelet families like Daubechies, Interpolets possess rational filter coefficients, are smooth, symmetric and therefore more suitable for use in numerical methods. Expressions for stiffness and mass matrices are developed based on connection coefficients, which are inner products of basis functions and their derivatives. An example in 1-D was formulated using Central Difference and Newmark schemes for time differentiation. Encouraging results were obtained even for large time steps. Results obtained in 2-D are compared with the standard Finite Difference Method for validation.展开更多

As it is known, when current pulses are imposed in their windings, high ripple torque is obtained. In order to reduce this ripple, a control strategy with modified current shapes is proposed. A workbench consisting of a machine prototype and the control system based on a microcontroller was built. These controllers were: a conventional PID, a fuzzy logic PID and a neural PID type. From experimental results, the effective reduction of the torque ripple was confirmed and the performance of the controllers was compared.展开更多

The methodology of the Transboundary Waters Assessment Programme for river basins proposed by the United Nations was used to evaluate risks within current and future scenarios. The results obtained allow the inference that the River Plate basin is in a moderate-risk category. However, high and very high risk values were observed for the pollution, dams and economic reliance indicators, which in the future horizon could promote conflicts regarding water resource use. In conclusion, states should establish their priorities and clear rules for monitoring, use and surveillance of water. This process should be done in an integrated manner, with due regard for international arrangements, and should be compatible with the policies and management instruments of the countries involved.展开更多

However current misconceptions about these biomaterials, their use and risks may compromise their correct application and development. To unveil these misconceptions, this work briefly reviewed concepts about bone remodeling, grafts classification and manufacturing processes, with a special focus on calcium phosphate materials as an example of a current employed biomaterial. Thus a search on the last decade was performed in Medline, LILACS, Scielo and other scientific electronic libraries using as keywords biomaterials, bone remodeling, regeneration, biocompatible materials, hydroxyapatite and therapeutic risks. Our search showed not only an accelerated biotechnological development that brought significant advances to biomaterials use on bone remodeling treatments but also several therapeutic risks that should not be ignored. The biomaterials specificity and limitations to clinical application point to the current need for developing safer products with better interactions with the biological microenvironments.展开更多

This is an ill-posed problem in the sense that, if the solution exists, it does not depend continuously on g. Considering the existence of a solution u(x, ) ∈ H 2 (R) and using a wavelet Galerkin method with Meyer multiresolution analysis, we regularize the ill-posedness of the problem. Furthermore we prove the uniqueness of the solution for this problem.展开更多

文摘Diabetes is a lifelong disease characterized by glucose metabolic imbalance,in which low insulin levels or impaired insulin signaling lead to hyperglycemic state.Within 20 years of diabetes progression,95%of patients will have diabetic retinopathy, the leading cause of visual defects in working-age people worldwide.Although diabetes is considered a microvascular disease, recent studies have shown that neurodegeneration precedes vascular changes within the diabetic visual system, albeit its mechanisms are still under investigation.Neuroinflammation and oxidative stress are intrinsically related phenomena, since macrophage/microglia and astrocytes are the main sources of reactive oxygen species during central nervous system chronic degenerative diseases, and both pathological processes are increased in the visual system during diabetes. The present review will focus on recent findings of the contribution of oxidative stress derived from neuroinflammation in the early neurodegenerative aspects of the diabetic visual system and their relationship with galectin-3.
文摘Managing inflammatory bowel disease(IBD)during the coronavirus disease 2019(COVID-19)pandemic has been a challenge faced by clinicians and their patients, especially concerning whether to proceed with biologics and immunosuppressive agents in the background of a global outbreak of a highly contagious new coronavirus(severe acute respiratory syndrome coronavirus 2,SARS-CoV-2).The knowledge about the impact of this virus on patients with IBD,although it is still scarce,is rapidly evolving. In particular, concerns surrounding medications’impact for IBD on the risk of acquiring SARS-CoV-2 infection or developing COVID-19,and potentially exacerbate viral replication and the COVID-19 course, are a current thinking of both practicing clinicians and providers caring for patients with IBD.Managing patients with IBD infected with SARS-CoV-2 depends on both the clinical activity of the IBD and the occasional development and severity of COVID-19.In this review,we summarize the current data regarding gastrointestinal involvement by SARS-CoV-2 and pharmacologic and surgical management for IBD concerning this infection, and the COVID-19 impact on both the patient's psychological functioning and endoscopy services, and we concisely summarize the telemedicine roles during the COVID-19 pandemic.
文摘AIM: To determine whether high-protein, high-fat, and low-carbohydrate diets can cause lesions in rat livers.METHODS: We randomly divided 20 female Wistar rats into a control diet group and an experimental diet group. Apoptosis was our principal finding; elevated plasma transaminases demonstrate hepatic lesions.
文摘This work aims at evaluating the mechanical behavior of sisal fiber reinforced cement mortar. The results obtained indicate that, while fiber reinforcement leads to a decrease in compressive strength, J-integral calculations at maximum load for the different notch root radii have indicated, particularly for the case of long fibers,a significant superiority of the reinforced material in comparison with the plain cement mortar, in consistence with the impact test data.

