As an epidemic disease,obesity is on the rise globally,seriously jeopardizing human health.China now has the largest overweight or obese population in the world.In addition to causing the typical physical symptoms of ...As an epidemic disease,obesity is on the rise globally,seriously jeopardizing human health.China now has the largest overweight or obese population in the world.In addition to causing the typical physical symptoms of obesity,obesity is often accompanied by a variety of psychosomatic diseases,which are not only closely related to health behaviors,but also psychosocial plays an important role in them.Although there are differences in the symptoms exhibited by the major systems,most of the modern research has been interpreted from the inflammatory and immune responses,which provides a direction to find the mechanism of obesity physical and mental diseases.After researching and explaining the mechanism,we give the countermeasures of Chinese medicine in treating obesity from both pharmacological and non-pharmacological treatments and explore the mechanism,hoping to build a bridge between Chinese medicine treatment and the mechanism of the above mentioned physical and mental diseases,and to provide scientific evidence for the efficacy of Chinese medicine.展开更多

This article reviews the recent data on the neural mechanism of acupuncture on GI movements. The mobile application function is divided into two directions: assisting acupoint positioning and perfecting acupoint knowledge system. The study does a relative review on Android and IOS, showing that the number of Android users is rather more prevailing than IOS. It suggests that intelligent acupuncture mobile applications should comply with the trend of acupuncture globalization in the future, developing multilingual versions. Further, the content should be characterized by the experience of different famous doctors to avoid homogenization. Technically, intelligent applications should continue to develop three-dimensional and augmented reality technology, to optimize the accuracy of acupoint positioning.展开更多

The automatic nervous system plays a main role in EA, but few studies exist on how vagovagal and sympathetic reflexes affect EA to regulate gastrointestinal motility.AIM To study the role of vagovagal and sympathetic reflexes in EA at ST36, as well as the associated receptor subtypes that are involved.METHODS Gastric motility was measured with a manometric balloon placed in the gastric antrum area in anesthetized animals. The peripheral nervous discharge was measured using a platinum electrode hooking the vagus or greater splanchnic nerve, and the central nervous discharge was measured with a glass microelectrode in the dorsal motor nucleus of the vagus(DMV). The effects and mechanisms of EA at ST36 were explored in male Sprague-Dawley rats which were divided in to a control group, vagotomy group, sympathectomy group, and microinjection group [including an artificial cerebrospinal fluid group, glutamate(L-Glu) group, and γ-aminobutyric acid(GABA) group] and in genetically modified male mice [β1β2 receptor-knockout(β1β2^(-/-)) mice, M2M3 receptorknockout(M2M3^(-/-)) mice, and wild-type control mice].RESULTS EA at ST36 promoted gastric motility during 30-120 s. During EA, both vagus and sympathetic nerve discharges increased, with a much higher frequency of vagus nerve discharge than sympathetic discharge. The gastric motility mediated by EA at ST36 was interdicted by vagotomy. However, gastric motility mediated by EA at ST36 was increased during 0-120 s by sympathectomy, which eliminated the delay effect of EA during 0-30 s, but it was lower than the control group during 30-120 s. Using gene knockout mice and their wild-type controls to explore the receptor mechanisms, we found that EA at ST36 decreased gastric motility in M2/3^(-/-) mice, and promoted gastric motility in β1/2^(-/-) mice. Extracellular recordings showed that EA at ST36 increased spikes of the DMV. Microinjection of L-Glu into the DMV increased gastric motility, while EA at ST36 decreased gastric motility during 0-60 s, and promoted gastric motility during 60-120 s.Injection of GABA reduced or increased gastric motility, and reduced the promoting gastric motility effect of EA at ST36.CONCLUSION These data suggest that EA at ST36 modulates gastric motility via vagovagal and sympathetic reflexes mediated through M2/3 and β1/2 receptors, respectively.Sympathetic nerve activity mediated through β1/2 receptors is associated with an early delay in modulation of gastric motility by EA at ST36.展开更多
