In this editorial,we have commented on the article that has been published in the recent issue of World Journal of Clinical Cases.The authors have described a case of unilateral thyroid cyst and have opined that the a...In this editorial,we have commented on the article that has been published in the recent issue of World Journal of Clinical Cases.The authors have described a case of unilateral thyroid cyst and have opined that the acute onset of infection may be linked to diabetes mellitus(DM).We have focused on the role of nutrition in the association between DM and infection.Patients with DM are at a high risk of infection,which could also be attributed to nutrition-related factors.Nutritional interventions for patients with diabetes are mainly based on a low-calorie diet,which can be achieved by adhering to a low-carbohydrate diet.However,dietary fiber supplementation is recommended to maintain the diversity of the gut microbiota.Furthermore,high-quality protein can prevent the increased risk of infection due to malnutrition.Supplementation of vitamins C,vitamins A,vitamins D,and folic acid improves blood sugar control and facilitates immune regulation.Mineral deficiencies augment the risk of infection,but the relationship with diabetes is mostly U-shaped and a good intake should be maintained.展开更多

BACKGROUND The working environment of submarine crews is also very special.They are in a closed,high-temperature,high-noise,high-vibration and narrow working and living space for a long time,and they suffer from physi...BACKGROUND The working environment of submarine crews is also very special.They are in a closed,high-temperature,high-noise,high-vibration and narrow working and living space for a long time,and they suffer from physical discomfort caused by seasickness,which will affect the mental health of officers and soldiers.American psychologists have achieved positive results in psychological resilience training for officers and soldiers from the perspective of positive psychology.At present,there are few reports on the correlation between psychological resilience in the field of domestic research on submarine crew psychology,and it is necessary to conduct further research.METHODS A total of 121 soldiers working in a confined space of a large ship were randomly divided into an experimental group and a control group.The 50 soldiers in the experimental group were given a training course intervention,while the 71 soldiers in the control group did not receive any intervention measures.The Pittsburgh Sleep Quality Index,Psychological Resilience Scale,military Psycho-logical Stress Self-Assessment Questionnaire,and General Self-Efficacy Scale scores were compared before and 6 months after the intervention.RESULTS Under the positive psychological control intervention,except for sleep efficiency(P=0.05),the difference between the remaining dimensions of the Pittsburgh Sleep Quality Index scores and the total scores of the experimental group compared with the control group was statistically significant(P<0.05);the assessment of the psychological condition showed that,in addition to the Psychological Stress Self-assessment Questionnaire for Military Personnel scores(P=0.05),the scores of the Mental Toughness Scale(Dispositional Resilience Scale Resilience II)in the experimental group,General Self-Efficacy Scale scores were statistically significant(P<0.05)compared to pre-intervention.CONCLUSION Positive psychological intervention and control can improve the sleep state and psychological state of officers and soldiers working in confined space at sea.展开更多

BACKGROUND Specific pulmonary infection could seriously threaten the health of pilots and their companions.The consequences are serious.We investigated the clinical diagnosis,treatment,and medical identification of sp...BACKGROUND Specific pulmonary infection could seriously threaten the health of pilots and their companions.The consequences are serious.We investigated the clinical diagnosis,treatment,and medical identification of specific pulmonary infections in naval pilots.CASE SUMMARY We analyzed the medical waiver and clinical data of four pilots with specific pulmonary infections,who had accepted treatment at the Naval Medical Center of Chinese People’s Liberation Army between January 2020 and November 2021,including three cases of tuberculosis and one of cryptococcal pneumonia.All cases underwent a series of comprehensive treatment courses.Three cases successfully obtained medical waiver for flight after being cured,while one was grounded after reaching the maximum flight life after being cured.CONCLUSION Chest computed tomography scanning should be used instead of chest radiography in pilots’physical examination.Most pilots with specific pulmonary infection can be cured and return to flight.展开更多
BACKGROUND Recently,two naval pilots in a two-seat trainer jet were forced to eject urgently due to sudden mechanical failure during night-time training.They were both successfully rescued and sent to the hospital for...BACKGROUND Recently,two naval pilots in a two-seat trainer jet were forced to eject urgently due to sudden mechanical failure during night-time training.They were both successfully rescued and sent to the hospital for emergency treatment.In this study,we investigate their ejection injuries and recovery process.CASE SUMMARY We analyzed the clinical data of the traumatic condition and recovery process from ejection injuries of two pilots who ejected from a failed trainer jet and survived.After being successfully rescued and sent to the hospital,they were diagnosed with multiple ejection injuries,including eye trauma,limb bone and joint injury,rib and spine injury,and so on.Both cases underwent fluid replacement,acid suppression,nutritional support,hemostasis,bone metabolism improvement,phlegm elimination,psychological measurement,blood circulation promotion and detumescence,physical therapy,and external fixation with braces for 1 mo before being discharged from hospital.They then recuperated in a sanatorium for 2 mo,and the related laboratory tests and supplementary examinations show that they recovered from all the above injuries.After successfully passing the psychological test and physical examination,they returned to flight duty 3 mo after ejection.CONCLUSION The causes and conditions of ejection injury in the pilots were very complex.Although they finally recovered quickly and were released,it also serves as a reminder that attention should be paid to pilots’ejection and parachute training in order to significantly reduce ejection injury and improve the ejection success rate.In addition,air defense support personnel should strengthen search and rescue and on-site emergency measures,and locate and rescue pilots in distress as early as possible to reduce subsequent injuries.展开更多

