Boceprevir plus peginterferon/ribavirin for treatment ofchronic hepatitis C in Russia

AIM: to evaluate addition of boceprevir to peginterferon/ribavirin(PR) in Russian patients with chronic hepatitis C virus(HCV).METHODS: treatment-naive(t N) and treatmentexperienced(t E) patients(who had failed prior treatment with PR for ≥ 12 wk) with chronic HCV genotype 1 infection were enrolled in this placebocontrolled, double-blind study. All patients initially received PR for 4 wk. Patients randomized to control treatment then received PR for an additional 44 wk. t N patients randomized to triple therapy received boceprevir(800 mg three times daily) plus PR for 24 wk and then further therapy according to treatment week 8(t W8) HCV RNA levels. t E patients received boceprevir plus PR for 32 wk and then further therapy according to t W8 HCV RNA levels. treatment was discontinued for t N patients with detectable HCV RNA at t W24 and t E patients with detectable HCV RNA at t W12 because of futility. the primary efficacy end point was sustained virologic response(SVR) defined as undetectable HCV RNA 24 wk after completing all study therapy.RESULTS: SVR was 74.8% in the boceprevir plus PR arm compared with 46.2% in the control arm, with a stratification-adjusted treatment difference of 29.2%(95%CI: 16.4-41.5; P < 0.0001). Rates of SVR were higher in the boceprevir arm in both t N and t E patient groups(t N 78.4% vs 56.3%; t E 69.4% vs 30.0%). Within t E patients, the rates of SVR were higher with boceprevir plus PR compared with PR, regardless of treatment failure type(null responder, partial responder, and relapser). Most patients receiving boceprevir plus PR in both t N(86%) and t E(71%) populations were eligible for reduced treatment duration. Anemia was increased in patients receiving boceprevir plus PR vs PR alone(47.2% vs 24.4%); there was a corresponding increase in ribavirin dose reduction and erythropoietin use. Among patients receiving boceprevir plus PR, SVR rates were similar in patients with anemia(< 10 g/d L) and those without anemia(71.2% vs 77.4%).CONCLUSION: Regulatory approval has been obtained for boceprevir plus PR in Russian patients with HCV genotype 1 infection based on the results of this study. 展开更多

Next-generation cellular networks are expected to provide users with innovative gigabits and terabits per second speeds and achieve ultra-high reliability,availability,and ultra-low latency.The requirements of such networks are the main challenges that can be handled using a range of recent technologies,including multi-access edge computing(MEC),artificial intelligence(AI),millimeterwave communications(mmWave),and software-defined networking.Many aspects and design challenges associated with the MEC-based 5G/6G networks should be solved to ensure the required quality of service(QoS).This article considers developing a complex MEC structure for fifth and sixth-generation(5G/6G)cellular networks.Furthermore,we propose a seamless migration technique for complex edge computing structures.The developed migration scheme enables services to adapt to the required load on the radio channels.The proposed algorithm is analyzed for various use cases,and a test bench has been developed to emulate the operator’s infrastructure.The obtained results are introduced and discussed. 展开更多

In ophthalmological practice,eyelid reconstruction is often needed because of the presence of defects that arise after the excision of a tumor or after injuries.Various methods for reconstructing these defects have been previously described.However,it is important to understand the basic principles underlying these techniques and their advantages and disadvantages to ensure the choice of the optimal technique in a particular case.We have analyzed the recent literature on new methods or modifications of existing ones to provide a brief overview of the reconstructive methods of the century for comparison.We searched PubMed and CyberLeninka for articles on restoring eyelid defects reported in the literature over the past 10 years.Most techniques can be considered modified classical reconstruction methods.Postoperative complications included aesthetic defects,such as scarring of the upper or lower eyelid,trichiasis,entropion,and lagophthalmos.Surgeons continue to search for the best methods for complex reconstructive surgery to achieve good cosmetic and aesthetic outcomes. 展开更多