Type 2 diabetes mellitus(T2DM)is a chronic metabolic disorder characterized by hyperglycemia and insulin resistance.The global prevalence of T2DM has reached epidemic proportions,affecting approximately 463 million ad...Type 2 diabetes mellitus(T2DM)is a chronic metabolic disorder characterized by hyperglycemia and insulin resistance.The global prevalence of T2DM has reached epidemic proportions,affecting approximately 463 million adults worldwide in 2019.Current treatments for T2DM include lifestyle modifications,oral antidiabetic agents,and insulin therapy.However,these therapies may carry side effects and fail to achieve optimal glycemic control in some patients.Therefore,there is a growing interest in the role of gut microbiota and more gut-targeted therapies in the management of T2DM.The gut microbiota,which refers to the community of microorganisms that inhabit the human gut,has been shown to play a crucial role in the regulation of glucose metabolism and insulin sensitivity.Alterations in gut microbiota composition and diversity have been observed in T2DM patients,with a reduction in beneficial bacteria and an increase in pathogenic bacteria.This dysbiosis may contribute to the pathogenesis of the disease by promoting inflammation and impairing gut barrier function.Several gut-targeted therapies have been developed to modulate the gut microbiota and improve glycemic control in T2DM.One potential approach is the use of probio-tics,which are live microorganisms that confer health benefits to the host when administered in adequate amounts.Several randomized controlled trials have demonstrated that certain probiotics,such as Lactobacillus and Bifidobacterium species,can improve glycemic control and insulin sensitivity in T2DM patients.Mechanisms may include the production of short-chain fatty acids,the improvement of gut barrier function,and the reduction of inflammation.Another gut-targeted therapy is fecal microbiota transplantation(FMT),which involves the transfer of fecal material from a healthy donor to a recipient.FMT has been used successfully in the treatment of Clostridioides difficile infection and is now being investigated as a potential therapy for T2DM.A recent randomized controlled trial showed that FMT from lean donors improved glucose metabolism and insulin sensitivity in T2DM patients with obesity.However,FMT carries potential risks,including transmission of infectious agents and alterations in the recipient's gut microbiota that may be undesirable.In addition to probiotics and FMT,other gut-targeted therapies are being investigated for the management of T2DM,such as prebiotics,synbiotics,and postbiotics.Prebiotics are dietary fibers that promote the growth of beneficial gut bacteria,while synbiotics combine probiotics and prebiotics.Postbiotics refer to the metabolic products of probiotics that may have beneficial effects on the host.The NIH SPARC program,or the Stimulating Peripheral Activity to Relieve Conditions,is a research initiative aimed at developing new therapies for a variety of health conditions,including T2DM.The SPARC program focuses on using electrical stimulation to activate peripheral nerves and organs,in order to regulate glucose levels in the body.The goal of this approach is to develop targeted,non-invasive therapies that can help patients better manage their diabetes.One promising area of research within the SPARC program is the use of electrical stimulation to activate the vagus nerve,which plays an important role in regulating glucose metabolism.Studies have shown that vagus nerve stimulation can improve insulin sensitivity and lower blood glucose levels in patients with T2DM.Gut-targeted therapies,such as probiotics and FMT,have shown potential for improving glycemic control and insulin sensitivity in T2DM patients.However,further research is needed to determine the optimal dose,duration,and safety of these therapies.展开更多
Diabetic foot ulceration is a devastating complication of diabetes that is associated with infection,amputation,and death,and is affecting increasing numbers of patients with diabetes mellitus.The pathogenesis of foot...Diabetic foot ulceration is a devastating complication of diabetes that is associated with infection,amputation,and death,and is affecting increasing numbers of patients with diabetes mellitus.The pathogenesis of foot ulcers is complex,and different factors play major roles in different stages.The refractory nature of foot ulcer is reflected in that even after healing there is still a high recurrence rate and amputation rate,which means that management and nursing plans need to be considered carefully.The importance of establishment of measures for prevention and management of DFU has been emphasized.Therefore,a validated and appropriate DFU classification matching the progression is necessary for clinical diagnosis and management.In the first part of this review,we list several commonly used classification systems and describe their application conditions,scope,strengths,and limitations;in the second part,we briefly introduce the common risk factors for DFU,such as neuropathy,peripheral artery disease,foot deformities,diabetes complications,and obesity.Focusing on the relationship between the risk factors and DFU progression may facilitate prevention and timely management;in the last part,we emphasize the importance of preventive education,characterize several of the most frequently used management approaches,including glycemic control,exercise,offloading,and infection control,and call for taking into account and weighing the quality of life during the formulation of treatment plans.Multidisciplinary intervention and management of diabetic foot ulcers(DFUs)based on the effective and systematic combination of these three components will contribute to the prevention and treatment of DFUs,and improve their prognosis.展开更多