In this editorial,we discuss the recent article by Zhao et al published in the World Journal of Diabetes,which highlights the importance of recognizing the risk indicators associated with diabetes mellitus(DM).Given t...In this editorial,we discuss the recent article by Zhao et al published in the World Journal of Diabetes,which highlights the importance of recognizing the risk indicators associated with diabetes mellitus(DM).Given the severe implications of healthcare-associated infections(HAIs)in hospitalized individuals-such as heightened mortality rates,prolonged hospitalizations,and increased costs-we focus on elucidating the connection between DM and nosocomial infections.Diabetic patients are susceptible to pathogenic bacterial invasion and subsequent infection,with some already harboring co-infections upon admission.Notably,DM is an important risk factor for nosocomial urinary tract infections and surgical site infections,which may indirectly affect the occurrence of nosocomial bloodstream infections,especially in patients with DM with poor glycemic control.Although evidence regarding the impact of DM on healthcare-associated pneumonias remains inconclusive,attention to this potential association is warranted.Hospitalized patients with DM should prioritize meticulous blood glucose management,adherence to standard operating procedures,hand hygiene practices,environmental disinfection,and rational use of drugs during hospitalization.Further studies are imperative to explore the main risk factors of HAIs in patients with DM,enabling the development of preventative measures and mitigating the occurrence of HAIs in these patients.展开更多

BACKGROUND The main pathophysiological basis of coronavirus disease 2019(COVID-19)causing respiratory failure is a cytokine storm and interleukin-6(IL-6)is an important component of the COVID-19 cytokine storm.As a sp...BACKGROUND The main pathophysiological basis of coronavirus disease 2019(COVID-19)causing respiratory failure is a cytokine storm and interleukin-6(IL-6)is an important component of the COVID-19 cytokine storm.As a specific antagonist of IL-6,tocilizumab may block the cytokine storm of COVID-19.The Diagnosis and Treatment Guidelines of New Coronavirus Pneumonia(7 th Edition)includes tocilizumab as a recommended drug for immunotherapy in severe and critical COVID-19 patients.However,the specific clinical efficacy of tocilizumab in the treatment of COVID-19 patients is worth studying.AIM To determine the clinical efficacy of tocilizumab in inhibiting the cytokine storm in COVID-19.METHODS In total,19 severe and critical COVID-19 patients were enrolled in this study,and were treated with tocilizumab in Optical Valley Campus of Hubei Maternal and Child Health Care Hospital from February 20 to March 31,2020.The imaging manifestations and clinical data before and after treatment were analyzed retrospectively,including routine peripheral venous blood tests,routine blood biochemical tests,coagulation test,C-reactive protein(CRP),IL-6,and arterial blood gas analysis.RESULTS Of the 19 patients in this group,13(68.4%)had significantly improved symptoms of COVID-19(5 patients were discharged directly and 8 patients were transferred after improvement)following treatment.One case was invalid,1 case was exacerbated,and 4 deaths(21.1%)were observed(all critical cases).The lymphocyte count,CRP,lactic acid,oxygenation index,fibrinogen(FIB)and IL-6 levels were significantly different in the improved group.CONCLUSION Tocilizumab treatment is effective against IL-6 in COVID-19 patients,but it does not completely inhibit the inflammation and cytokine storm in all patients with COVID-19.In the clinical treatment of COVID-19 patients,attention should be paid to the timing of drug administration and other adjuvant treatments.展开更多
BACKGROUND The novel coronavirus disease 2019(COVID-19)has spread widely around the world with strong infectivity,rapid mutation and a high mortality rate.Mechanical ventilation has been included in the Diagnosis and ...BACKGROUND The novel coronavirus disease 2019(COVID-19)has spread widely around the world with strong infectivity,rapid mutation and a high mortality rate.Mechanical ventilation has been included in the Diagnosis and Treatment Protocol for Novel Coronavirus Pneumonia(Trial Version 8)as an important treatment for severe and critical COVID-19 patients,but its clinical efficacy in COVID-19 patients is various.Therefore,it is necessary to study the influencing factors on the efficacy of mechanical ventilation in severe and critical COVID-19 patients.AIM The aim of this study was to determine the influencing factors on the efficacy of mechanical ventilation in severe and critical COVID-19 patients.METHODS A total of 27 severe and critical COVID-19 patients were enrolled in this study and treated with mechanical ventilation at the Optical Valley Campus of Hubei Maternal and Child Health Care Hospital(Wuhan,Hubei Province)from February 20,2020 to April 5,2020.According to the final treatment outcomes,the patients were divided into the“effective group”and“death group.”The clinical data of the two groups,such as the treatment process and final outcome,were retrospectively analyzed in order to determine the specific curative effects on the two groups and the reasons for the differences in such curative effects,as well as to explore the factors related to death.RESULTS This study enrolled 27 severe and critical COVID-19 patients,including 17 males(63.0%)and 10 females(37.0%).Their ages were 74.41±11.73-years-old,and 19 patients(70.4%)were over 70-years-old.Severe COVID-19 patients over 70-yearsold who were treated with mechanical ventilation died in 14 cases(82.4%);thus,this was the peak age.A total of 17 patients died of basic disease,16 of whom had more than two basic diseases.The basic diseases were hypertension,diabetes,and cardiovascular and cerebrovascular diseases.At the same time,13 patients(76.5%)died from an abnormal increase in blood glucose.Among them,eight had diabetes before contracting COVID-19 and five had a stress-induced increase in blood glucose after contracting COVID-19.Diabetic ketoacidosis occurred in one case.The use of tocilizumab may be a double-edged sword that carries a certain risk in clinical usage.Among the patients who died,16(94.1%)went into septic shock at the end.There were significant differences in the degree of infection,cardiac and renal function,and blood glucose between the death group and effective group.CONCLUSION Age,blood glucose,cardiac and renal function,and inflammatory reaction are important indicators of poor prognosis for mechanical ventilation in severe and critical COVID-19 patients.展开更多