Chronic hepatitis B(CHB) is a widespread infectious disease with unfavorable outcomes and life-threatening consequences for patients, in spite of modern vaccination and antiviral treatment modalities. Cutting-edge experimental approaches have demonstrated key pathways that involve cross-talk between viral particles and host immune cells. All events, including penetration of hepatitis B virus(HBV) particles into host cells, establishing persistence, and chronization of CHB infection, and possibility of complete elimination of HBV particles are controlled by the immune system. Researchers have paid special attention to the replication capacity of HBV in host cells, which is associated with cellular changes that reflect presentation of viral antigens and variability of HBV antigen features. In addition, specific HBV proteins have an immune-modulating ability to initiate molecular mechanisms that "avoid" control by the immune system. The relationship between immunological shifts and chronic infection stages has been intensively studied since it was recognized that the immune system is a direct participant in the recurrent(cyclic) nature of CHB. Understanding the wide diversity of molecular pathways and the crosstalk between innate and adaptive immune system components will provide fresh insight into CHB immune pathogenesis and the possibilities of developing new treatment strategies for this disease. 展开更多

Determination of the causes and the effects of storage conditions on the quality of silo stored wheat(Triticum aestivum)in Zimbabwe 被引量:5

There are still cases of millers returning poor quality red wheat to the Zimbabwe Grain Marketing Board(GMB)and this has been an ongoing problem over the past few years.A larger amount of this wheat has discoloured and damaged embryos and it is discounted by millers because the germs are brittle and they crumble easily.There have been also many rejections of the red wheat particularly by major traders.Therefore there was an urgent need to investigate the causes and effects of storage conditions on the quality of silo-stored red wheat,since red wheat is one of human beings’main food supplies.A representative sample of 2.25 kg of red winter wheat was randomly collected from the common red winter wheat incoming to the Grain Marketing Board Depot for storage.This representative sample of 2.25 kg was used as the control sample and its test density was determined.The control sample was then finely ground and analysed for protein,moisture,ash,aflatoxins and falling number.The red winter wheat was then stored in six different silos for a period of 5 months,with each silo having different humidity and temperature conditions.Representative samples of 4.5 kg were randomly collected monthly from each silo during the storage period.The test densities of the representative samples were determined.These representative samples were then finely ground and analysed for protein,moisture,ash,aflatoxins,and falling number.The results of the red wheat in storage were then compared with those of the control sample and analysed by analysis of variance(ANOVA)at the 5%level of significance.Results obtained after data analysis suggest that there were significant differences in the protein content,moisture content and falling number of the wheat before and after storage.However,differences in test density,aflatoxin and ash contents of the wheat before and after storage were not statistically significant at the 5%level of significance.The deterioration in wheat quality was attributed to the high storage tempera-ture and humidity conditions.It was also concluded that the optimum conditions for wheat storage are a temperature of 15 oC and a humidity of 60%. 展开更多