文摘The liver is a multifaceted organ;its location and detoxifying function expose this organ to countless injuries.Acute-on-chronic failure liver(ACLF)is a severe syndrome that affects the liver due to acute decompensation in patients with chronic liver disease. An infection environment, ascites, increased liver enzymes and prothrombin time, encephalopathy and fast-evolving multiorgan failure, leading to death, usually accompany this. The pathophysiology remains poorly understand. In this context, animal models become a very useful tool in this regard,as understanding;the disease may be helpful in developing novel therapeutic methodologies for ACLF.However, although animal models display several similarities to the human condition, they do not represent all ACLF manifestations, resulting in significant challenges. An initial liver cirrhosis framework followed by the induction of an acute decompensation by administering lipopolysaccharide and D-Ga IN,potentiating liver damage supports the methodologies applied to induce experimental ACLF.The entire methodology has been described mostly for rats.Nevertheless,a quick Pub Med database search indicates about 30 studies concerning ACFL models and over 1000 regarding acute liver failure models. These findings demonstrate the clear need to establish easily reproducible ACFL models to elucidate questions about this quickly established and often fatal syndrome.
文摘This paper presents the formulation of finite elements based on Deslauriers-Dubuc interpolating scaling functions, also known as Interpolets, for their use in wave propagation modeling. Results obtained in 2-D are compared with the standard Finite Difference Method for validation.

文摘Background:Animal models are widely used in scientific research in order to obtain information from a whole organism under a specific set of experimental conditions.Various lineages of mice have been used to investigate diseases and new therapeutic strategies, and, consequently, hematological and biochemical tests in these laboratory animals are essential to validate scientific studies. Our study seeks to establish reference values for hematological and biochemical parameters of four lineages of mice. Methods:We evaluated the hematological and biochemical profiles of 20 males and 20 females from the lineages Swiss(heterogeneous),BALB/c and C57BL/6(isogenic),and B6D2F1(hybrid),totaling 160 mice.Analysis were standardized using the systems pocH-100iV Diff™for 19 hematological parameters and VITROS®350 for 12 biochemical parameters. Results:Results are shown as means and standard deviation, grouped by lineage and genre.Comparing the values obtained in this study with the values from previous studies, some variations were detected, which could be explained by differences in methodologies or individual variability. Conclusion:Thus our study shows that knowledge and disclosure of the values of physiological parameters of laboratory animals is necessary, and emphasises the importance of considering variations influenced by gender, lineage and genotype in the choice of the best experimental model.
文摘Three speed controllers for an axial magnetic flux switched reluctance motor with only one stator, are described and experimentally tested. From experimental results, the effective reduction of the torque ripple was confirmed and the performance of the controllers was compared.

文摘Scaffold-free techniques in the developmental tissue engineering area are designed to mimic in vivo embryonic processes with the aim of biofabricating,in vitro, tissues with more authentic properties.Cell clusters called spheroids are the basis for scaffold-free tissue engineering. In this review,we explore the use of spheroids from adult mesenchymal stem/stromal cells as a model in the developmental engineering area in order to mimic the developmental stages of cartilage and bone tissues.Spheroids from adult mesenchymal stromal/stem cells lineages recapitulate crucial events in bone and cartilage formation during embryogenesis, and are capable of spontaneously fusing to other spheroids, making them ideal building blocks for bone and cartilage tissue engineering. Here,we discuss data from ours and other labs on the use of adipose stromal/stem cell spheroids in chondrogenesis and osteogenesis in vitro.Overall, recent studies support the notion that spheroids are ideal"building blocks"for tissue engineering by“bottom-up”approaches,which are based on tissue assembly by advanced techniques such as three-dimensional bioprinting.Further studies on the cellular and molecular mechanisms that orchestrate spheroid fusion are now crucial to support continued development of bottom-up tissue engineering approaches such as three-dimensional bioprinting.
文摘The present article sought to address the issue of shared management of water resources among sovereign states in Latin America, focusing on the hydrographic basin of the River Plate. This process should be done in an integrated manner, with due regard for international arrangements, and should be compatible with the policies and management instruments of the countries involved.

文摘Presently, several different graft materials are employed in regenerative or corrective bone surgery. The biomaterials specificity and limitations to clinical application point to the current need for developing safer products with better interactions with the biological microenvironments.
文摘We consider the problem K(x)u xx = u tt , 0 < x < 1, t ≥ 0, with the boundary condition u(0,t) = g(t) ∈ L 2 (R) and u x (0, t ) = 0, where K(x) is continuous and 0 < α≤ K (x) < +∞. Furthermore we prove the uniqueness of the solution for this problem.
文摘In this paper, a geo-environmental diagnostic was implemented to classify susceptibility to desertification in southern Mozambique (Chicualacuala) and deliver responses to revert occurring land degradation. The process of environmental diagnostic is a useful approach to identify the very processes and phenomena belonging to the wide ranging concept of land degradation.