BACKGROUND Colorectal cancer(CRC)is a highly prevalent malignancy worldwide,and new therapeutic targets urgently need to be found to prolong patient survival.5-methoxytryptophan(5-MTP)is a tryptophan metabolite found ...BACKGROUND Colorectal cancer(CRC)is a highly prevalent malignancy worldwide,and new therapeutic targets urgently need to be found to prolong patient survival.5-methoxytryptophan(5-MTP)is a tryptophan metabolite found in animals and humans.However,the effects of 5-MTP on proliferation and apoptosis of CRC cells are currently unknown.AIM To investigate the effects of 5-MTP on the proliferation,migration,invasion,and apoptosis abilities of CRC cells.Additionally,we seek to explore whether 5-MTP has the potential to be utilized as a drug for the treatment of CRC.METHODS In order to evaluate the effect of 5-MTP on CRC cells,a series of experiments were conducted for evaluation.Colony formation assay and Cell Counting Kit 8 assays were used to investigate the impact of 5-MTP on the proliferation of CRC cell lines.Cell cycle assays were employed to examine the effect of 5-MTP on cellular growth.In addition,we investigated the effects of 5-MTP on apoptosis and reactive oxygen species in HCT-116 cells.To obtain a deeper understanding of how 5-MTP affects CRC,we conducted a study to examine its influence on the PI3K/Akt signaling pathway in CRC cells.RESULTS This article showed that 5-MTP promoted apoptosis and cell cycle arrest and inhibited cell proliferation in CRC cells.In many articles,it has been reported that PI3K/Akt/FoxO3a signaling pathway is one of the most important signaling pathways involved in internal regulating cell proliferation and differentiation. Nevertheless, 5-MTP combined with PI3K/Akt/FoxO3a signaling pathway inhibitors significantly promotedapoptosis and cell cycle arrest and inhibited cell proliferation in CRC cells compared with 5-MTP alone in ourstudy.CONCLUSIONTherefore, there is strong evidence that 5-MTP can be used as an effective medicine for CRC treatment.展开更多
Purpose:The present study aimed to analyze the influence of muscle activation on lumbar injury under aspecific+Gz load.Methods:A hybrid finite element human body model with detailed lumbar anatomy and lumbar muscleact...Purpose:The present study aimed to analyze the influence of muscle activation on lumbar injury under aspecific+Gz load.Methods:A hybrid finite element human body model with detailed lumbar anatomy and lumbar muscleactivation capabilities was developed.Using the specific+Gz loading acceleration as input,the kinematicand biomechanical responses of the occupant's lower back were studied for both activated and deactivated states of the lumbar muscles.Results:The results indicated that activating the major lumbar muscles enhanced the stability of theoccupant's torso,which delayed the contact between the occupant's head and the headrest.Lumbarmuscle activation led to higher strain and stress output in the lumbar spine under+Gz load,such as themaximum Von Mises stress of the vertebrae and intervertebral discs increased by 177.9%and 161.8%,respectively,and the damage response index increased by 84.5%.Conclusion:In both simulations,the occupant's risk of lumbar injury does not exceed 10%probability.Therefore,the activation of muscles could provide good protection for maintaining the lumbar spine andreduce the effect of acceleration in vehicle travel direction.展开更多