AIM To provide a clear understanding of viral hepatitis epidemiology and their clinical burdens in Somalia.METHODS A systematic review and meta-analysis was conducted as Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. A comprehensive literature search of published studies on viral hepatitis was performed from 1977-2016 in Pub Med, Google Scholar, Science Direct, World Health Organization African Index Medicus and the Africa Journals Online databases, as well as on the Ministry of Health website. We also captured unpublished articles that were not available on online systems.RESULTS Twenty-nine studies from Somalia and Somali immigrants(United Kingdom,United States,Italy,Libya)with a combined sample size for each type of viral hepatitis[hepatitis A virus(HAV):1564,hepatitis B virus(HBV):8756,hepatitis C virus(HCV):6257,hepatitis D virus(HDV):375 and hepatitis E virus(HEV):278]were analyzed.The overall pooled prevalence rate of HAV was 90.2%(95%CI:77.8%to 96%).The HAV prevalence among different age groups was as follows:<1 year old,61.54%(95%CI:40.14%to79.24%);1-10 years old,91.91%(95%CI:87.76%to94.73%);11-19 years old,96.31%(95%CI:92.84%to 98.14%);20-39 years old,91.3%(95%CI:83.07%to 95.73%);and>40 years old,86.96%(95%CI:75.68%to 93.47%).The overall pooled prevalence of HBV was 18.9%(95%CI:14%to 29%).The overall pooled prevalence among subgroups of HBV was20.5%(95%CI:5.1%to 55.4%)in pregnant women;5.7%(95%CI:2.7%to 11.5%)in children;39.2%(95%CI:33.4%to 45.4%)in patients with chronic liver disease,including hepatocellular carcinoma(HCC);7.7%(95%CI:4.2%to 13.6%),12.4%(95%CI:6.3%to 23.0%)and 11.8%(95%CI:5.3%to 24.5%)in age groups<20 years old,20-39 years old and>40years old,respectively.The HBV prevalence among risk groups was 20%(95%CI:7.19%to 44.64%)in female prostitutes,21.28%(95%CI:7.15%to48.69%)in hospitalized adults,5.56%(95%CI:0.99%to 25.62%)in hospitalized children,60%(95%CI:31.66%to 82.92%)in patients with acute hepatitis,33.55%(95%CI:14.44%to 60.16%)in patients with ancylostomiasis,12.34%(95%CI:7.24%to 20.26%)in patients with leprosy and 20.19%(95%CI:11.28%to33.49%)in schistosomiasis patients.The overall pooled prevalence of HCV was estimated as 4.84%(95%CI:3.02%to 7.67%).The prevalence rates among blood donors,risk groups,children and patients chronic liver disease(including HCC)was 0.87%(95%CI:0.33%to 2.30%),2.43%(95%CI:1.21%to 4.8%),1.37%(95%CI:0.76%to 2.46%)and 29.82%(95%CI:15.84%to 48.98%),respectively.The prevalence among genotypes of HCV was 21.9%(95%CI:15.36%to 30.23%)in genotype 1,0.87%(95%CI:0.12%to 5.9%)in genotype 2,25.21%(95%CI:18.23%to 33.77%)in genotype 3,46.24%(95%CI:37.48%to 55.25%)in genotype 4,2.52%(95%CI:0.82%to7.53%)in genotype 5,and 1.19%(95%CI:0.07%to16.38%)in genotype 6.The overall pooled prevalence of HDV was 28.99%(95%CI:16.38%to 45.96%).The HDV prevalence rate among patients with chronic liver disease,including HCC,was 43.77%(95%CI:35.09%to 52.84%).The overall pooled prevalence of HEV was46.86%(95%CI:5.31%to 93.28%).CONCLUSION Our study demonstrates a high prevalence of all forms of viral hepatitis in Somalia and it also indicates that chronic HBV was the commonest cause of chronic liver disease.This highlights needs for urgent public health interventions and strategic policy directions to controlling the burden of the disease. 展开更多

The HIV/AIDS epidemic has grown on an unprecedented scale since it was first recognized, and presently it is considered as a well known global public health crisis. In making the health policy where millions of populations are at risk, it is necessary to understand the knowledge level about HIV/AIDS. So, this study aimed at to investigate the knowledge level about HIV/AIDS among the villagers. Data and necessary information were collected from one hundred villagers of Salmanpur from Comilla district, Bangladesh through a structured questionnaire. The collected data were used for univariate analysis, to carry out the description of the variables;and to justify the total score of knowledge about HIV/AIDS among the respondents, Cronbach’s alpha (α) was calculated. The results revealed that almost all the respondents (92.00%) were ever heard about HIV/AIDS and half of them (51.00%) were known it as a communicable disease. Majority of the respondents (62.00%) believed that it is caused by heterosexual relation as well as sex with HIV positive persons. However, a good number of respondents were found unaware about the major causes and preventive ways of AIDS. Less than half of the respondents (45.00%) had medium knowledge (α = 0.34 - 0.66) and only 36.00% had the enough knowledge about HIV/AIDS. The knowledge level is increasing with the increase of education level, and the respondents who involved in agricultural works are found the lowest knowledge level (47.00%) about HIV/AIDS. The villagers were known about HIV/AIDS but they do not have accurate information about the ways in which AIDS virus can or cannot be transmitted. Therefore, effective programs should be taken to increase the knowledge level about HIV/AIDS among the villagers in Bangladesh. 展开更多