Acupuncture has been widely used for treating diseases since the ancient days in China,but the mechanism by which acupuncture exerts such powerful roles is unclear.Epigenetics,including DNA methylation,histone modific...Acupuncture has been widely used for treating diseases since the ancient days in China,but the mechanism by which acupuncture exerts such powerful roles is unclear.Epigenetics,including DNA methylation,histone modification,and post-transcriptional regulation of miRNAs,is the study of heritable changes in gene expression that do not include DNA sequence alterations.Epigenetics has become a new strategy for the basic and clinical research of acupuncture in the last decade.Some investigators have been trying to illustrate the mechanism of acupuncture from an epigenetics perspective,which has shed new lights on the mechanisms and applications of acupuncture.Moreover,the introduction of epigenetics into the regulatory mechanism in acupuncture treatment has provided more objective and scientific support for acupuncture theories and brought new opportunities for the improvement of acupuncture studies.In this paper,we reviewed the literatures that has demonstrated that acupuncture could directly or indirectly affect epigenetics,in order to highlight the progress of acupuncture studies correlated to epigenetic regulations.We do have to disclose that the current evidence in this review is not enough to cover all the complex interactions between multiple epigenetic modifications and their regulations.However,the up-to-date results can help us to better understand acupuncture’s clinical applications and laboratory research.展开更多
Objective:Moxibustion,a common therapy in traditional Chinese medicine,has potential benefits for treating decreased ovarian reserve(DOR).The present study investigates the protective effect of moxibustion in a rat mo...Objective:Moxibustion,a common therapy in traditional Chinese medicine,has potential benefits for treating decreased ovarian reserve(DOR).The present study investigates the protective effect of moxibustion in a rat model of DOR and explores the possible mechanisms.Methods:Sixty-four female Sprague-Dawley rats were randomly divided into four groups:control,DOR,moxibustion(MOX),and hormone replacement therapy(HRT).The DOR rat model was established by intragastric administration of 50 mg/kg Tripterygium glycoside suspension(TGS),once daily for 14 days.MOX and HRT treatments were given from the day TGS administration was initiated.The ovarian reserve function was evaluated by monitoring the estrus cycle,morphological changes in ovaries,levels of serum estradiol(E2),follicle-stimulating hormone(FSH),luteinizing hormone(LH),and anti-Mullerian hormone(AMH),pregnancy rate and embryo numbers.Terminal-deoxynucleotidyl transferase-mediated nickend-labeling staining was used to identify ovarian granulosa cell apoptosis,while the protein and m RNA expressions of Bax,B-cell lymphoma-2(Bcl-2),phosphatidylinositol 3-kinase(PI3 K)and protein kinase B(AKT)in ovarian tissues were examined by immunohistochemistry,Western blot and quantitative reverse transcription-polymerase chain reaction.Results:Compared with the DOR group,MOX improved the disordered estrous cycle,promoted follicular growth,reduced the number of atresia follicles,increased the concentrations of serum E2 and AMH,and decreased serum FSH and LH concentrations.More importantly,the pregnancy rate and embryo numbers in DOR rats were both upregulated in the MOX treatment group,compared to the untreated DOR model.Further,we found that the MOX group had reduced apoptosis of ovarian granulosa cells,increased Bcl-2 expression and reduced expression of Bax.Furthermore,the PI3 K/AKT signaling pathway was triggered by the moxibustion treatment.Conclusion:Moxibustion improved ovarian function and suppressed apoptosis of ovarian granulosa cells in a rat model of DOR induced by TGS,and the mechanism may involve the PI3 K/AKT signaling pathway.展开更多