The coronavirus disease 2019(COVID-19)pandemic caused by severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)is spreading at an alarming rate,and it has created an unprecedented health emergency threatening ten...The coronavirus disease 2019(COVID-19)pandemic caused by severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)is spreading at an alarming rate,and it has created an unprecedented health emergency threatening tens of millions of people worldwide.Previous studies have indicated that SARS-CoV-2 ribonucleic acid could be detected in the feces of patients even after smearnegative respiratory samples.However,demonstration of confirmed fecal-oral transmission has been difficult.Clinical studies have shown an incidence rate of gastrointestinal(GI)symptoms ranging from 2%to 79.1%in patients with COVID-19.They may precede or accompany respiratory symptoms.The most common GI symptoms included nausea,diarrhea,and abdominal pain.In addition,some patients also had liver injury,pancreatic damage,and even acute mesenteric ischemia/thrombosis.Although the incidence rates reported in different centers were quite different,the digestive system was the clinical component of the COVID-19 section.Studies have shown that angiotensinconverting enzyme 2,the receptor of SARS-CoV-2,was not only expressed in the lungs,but also in the upper esophagus,small intestine,liver,and colon.The possible mechanism of GI symptoms in COVID-19 patients may include direct viral invasion into target cells,dysregulation of angiotensin-converting enzyme 2,immune-mediated tissue injury,and gut dysbiosis caused by microbiota.Additionally,numerous experiences,guidelines,recommendations,and position statements were published or released by different organizations and societies worldwide to optimize the management practice of outpatients,inpatients,and endoscopy in the era of COVID-19.In this review,based on our previous work and relevant literature,we mainly discuss potential fecal-oral transmission,GI manifestations,abdominal imaging findings,relevant pathophysiological mechanisms,and infection control and prevention measures in the time of COVID-19.展开更多

In modern society,subarachnoid hemorrhage,mostly caused by intracranial aneurysm rupture,is accompanied by high disability and mortality rate,which has become a major threat to human health.Till now,the etiology of in...In modern society,subarachnoid hemorrhage,mostly caused by intracranial aneurysm rupture,is accompanied by high disability and mortality rate,which has become a major threat to human health.Till now,the etiology of intracranial aneurysm has not been entirely clarified.In recent years,more and more studies focus on the relationship between hemodynamics and intracranial aneurysm.Under the physiological condition,the mechanical force produced by the stable blood flow in the blood vessels keeps balance with the structure of the blood vessels.When the blood vessels are stimulated by the continuous abnormal blood flow,the functional structure of the blood vessels changes,which becomes the pathophysiological basis of the inflammation and atherosclerosis of the blood vessels and further promotes the occurrence and development of the intracranial aneurysm.This review will focus on the relationship between hemodynamics and intracranial aneurysms,will discuss the mechanism of occurrence and development of intracranial aneurysms,and will provide a new perspective for the research and treatment of intracranial aneurysms.展开更多

文摘In this editorial,we have commented on the article that has been published in the recent issue of World Journal of Clinical Cases.The authors have described a case of unilateral thyroid cyst and have opined that the acute onset of infection may be linked to diabetes mellitus(DM).We have focused on the role of nutrition in the association between DM and infection.Patients with DM are at a high risk of infection,which could also be attributed to nutrition-related factors.Nutritional interventions for patients with diabetes are mainly based on a low-calorie diet,which can be achieved by adhering to a low-carbohydrate diet.However,dietary fiber supplementation is recommended to maintain the diversity of the gut microbiota.Furthermore,high-quality protein can prevent the increased risk of infection due to malnutrition.Supplementation of vitamins C,vitamins A,vitamins D,and folic acid improves blood sugar control and facilitates immune regulation.Mineral deficiencies augment the risk of infection,but the relationship with diabetes is mostly U-shaped and a good intake should be maintained.