As the number of space objects(SO)increases,collision avoidance problem in the rendezvous tasks or reconstellation of satellites with SO has been paid more attention,and the dangerous area of a possible collision should be derived.In this paper,a maneuvering method is proposed for avoiding collision with a space debris object in the phasing orbit of the initial optimal solution.Accordingly,based on the plane of eccentricity vector components,relevant dangerous area which is bounded by two parallel lines is formulated.The axises of eccentricity vector system pass through the end of eccentricity vector of phasing orbit in the optimal solution,and orientation of axis depends on the latitude argument where a collision will occur.The dangerous area is represented especially with the graphical dialogue,and it allows to find a compromise between the SO avoiding and the fuel consumption reduction.The proposed method to solve the collision avoidance problem provides simplicity to calculate rendezvous maneuvers,and possibility to avoid collisions from several collisions or from“slow”collisions in a phasing orbit,when the protected spacecraft and the object fly dangerously close to each other for a long period. 展开更多

Lungs are a vital human body organ,and different Obstructive Lung Diseases(OLD)such as asthma,bronchitis,or lung cancer are caused by shortcomings within the lungs.Therefore,early diagnosis of OLD is crucial for such patients suffering from OLD since,after early diagnosis,breathing exercises and medical precautions can effectively improve their health state.A secure non-invasive early diagnosis of OLD is a primordial need,and in this context,digital image processing supported by Artificial Intelligence(AI)techniques is reliable and widely used in the medical field,especially for improving early disease diagnosis.Hence,this article presents an AIbased non-invasive and secured diagnosis for OLD using physiological and iris features.This research work implements different machine-learning-based techniques which classify various subjects,which are healthy and effective patients.The iris features include gray-level run-length matrix-based features,gray-level co-occurrence matrix,and statistical features.These features are extracted from iris images.Additionally,ten different classifiers and voting techniques,including hard and soft voting,are implemented and tested,and their performances are evaluated using several parameters,which are precision,accuracy,specificity,F-score,and sensitivity.Based on the statistical analysis,it is concluded that the proposed approach offers promising techniques for the non-invasive early diagnosis of OLD with an accuracy of 97.6%. 展开更多

Spectrophotometric titration of aqueous solutions of BNHА by salts of some transition metals allowed to calculate the composition and formation constants of the metal complexes. The crystal structure of Cu (BNHА)2 is studied by X-ray diffraction. The Cu atom is coordinated by four O atoms of two bidentate ligands, which close 5-membered chelate rings. The N-O (1.306 ? - 1.320 ?) and N-N (1.274?? and 1.275? ) bond lengths indicate that π electrons are delocalized over the chelating groups. Geophysical investigation for Groundwater which entails the use of vertical electrical sounding (VES) methods was carried out in parts of the Idemili area of Anambra State, Nigeria. Geologically the study area falls within the Nanka Sand, belonging to the Ameki Group in the Tertiary Niger Delta Basin. The investigation intends amongst others to assess the groundwater potential of the area, find out the existence of possible aquifer(s) within the area, and estimate the possible depth of a borehole. 15 VES stations were established at the site using Ohmega System Tetrameter and Schlumberger electrode configuration with current electrode spacing (AB/2) of 350 meters. The geologic parameters, such as the number of geo-electric layers, the thickness of the geo-electric units, and their resistivities were interpreted from the VES curves. A qualitative assessment of the curves shows that a model with 5 layers would sufficiently model the field curves. The thickness value ranged from 0.1 to 86 m, the resistivity ranged from 258 to 13,258 Ωm, and the depth ranged between 1.2 and 109 m. Quantitative interpretation of field curves involved inversion with appropriate parameters using Zond Res1D software. 展开更多