文摘Type 2 diabetes mellitus(T2DM)is a chronic metabolic disorder characterized by hyperglycemia and insulin resistance.The global prevalence of T2DM has reached epidemic proportions,affecting approximately 463 million adults worldwide in 2019.Current treatments for T2DM include lifestyle modifications,oral antidiabetic agents,and insulin therapy.However,these therapies may carry side effects and fail to achieve optimal glycemic control in some patients.Therefore,there is a growing interest in the role of gut microbiota and more gut-targeted therapies in the management of T2DM.The gut microbiota,which refers to the community of microorganisms that inhabit the human gut,has been shown to play a crucial role in the regulation of glucose metabolism and insulin sensitivity.Alterations in gut microbiota composition and diversity have been observed in T2DM patients,with a reduction in beneficial bacteria and an increase in pathogenic bacteria.This dysbiosis may contribute to the pathogenesis of the disease by promoting inflammation and impairing gut barrier function.Several gut-targeted therapies have been developed to modulate the gut microbiota and improve glycemic control in T2DM.One potential approach is the use of probio-tics,which are live microorganisms that confer health benefits to the host when administered in adequate amounts.Several randomized controlled trials have demonstrated that certain probiotics,such as Lactobacillus and Bifidobacterium species,can improve glycemic control and insulin sensitivity in T2DM patients.Mechanisms may include the production of short-chain fatty acids,the improvement of gut barrier function,and the reduction of inflammation.Another gut-targeted therapy is fecal microbiota transplantation(FMT),which involves the transfer of fecal material from a healthy donor to a recipient.FMT has been used successfully in the treatment of Clostridioides difficile infection and is now being investigated as a potential therapy for T2DM.A recent randomized controlled trial showed that FMT from lean donors improved glucose metabolism and insulin sensitivity in T2DM patients with obesity.However,FMT carries potential risks,including transmission of infectious agents and alterations in the recipient's gut microbiota that may be undesirable.In addition to probiotics and FMT,other gut-targeted therapies are being investigated for the management of T2DM,such as prebiotics,synbiotics,and postbiotics.Prebiotics are dietary fibers that promote the growth of beneficial gut bacteria,while synbiotics combine probiotics and prebiotics.Postbiotics refer to the metabolic products of probiotics that may have beneficial effects on the host.The NIH SPARC program,or the Stimulating Peripheral Activity to Relieve Conditions,is a research initiative aimed at developing new therapies for a variety of health conditions,including T2DM.The SPARC program focuses on using electrical stimulation to activate peripheral nerves and organs,in order to regulate glucose levels in the body.The goal of this approach is to develop targeted,non-invasive therapies that can help patients better manage their diabetes.One promising area of research within the SPARC program is the use of electrical stimulation to activate the vagus nerve,which plays an important role in regulating glucose metabolism.Studies have shown that vagus nerve stimulation can improve insulin sensitivity and lower blood glucose levels in patients with T2DM.Gut-targeted therapies,such as probiotics and FMT,have shown potential for improving glycemic control and insulin sensitivity in T2DM patients.However,further research is needed to determine the optimal dose,duration,and safety of these therapies.

文摘Diabetic foot ulceration is a devastating complication of diabetes that is associated with infection,amputation,and death,and is affecting increasing numbers of patients with diabetes mellitus.The pathogenesis of foot ulcers is complex,and different factors play major roles in different stages.The refractory nature of foot ulcer is reflected in that even after healing there is still a high recurrence rate and amputation rate,which means that management and nursing plans need to be considered carefully.The importance of establishment of measures for prevention and management of DFU has been emphasized.Therefore,a validated and appropriate DFU classification matching the progression is necessary for clinical diagnosis and management.In the first part of this review,we list several commonly used classification systems and describe their application conditions,scope,strengths,and limitations;in the second part,we briefly introduce the common risk factors for DFU,such as neuropathy,peripheral artery disease,foot deformities,diabetes complications,and obesity.Focusing on the relationship between the risk factors and DFU progression may facilitate prevention and timely management;in the last part,we emphasize the importance of preventive education,characterize several of the most frequently used management approaches,including glycemic control,exercise,offloading,and infection control,and call for taking into account and weighing the quality of life during the formulation of treatment plans.Multidisciplinary intervention and management of diabetic foot ulcers(DFUs)based on the effective and systematic combination of these three components will contribute to the prevention and treatment of DFUs,and improve their prognosis.

