Difficulties in the Management of Systemic Autoimmune Diseases in Saint-Louis Du Senegal through the Analysis of a Series of 70 Observations

The objective of this work is to identify the difficulti... <strong>Introduction:</strong> Systemic Autoimmune Diseases (SAID) long considered very rare in Africa are increasingly the subject of publications. The objective of this work is to identify the difficulties in the management of these pathologies in an internal medicine department in northern Senegal by analyzing the epidemiological, clinical-biological, therapeutic and evolutionary aspects of SAID. <strong>Methods:</strong> This was a descriptive cross-sectional study carried out in the internal medicine department of the Saint-Louis University Hospital Center. Included were all the files of patients followed in outpatient and/or hospitalization for autoimmune diseases according to the criteria of the American College of Rheumatology, during the period from January 2017 to December 2020. The data were analyzed using SPSS software version 21.0. As the study was descriptive, no statistical test was performed. <strong>Results: </strong>Out of 3800 patients, 70 presented SAID, <i>i.e. </i> a hospital prevalence of 1.8%. Polyarthritis was the first reason for consultation in 97% followed by skin manifestations in 8%. The patients had positive anti-nuclear autoantibodies in 88% of cases. Rheumatoid arthritis was the predominant condition (71%) followed by systemic lupus erythematosus (SLE) (15%) and undifferentiated autoimmune diseases in 10%. Eleven percent (11%) of patients had an associated autoimmune disease. Corticosteroids were used in the treatment of these conditions in 97% of cases and methotrexate was the most prescribed immunosuppressant (54%). Thirty-two percent (32%) of patients are lost to follow-up. <strong>Conclusion:</strong> SAID are diverse and under diagnosed;they are characterized by diagnostic delay above all linked to access to specialists and sometimes to the high cost of paraclinical examinations, in particular immunology. Treatment remains based primarily on corticosteroid therapy and conventional immunosuppressants in the face of the unavailability of biotherapies. 展开更多

Li-ion batteries (Libs) are a mature technology widely used for energy storage in various electronic devices. Nowadays, this technology has become a leading candidate for the portable electronics market and for electric vehicles due to its good performance. As a result, the demand for Libs containing critical metals, rare earth elements and precious metals is increasing day by day with the accelerated upgrades of consumer electronics, which promotes the supply risk of many mining resources. In addition, the problems associated with the production of end-of-life Lib are increasing on a global scale. Used Libs are e-waste containing significant levels of critical raw materials (such as Co, Li, Mn and Ni) along with harmful substances. Without proper management of Lib waste, these precious metals and toxic substances may end up in nature and cause environmental and public health problems. In order to preserve nature, ensure sustainable resource management and stimulate the circular economy, it has become crucial to properly manage and recycle end-of-life Li-ion batteries. By the way, conventional methods focusing on pyrometallurgical treatments combined with hydrometallurgical treatment are widely studied to recover design metals from Libs waste. It is in this context that we have conducted this bibliographic synthesis, focusing on the efficiency of the solvents employed and their competitiveness for a more environmentally friendly economic management. In this manuscript, recent leaching, solvent extraction, electrodeposition and precipitation strategies to recover precious metals from end-of-life Li-ion battery designs are reviewed and the evolution of these processes is discussed. 展开更多

Introduction: Unclassified inflammatory rheumatism is persistent inflammatory arthralgias with or without synovitis without sufficient classification criteria for an inflammatory rheumatism or a well-defined connective disease. Their outcome is variable and has been little studied in Africa. We report the epidemiological, paraclinical and evolutionary characteristics of seven cases of indeterminate polyarthritis. Material and Method: From January 2012 to May 2021, we selected all the files of patients followed up for unclassified inflammatory rheumatism and in whom a precise diagnosis was retained during the course of the disease. The diagnosis of unclassified inflammatory rheumatism was retained after biological, immunological and radiological explorations without specificities. Our study took place in the Internal Medicine Department of Aristide Le Dantec Hospital (HALD). Results: Seven cases out of 274 consulted were collected. They consisted of seven women with an average age of 39 years at the time of the first consultation (extremes: 19 and 67 years). All seven patients presented with inflammatory polyarthralgia or non-deforming, non-erosive peripheral polyarthritis, without extra-articular manifestations. A biological inflammatory syndrome was present in all seven patients. Autoantibodies (rheumatoid factor, ANA, anti-ENA and ACPA) were negative in all patients. The first-line treatments in all seven cases were hydroxychloroquine (200 - 400 mg/day) and prednisone (5 - 10 mg/day). Methotrexate was added in 3 cases. During follow-up, 2 cases progressed to Polyarthritis Rheumatoid (PR) after 3 and 4 years. Two cases progressed to Sjögren’s syndrome after 4 and 5 years. Two patients progressed to systemic scleroderma after 5 and 8 years. One patient progressed to lupus after 5 years. Conclusion: Regular follow-up is essential in IAN. It allows the early diagnosis of IJR or a well-differentiated connective tissue disease and ensures adequate management, especially early. 展开更多