文摘The incidence of Crohn’s disease(CD)has increased in recent years,with most patients requiring intestinal resection.Complications after intestinal resection for CD can lead to poor prognosis and recurrence,among which infectious complic-ations are the most common.This study aimed to investigate the common risk factors,including medications,preoperative nutritional status,surgery-related factors,microorganisms,lesion location and type,and so forth,causing infectious complications after intestinal resection for CD,and to propose corresponding preventive measures.The findings provided guidance for identifying suscept-ibility factors and the early intervention and prevention of infectious complic-ations after intestinal resection for CD in clinical practice.
文摘BACKGROUND The working environment of submarine crews is also very special.They are in a closed,high-temperature,high-noise,high-vibration and narrow working and living space for a long time,and they suffer from physical discomfort caused by seasickness,which will affect the mental health of officers and soldiers.American psychologists have achieved positive results in psychological resilience training for officers and soldiers from the perspective of positive psychology.At present,there are few reports on the correlation between psychological resilience in the field of domestic research on submarine crew psychology,and it is necessary to conduct further research.METHODS A total of 121 soldiers working in a confined space of a large ship were randomly divided into an experimental group and a control group.The 50 soldiers in the experimental group were given a training course intervention,while the 71 soldiers in the control group did not receive any intervention measures.The Pittsburgh Sleep Quality Index,Psychological Resilience Scale,military Psycho-logical Stress Self-Assessment Questionnaire,and General Self-Efficacy Scale scores were compared before and 6 months after the intervention.RESULTS Under the positive psychological control intervention,except for sleep efficiency(P=0.05),the difference between the remaining dimensions of the Pittsburgh Sleep Quality Index scores and the total scores of the experimental group compared with the control group was statistically significant(P<0.05);the assessment of the psychological condition showed that,in addition to the Psychological Stress Self-assessment Questionnaire for Military Personnel scores(P=0.05),the scores of the Mental Toughness Scale(Dispositional Resilience Scale Resilience II)in the experimental group,General Self-Efficacy Scale scores were statistically significant(P<0.05)compared to pre-intervention.CONCLUSION Positive psychological intervention and control can improve the sleep state and psychological state of officers and soldiers working in confined space at sea.

文摘Routine pediatric vaccination is one of the most effective public health inter-ventions for the control of a number of fatal diseases.However,during the coronavirus disease 2019 pandemic,routine pediatric vaccination rates were severely affected by disruptions of health services and vaccine confidence issues.Governments and the United Nations have taken measures to re-establish routine pediatric vaccination,while additional efforts are needed to catch up and develop plans to ensure routine vaccination services for the future pandemics.

文摘BACKGROUND Specific pulmonary infection could seriously threaten the health of pilots and their companions.The consequences are serious.We investigated the clinical diagnosis,treatment,and medical identification of specific pulmonary infections in naval pilots.CASE SUMMARY We analyzed the medical waiver and clinical data of four pilots with specific pulmonary infections,who had accepted treatment at the Naval Medical Center of Chinese People’s Liberation Army between January 2020 and November 2021,including three cases of tuberculosis and one of cryptococcal pneumonia.All cases underwent a series of comprehensive treatment courses.Three cases successfully obtained medical waiver for flight after being cured,while one was grounded after reaching the maximum flight life after being cured.CONCLUSION Chest computed tomography scanning should be used instead of chest radiography in pilots’physical examination.Most pilots with specific pulmonary infection can be cured and return to flight.

文摘BACKGROUND Recently,two naval pilots in a two-seat trainer jet were forced to eject urgently due to sudden mechanical failure during night-time training.They were both successfully rescued and sent to the hospital for emergency treatment.In this study,we investigate their ejection injuries and recovery process.CASE SUMMARY We analyzed the clinical data of the traumatic condition and recovery process from ejection injuries of two pilots who ejected from a failed trainer jet and survived.After being successfully rescued and sent to the hospital,they were diagnosed with multiple ejection injuries,including eye trauma,limb bone and joint injury,rib and spine injury,and so on.Both cases underwent fluid replacement,acid suppression,nutritional support,hemostasis,bone metabolism improvement,phlegm elimination,psychological measurement,blood circulation promotion and detumescence,physical therapy,and external fixation with braces for 1 mo before being discharged from hospital.They then recuperated in a sanatorium for 2 mo,and the related laboratory tests and supplementary examinations show that they recovered from all the above injuries.After successfully passing the psychological test and physical examination,they returned to flight duty 3 mo after ejection.CONCLUSION The causes and conditions of ejection injury in the pilots were very complex.Although they finally recovered quickly and were released,it also serves as a reminder that attention should be paid to pilots’ejection and parachute training in order to significantly reduce ejection injury and improve the ejection success rate.In addition,air defense support personnel should strengthen search and rescue and on-site emergency measures,and locate and rescue pilots in distress as early as possible to reduce subsequent injuries.