There are many similarities between the national growth of Brazil and Indonesia.Both countries are rich in natural resources,play a vital role in the establishment of regional blocs,are members of G-20 and adhere to the principles of preventive diplomacy.The objective of this article is to show the role of the concept of strategic partnerships in the foreign policy of Brazil,whose international influence in the UN and other global structures is actively growing,and also to identify the characteristics of such partnership with Indonesia.Southeast Asia is becoming extremely important for the international economic system,and Indonesia,located on 17,000 islands,has an important geostrategic position,since all major sea routes in the Indian and Pacific Ocean pass through it.Indonesia is currently distinguished thanks to its steady economic growth,political stability,integration into the global economy,and increased activity on international platforms.Consequently,it is no surprise that Brazilian-Indonesian strategic partnership has started to manifest itself not only in the development of comprehensive ties,but also in the cooperation in upholding the ideas of sustainable development,disarmament,as well as the peaceful resolution of conflicts on the global level.The authors utilized a problematic-chronological method that allowed to analyze the areas of contacts between Brazil and Indonesia in the global arena,to find the problems and promising areas of partnership—including the oil and gas sector,the exchange of technologies,the creation of favorable investment climate,the coordination of activity in international organizations,and willingness to take into consideration mutual interests. 展开更多

A case report presents a progression of autoimmune thyroiditis with an abnormal enlargement of the thyroid glands and increased thyreotropin hormone concentration-associated with interferon treatment in human papillomavirus infected patient with the autoimmune thyroiditis and a daily L-thyroxin hormone replacement therapy background. Observation was supplemented with a brief review of literature and discussion. On the basis of this observation and a brief review of literature authors suggested that the potential adverse effects of interferon therapy are overbalanced than its benefits for gynecological patients, therefore any interferon treatment should be recommended with strict indications as well as after screening of conditions and functions of thyroid glands and other interferon target organs to avoid interferon treatment side effects. Practitioners especially gynecologists should inform their patients about pleiotropic interferon effects and its high frequent and wide range side effects before to start such kind of treatment. 展开更多

Evaluation of Safety and Efficacy of the Maximus<sup>TM</sup>System for Facial Wrinkles

Introduction: There is a growing demand for non-invasive methods with no down time and minimal risk for facial skin rejuvenation and treatment of wrinkles. The Maximus system, based on TriLipo technology, combines radiofrequency and Dynamic Muscle Activation (DMA). We evaluated the safety and efficacy of the MaximusTM system for the treatment of facial wrinkles. Methods: Twenty women received 8 weekly treatments for facial wrinkles using the Maximus system. Treatment efficacy was assessed by comparing pre- and post-treatment photographs, assessing skin characteristics using the 3D Visioscan system, assessing skin microtopography using the DUB ultrasonic scanning system, assessing microcirculation conditions of facial skin using Laser Doppler Flowmetry (LDF) and subjective evaluation of skin improvement. Results: No adverse events were observed. Following the treatments, positive changes in skin flakiness (scaliness), roughness, smoothness and wrinkles were demonstrated using Visioscan. A thickening of the epidermal-dermal layer and increased structural homogeneity were observed by ultrasound, hyperechogenicity was increased and areas of hypoechogenicity reduced. Microcirculation was improved, corresponding with a positive trend for improved skin characteristics. Conclusions: The Maximus system powered by the TriLipo technology is a non-invasive, effective, safe, and virtually painless treatment for reduction of wrinkles and facial skin rejuvenation. 展开更多