文摘Hypertension is associated with at least 7.6 million annual deaths worldwide.While pharmacotherapy may provide good control for blood pressure,it sometimes induces adverse effects.Meanwhile,acupuncture has been used for the treatment of cardiovascular diseases,such as hypertension,coronary artery disease,and stroke,but its mechanisms of actions remain poorly understood.The efficacy of acupuncture depends on multiple constituent elements including acupoints,manipulation skills,and implementation programs,which are termed as acupuncture prescription.This review summarized the previous information of experimental use of acupuncture on animals including species,hypertension models,acupoints selection,acupoint location,stimulation protocols,and evaluation of effectiveness to provide useful guidance for researchers when performing acupuncture in animal experiments.

文摘Acupuncture has been widely used for treating diseases since the ancient days in China,but the mechanism by which acupuncture exerts such powerful roles is unclear.Epigenetics,including DNA methylation,histone modification,and post-transcriptional regulation of miRNAs,is the study of heritable changes in gene expression that do not include DNA sequence alterations.Epigenetics has become a new strategy for the basic and clinical research of acupuncture in the last decade.Some investigators have been trying to illustrate the mechanism of acupuncture from an epigenetics perspective,which has shed new lights on the mechanisms and applications of acupuncture.Moreover,the introduction of epigenetics into the regulatory mechanism in acupuncture treatment has provided more objective and scientific support for acupuncture theories and brought new opportunities for the improvement of acupuncture studies.In this paper,we reviewed the literatures that has demonstrated that acupuncture could directly or indirectly affect epigenetics,in order to highlight the progress of acupuncture studies correlated to epigenetic regulations.We do have to disclose that the current evidence in this review is not enough to cover all the complex interactions between multiple epigenetic modifications and their regulations.However,the up-to-date results can help us to better understand acupuncture’s clinical applications and laboratory research.

文摘Objective:Moxibustion,a common therapy in traditional Chinese medicine,has potential benefits for treating decreased ovarian reserve(DOR).The present study investigates the protective effect of moxibustion in a rat model of DOR and explores the possible mechanisms.Methods:Sixty-four female Sprague-Dawley rats were randomly divided into four groups:control,DOR,moxibustion(MOX),and hormone replacement therapy(HRT).The DOR rat model was established by intragastric administration of 50 mg/kg Tripterygium glycoside suspension(TGS),once daily for 14 days.MOX and HRT treatments were given from the day TGS administration was initiated.The ovarian reserve function was evaluated by monitoring the estrus cycle,morphological changes in ovaries,levels of serum estradiol(E2),follicle-stimulating hormone(FSH),luteinizing hormone(LH),and anti-Mullerian hormone(AMH),pregnancy rate and embryo numbers.Terminal-deoxynucleotidyl transferase-mediated nickend-labeling staining was used to identify ovarian granulosa cell apoptosis,while the protein and m RNA expressions of Bax,B-cell lymphoma-2(Bcl-2),phosphatidylinositol 3-kinase(PI3 K)and protein kinase B(AKT)in ovarian tissues were examined by immunohistochemistry,Western blot and quantitative reverse transcription-polymerase chain reaction.Results:Compared with the DOR group,MOX improved the disordered estrous cycle,promoted follicular growth,reduced the number of atresia follicles,increased the concentrations of serum E2 and AMH,and decreased serum FSH and LH concentrations.More importantly,the pregnancy rate and embryo numbers in DOR rats were both upregulated in the MOX treatment group,compared to the untreated DOR model.Further,we found that the MOX group had reduced apoptosis of ovarian granulosa cells,increased Bcl-2 expression and reduced expression of Bax.Furthermore,the PI3 K/AKT signaling pathway was triggered by the moxibustion treatment.Conclusion:Moxibustion improved ovarian function and suppressed apoptosis of ovarian granulosa cells in a rat model of DOR induced by TGS,and the mechanism may involve the PI3 K/AKT signaling pathway.