文摘In this editorial,we discuss the recent article by Zhao et al published in the World Journal of Diabetes,which highlights the importance of recognizing the risk indicators associated with diabetes mellitus(DM).Given the severe implications of healthcare-associated infections(HAIs)in hospitalized individuals-such as heightened mortality rates,prolonged hospitalizations,and increased costs-we focus on elucidating the connection between DM and nosocomial infections.Diabetic patients are susceptible to pathogenic bacterial invasion and subsequent infection,with some already harboring co-infections upon admission.Notably,DM is an important risk factor for nosocomial urinary tract infections and surgical site infections,which may indirectly affect the occurrence of nosocomial bloodstream infections,especially in patients with DM with poor glycemic control.Although evidence regarding the impact of DM on healthcare-associated pneumonias remains inconclusive,attention to this potential association is warranted.Hospitalized patients with DM should prioritize meticulous blood glucose management,adherence to standard operating procedures,hand hygiene practices,environmental disinfection,and rational use of drugs during hospitalization.Further studies are imperative to explore the main risk factors of HAIs in patients with DM,enabling the development of preventative measures and mitigating the occurrence of HAIs in these patients.

文摘Severe acute respiratory syndrome coronavirus-2 is a highly contagious positive-sense,single-stranded RNA virus that has rapidly spread worldwide.As of December 17,2023,772838745 confirmed cases including 6988679 deaths have been reported globally.This virus primarily spreads through droplets,airborne transmission,and direct contact.Hospitals harbor a substantial number of confirmed coronavirus disease 2019(COVID-19)patients and asymptomatic carriers,accompanied by high population density and a larger susceptible population.These factors serve as potential triggers for nosocomial infections,posing a threat during the COVID-19 pandemic.Nosocomial infections occur to varying degrees across different countries worldwide,emphasizing the urgent need for a practical approach to prevent and control the intra-hospital spread of COVID-19.This study primarily concentrated on a novel strategy combining preventive measures with treatment for combating COVID-19 nosocomial infections.It suggests preventive methods,such as vaccination,disinfection,and training of heathcare personnel to curb viral infections.Additionally,it explored therapeutic strategies targeting cellular inflammatory factors and certain new medications for COVID-19 patients.These methods hold promise in rapidly and effectively preventing and controlling nosocomial infections during the COVID-19 pandemic and provide a reliable reference for adopting preventive measures in the future pandemic.
文摘BACKGROUND The main pathophysiological basis of coronavirus disease 2019(COVID-19)causing respiratory failure is a cytokine storm and interleukin-6(IL-6)is an important component of the COVID-19 cytokine storm.As a specific antagonist of IL-6,tocilizumab may block the cytokine storm of COVID-19.The Diagnosis and Treatment Guidelines of New Coronavirus Pneumonia(7 th Edition)includes tocilizumab as a recommended drug for immunotherapy in severe and critical COVID-19 patients.However,the specific clinical efficacy of tocilizumab in the treatment of COVID-19 patients is worth studying.AIM To determine the clinical efficacy of tocilizumab in inhibiting the cytokine storm in COVID-19.METHODS In total,19 severe and critical COVID-19 patients were enrolled in this study,and were treated with tocilizumab in Optical Valley Campus of Hubei Maternal and Child Health Care Hospital from February 20 to March 31,2020.The imaging manifestations and clinical data before and after treatment were analyzed retrospectively,including routine peripheral venous blood tests,routine blood biochemical tests,coagulation test,C-reactive protein(CRP),IL-6,and arterial blood gas analysis.RESULTS Of the 19 patients in this group,13(68.4%)had significantly improved symptoms of COVID-19(5 patients were discharged directly and 8 patients were transferred after improvement)following treatment.One case was invalid,1 case was exacerbated,and 4 deaths(21.1%)were observed(all critical cases).The lymphocyte count,CRP,lactic acid,oxygenation index,fibrinogen(FIB)and IL-6 levels were significantly different in the improved group.CONCLUSION Tocilizumab treatment is effective against IL-6 in COVID-19 patients,but it does not completely inhibit the inflammation and cytokine storm in all patients with COVID-19.In the clinical treatment of COVID-19 patients,attention should be paid to the timing of drug administration and other adjuvant treatments.
文摘BACKGROUND The novel coronavirus disease 2019(COVID-19)has spread widely around the world with strong infectivity,rapid mutation and a high mortality rate.Mechanical ventilation has been included in the Diagnosis and Treatment Protocol for Novel Coronavirus Pneumonia(Trial Version 8)as an important treatment for severe and critical COVID-19 patients,but its clinical efficacy in COVID-19 patients is various.Therefore,it is necessary to study the influencing factors on the efficacy of mechanical ventilation in severe and critical COVID-19 patients.AIM The aim of this study was to determine the influencing factors on the efficacy of mechanical ventilation in severe and critical COVID-19 patients.METHODS A total of 27 severe and critical COVID-19 patients were enrolled in this study and treated with mechanical ventilation at the Optical Valley Campus of Hubei Maternal and Child Health Care Hospital(Wuhan,Hubei Province)from February 20,2020 to April 5,2020.According to the final treatment outcomes,the patients were divided into the“effective group”and“death group.”The clinical data of the two groups,such as the treatment process and final outcome,were retrospectively analyzed in order to determine the specific curative effects on the two groups and the reasons for the differences in such curative effects,as well as to explore the factors related to death.RESULTS This study enrolled 27 severe and critical COVID-19 patients,including 17 males(63.0%)and 10 females(37.0%).Their ages were 74.41±11.73-years-old,and 19 patients(70.4%)were over 70-years-old.Severe COVID-19 patients over 70-yearsold who were treated with mechanical ventilation died in 14 cases(82.4%);thus,this was the peak age.A total of 17 patients died of basic disease,16 of whom had more than two basic diseases.The basic diseases were hypertension,diabetes,and cardiovascular and cerebrovascular diseases.At the same time,13 patients(76.5%)died from an abnormal increase in blood glucose.Among them,eight had diabetes before contracting COVID-19 and five had a stress-induced increase in blood glucose after contracting COVID-19.Diabetic ketoacidosis occurred in one case.The use of tocilizumab may be a double-edged sword that carries a certain risk in clinical usage.Among the patients who died,16(94.1%)went into septic shock at the end.There were significant differences in the degree of infection,cardiac and renal function,and blood glucose between the death group and effective group.CONCLUSION Age,blood glucose,cardiac and renal function,and inflammatory reaction are important indicators of poor prognosis for mechanical ventilation in severe and critical COVID-19 patients.