The structure of the titled compound has been determined from single crystal X-ray diffraction data. The hydrated C7H5N2NaO52.5H2O crystallizes in the monoclinic space group C2/c, with a(?) 16.408(3);b(?) 12.446(3);c(?) 13.716(3);(o) 126.34(3). The planar organic anion exists in a triketo-dihydroxyimino form with the C–O and C–N distances from 1.220(2) to 1.271(2)?? and from 1.292(2) to 1.293?? respectively. In the IR spectrum of I, the sharp absorption band occurred at 1681 cm-1 due to C=O stretching indicating the strong H-interactions. The purpose of this study was to determine the thickness of skin and fat folds in Kyrgyz women of various ages, taking into account their somatotypological profile. Using the method of complex anthropometry, including the determination of the values of 21 anthropometric parameters, the physical status of 1028 Kyrgyz women of different age groups was studied youth (16 - 20 years old 310 girls), mature age (1st period, 21 - 35 years old 308 women;2nd period, 36 - 55 years 410 women) living in Osh, Kyrgyzstan. For somatotyping, we used the scheme of constitutional diagnostics. Seven somatotypes distinguished within three constitutional groups. The subcutaneous fat was measured by caliperometry. Statistical processing carried out using the statistical programs Microsoft Excel and the STATISTICA package (v. 6.0). To determine the reliability of the differences between the indicators, the Student’s test was used (p < 0.05). The results demonstrate that within each somatotype there are broad changes in the studied anthropometric indicator. Thus, the thickness of the skin and fat folds is the smallest in women of asthenic, athletic and stenoplastic (p < 0.05), and the largest in representatives of the euriplastic and pycnic somatotypes (p < 0.05). Thus, indicators of physical status in adolescence and adulthood in women have a pronounced somatotypological specificity. These materials on the physical development and constitutional and typological characteristics of the studied population of women applicable for a personalized approach in the context of a relative norm. 展开更多

The two-parametric family of two-connected abelian Lie groups is obtained. The necessity of existence of so-called imaginary charges and electromagnetic fields with negative energy density (dark photons) is derived. The possibilities when the overall Lagrangian represents a sum or difference of two identical Lagrangians for the visible and hidden sectors (i.e. copies of unbroken U(1)) are ruled out by the extended symmetry. The distinction between the two types of fields resides in the fact that for one of them current and electromagnetic kinetic terms in Lagrangians are identical in sign, whereas for another type these terms are opposite in sign. As a consequence, and in contrast to the common case, like imaginary charges attract and unlike charges repel. Some cosmological issues of the proposed hypothesis are discussed. Particles carrying imaginary charges are proposed as one of the components of dark matter. Such a matter would be imaginarily charged on a large scale for the reason that dark atoms carry non-compensated charges. It leads to important predictions for matter distribution, interaction and other physical properties being different from what is observed in dominant dark matter component in the standard model. These effects of imaginary charges depend on their density and could be distinguished in future observations. Dark electromagnetic fields can play crucial dynamical role in the very early universe as they may dominate in the past and violate weak energy condition which provides physical grounds for bouncing cosmological scenarios pouring a light on the problem of origin of the expanding matter flow. 展开更多

Cooper-Carbon Nanostructured Composite Coatings with Controlled Structure

Samples of the Cu-C composite coatings were produced by high-speed ion-plasma magnetron sputtering (HiPMS) with mosaic target (MT). Nanostructure, roughness, mechanical properties as well as electric properties of coatings depend on carbon-cooper ratio. So 10% - 15% cooper con centration (atomic) results in to dry friction coefficient decrease up to 0.1 and wear resistance increase up to 10<sup>-17</sup> m3/N·m. The HiPMS method allowed us to effectively sputter very different materials like carbon and cooper at the same velocity with required proportion. At the same time, one could obtain a uniform mixture of ingredients on the atomic level. The coating corresponds to a mixture of nano-sized phases of carbon and cooper. Crystalline dispersion of samples depends on carbon concentration. The obtained products are characterized by XRD, UV (257 nm) and visible (514 nm) Raman spectroscopy. The morphologies are studied with TEM and AFM. Mechanical properties were investigated with friction coefficient and micro hardness tests. There was no chemical interaction during Cu-C precipitation on hot substrate (625 - 725 K). The mechanical properties can be explained by free electrons capture with carbon nanostructures and space charge formation. 展开更多