文摘AIM:To explore the effects of laser-activated remote phosphors(LARP)on visual function in guinea pigs.METHODS:Electroretinogram(ERG)of guinea pigs were observed after LARP irradiation at different frequencies and irradiation times.We evaluated the expression of rhodopsin,β-catenin,connexin36,calretinin,and calbindin in the retina of guinea pigs and measured the density of photoreceptor cells after high-frequency LARP irradiation.RESULTS:After LARP irradiation,the ERG results showed that the amplitude of the dark-adapted 3.0 b-wave of the model eye was lower than that of the control eye after high-frequency irradiation(P<0.05).The expression of rhodopsin,β-catenin,connexin36,calretinin,and calbindin in the retina of guinea pig declined.CONCLUSION:There is frequency cumulative damage effect on the retina that relates to LARP illumination frequency.This has significance for staff visual protection policies under LARP lighting conditions.
文摘Coronavirus disease 2019(COVID-19)caused by severe acute respiratory syndrome coronavirus-2 has become a worldwide public health crisis.Studies have demonstrated that diabetes and dyslipidaemia are common comorbidities and could be high-risk factors for severe COVID-19.Vitamin D,a group of fatsoluble compounds responsible for intestinal absorption of calcium,magnesium,and phosphate,has been widely used as a dietary supplement for the prevention and treatment of numerous diseases,including infectious and non-infectious diseases,due to its high cost-effectiveness;safety;tolerability;and anti-thrombotic,anti-inflammatory,antiviral,and immunomodulatory properties.In this letter to the editor,we mainly discuss the potential role of vitamin D in patients with diabetes,dyslipidaemia,and COVID-19.
文摘BACKGROUND Colorectal cancer(CRC)is a highly prevalent malignancy worldwide,and new therapeutic targets urgently need to be found to prolong patient survival.5-methoxytryptophan(5-MTP)is a tryptophan metabolite found in animals and humans.However,the effects of 5-MTP on proliferation and apoptosis of CRC cells are currently unknown.AIM To investigate the effects of 5-MTP on the proliferation,migration,invasion,and apoptosis abilities of CRC cells.Additionally,we seek to explore whether 5-MTP has the potential to be utilized as a drug for the treatment of CRC.METHODS In order to evaluate the effect of 5-MTP on CRC cells,a series of experiments were conducted for evaluation.Colony formation assay and Cell Counting Kit 8 assays were used to investigate the impact of 5-MTP on the proliferation of CRC cell lines.Cell cycle assays were employed to examine the effect of 5-MTP on cellular growth.In addition,we investigated the effects of 5-MTP on apoptosis and reactive oxygen species in HCT-116 cells.To obtain a deeper understanding of how 5-MTP affects CRC,we conducted a study to examine its influence on the PI3K/Akt signaling pathway in CRC cells.RESULTS This article showed that 5-MTP promoted apoptosis and cell cycle arrest and inhibited cell proliferation in CRC cells.In many articles,it has been reported that PI3K/Akt/FoxO3a signaling pathway is one of the most important signaling pathways involved in internal regulating cell proliferation and differentiation. Nevertheless, 5-MTP combined with PI3K/Akt/FoxO3a signaling pathway inhibitors significantly promotedapoptosis and cell cycle arrest and inhibited cell proliferation in CRC cells compared with 5-MTP alone in ourstudy.CONCLUSIONTherefore, there is strong evidence that 5-MTP can be used as an effective medicine for CRC treatment.

文摘Purpose:The present study aimed to analyze the influence of muscle activation on lumbar injury under aspecific+Gz load.Methods:A hybrid finite element human body model with detailed lumbar anatomy and lumbar muscleactivation capabilities was developed.Using the specific+Gz loading acceleration as input,the kinematicand biomechanical responses of the occupant's lower back were studied for both activated and deactivated states of the lumbar muscles.Results:The results indicated that activating the major lumbar muscles enhanced the stability of theoccupant's torso,which delayed the contact between the occupant's head and the headrest.Lumbarmuscle activation led to higher strain and stress output in the lumbar spine under+Gz load,such as themaximum Von Mises stress of the vertebrae and intervertebral discs increased by 177.9%and 161.8%,respectively,and the damage response index increased by 84.5%.Conclusion:In both simulations,the occupant's risk of lumbar injury does not exceed 10%probability.Therefore,the activation of muscles could provide good protection for maintaining the lumbar spine andreduce the effect of acceleration in vehicle travel direction.

文摘The coronavirus disease 2019(COVID-19)pandemic caused by severe acute respiratory syndrome coronavirus-2 has brought serious challenges for the medical field.Patients with COVID-19 usually have respiratory symptoms.However,liver dysfunction is not an uncommon presentation.Additionally,the degree of liver dysfunction is associated with the severity and prognosis of COVID-19.Prevention,diagnosis,and treatment of malnutrition should be routinely recommended in the management of patients with COVID-19,especially in those with liver dysfunction.Recently,a large number of studies have reported that nutrition therapy measures,including natural dietary supplements,vitamins,minerals and trace elements,and probiotics,might have potential hepatoprotective effects against COVID-19-related liver dysfunction via their antioxidant,antiviral,anti-inflammatory,and positive immunomodulatory effects.This review mainly focuses on the possible relationship between COVID-19 and liver dysfunction,nutritional and metabolic characteristics,nutritional status assessment,and nutrition therapy to provide a reference for the nutritionists while making evidence-based nutritional decisions during the COVID-19 pandemic.
文摘The diagonal inerter is integrated into a suspension vibration reduction system(SVRS).The dynamic model of the SVRS with diagonal inerter and damping is established.The dynamic model is of strong geometric nonlinearity.The retaining nonlinearity up to cubic terms is validated under impact excitation.The conditions omitting the static deformation are determined.The effects of the diagonal inerter on the vibration reduction performance of the SVRS are explored under impact and random excitations.The vibration reduction performance of the proposed SVRS with both diagonal inerter and damping is better than that of either the SVRS without them or the SVRS with the diagonal damping only.
文摘The coronavirus disease 2019(COVID-19)pandemic caused by severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)is spreading at an alarming rate,and it has created an unprecedented health emergency threatening tens of millions of people worldwide.Previous studies have indicated that SARS-CoV-2 ribonucleic acid could be detected in the feces of patients even after smearnegative respiratory samples.However,demonstration of confirmed fecal-oral transmission has been difficult.Clinical studies have shown an incidence rate of gastrointestinal(GI)symptoms ranging from 2%to 79.1%in patients with COVID-19.They may precede or accompany respiratory symptoms.The most common GI symptoms included nausea,diarrhea,and abdominal pain.In addition,some patients also had liver injury,pancreatic damage,and even acute mesenteric ischemia/thrombosis.Although the incidence rates reported in different centers were quite different,the digestive system was the clinical component of the COVID-19 section.Studies have shown that angiotensinconverting enzyme 2,the receptor of SARS-CoV-2,was not only expressed in the lungs,but also in the upper esophagus,small intestine,liver,and colon.The possible mechanism of GI symptoms in COVID-19 patients may include direct viral invasion into target cells,dysregulation of angiotensin-converting enzyme 2,immune-mediated tissue injury,and gut dysbiosis caused by microbiota.Additionally,numerous experiences,guidelines,recommendations,and position statements were published or released by different organizations and societies worldwide to optimize the management practice of outpatients,inpatients,and endoscopy in the era of COVID-19.In this review,based on our previous work and relevant literature,we mainly discuss potential fecal-oral transmission,GI manifestations,abdominal imaging findings,relevant pathophysiological mechanisms,and infection control and prevention measures in the time of COVID-19.

文摘Bone morphogenetic protein belongs to transcription growth factor superfamilyβ;bone morphogenetic protein signal pathway regulates cell proliferation,differentiation,and apoptosis among different tissues.Cerebrovascular system supplies sufficient oxygen and blood into brain to maintain its normal function.The disorder of cerebrovascular system will result into serious cerebrovascular diseases,which is gradually becoming a major threat to human health in modern society.In recent decades,many studies have revealed the underlying biology and mechanism of bone morphogenetic protein signal pathway played in cerebrovascular system.This review will discuss the relationship between the two aspects,aiming to provide new perspective for non-invasive treatment and basic research of cerebrovascular diseases.
