BACKGROUND Endoscopic rubber band ligation(ERBL)is a nonsurgical technique for the treatment of symptomatic internal hemorrhoids but is limited by recurrence and post-procedural pain.AIM To evaluate satisfaction,long-...BACKGROUND Endoscopic rubber band ligation(ERBL)is a nonsurgical technique for the treatment of symptomatic internal hemorrhoids but is limited by recurrence and post-procedural pain.AIM To evaluate satisfaction,long-term recurrence,and post-procedural pain in managing internal hemorrhoids using a combination of polidocanol foam sclerotherapy and ERBL.METHODS This was a prospective,multicenter,randomized study.A total of 195 consecutive patients diagnosed with grade II-III internal hemorrhoids were enrolled from four tertiary hospitals and randomly divided into a cap-assisted endoscopic polidocanol foam sclerobanding(EFSB)or an ERBL group.All patients were followed-up for 12 months.Symptom-based severity and post-procedural pain were assessed using a hemorrhoid severity score(HSS)and a visual analog scale(VAS).Continuous variables were reported as medians and interquartile range.RESULTS One hundred and ninety-five patients were enrolled,with 98 in the EFSB group.HSS was lower in the EFSB group than in the ERBL group at 8 weeks[4.0(3.0-5.0)vs 5.0(4.0-6.0),P=0.003]and 12-month[2.0(1.0-3.0)vs 3.0(2.0-3.0),P<0.001]of follow-up.The prolapse recurrence rate was lower in the EFSB group at 12 months(11.2%vs 21.6%,P=0.038).Multiple linear regression analysis demonstrated that EFSB treatment[B=-0.915,95%confidence interval(CI):−1.301 to−0.530,P=0.001]and rubber band number(B=0.843,95%CI:0.595-1.092,P<0.001)were negatively and independently associated with the VAS score 24 hours post-procedure.The median VAS was lower in the EFSB group than in the ERBL[2.0(1.0-3.0)vs 3.0(2.0-4.0),P<0.001].CONCLUSION Cap-assisted EFSB provided long-term satisfaction and effective relief from the recurrence of prolapse and pain 24 hours post-procedure.展开更多

BACKGROUND Perivascular epithelioid cell tumor(PEComa)represents a group of rare mesenchymal tumors.PEComa can occur in many organs but is rare in the colorectum,especially in children.Furthermore,PEComa is a rare cau...BACKGROUND Perivascular epithelioid cell tumor(PEComa)represents a group of rare mesenchymal tumors.PEComa can occur in many organs but is rare in the colorectum,especially in children.Furthermore,PEComa is a rare cause of intussusception,the telescoping of a segment of the gastrointestinal tract into an adjacent one.We describe a rare case of pediatric PEComa complicated with intussusception and anal incarceration,and conduct a review of the current literature.CASE SUMMARY A 12-year-old girl presented with abdominal pain and abdominal ultrasound suggested intussusception.Endoscopic direct-vision intussusception treatment and colonoscopy was performed.A spherical tumor was discovered in the transverse colon and removed by surgery.Postoperative pathologic analyses revealed that the tumor volume was 5.0 cm×4.5 cm×3.0 cm and the tumor tissue was located in the submucosa of the colon,arranged in an alveolar pattern.The cell morphology was regular,no neoplastic necrosis was observed,and nuclear fission was rare.The immunohistochemical staining results were as follows:Human melanoma black 45(HMB 45)(+),cluster of differentiation 31(CD31)(+),cytokeratin(-),melanoma-associated antigen recognized by T cells(-),smooth muscle actin(-),molleya(-),desmin(-),S-100(-),CD117(-),and Ki67(positive rate in hot spot<5%).Combined with the results of pathology and immunohistochemistry,we diagnosed the tumor as PEComa.Postoperative recovery was good at the 4 mo follow-up.CONCLUSION The diagnosis of PEComa mainly depends on pathology and immunohistochemistry.Radical resection is the preferred treatment method.展开更多
This study assessed the effects of a simulated high-altitude environment on the reproductive system of prepubertal male rats and the reversibility of these effects upon return to a normal environment.Three-week-old ma...This study assessed the effects of a simulated high-altitude environment on the reproductive system of prepubertal male rats and the reversibility of these effects upon return to a normal environment.Three-week-old male Wistar rats were randomly allocated to 4 groups that were exposed to different conditions:a normal environment for 6 weeks and 12 weeks,respectively,hypobaric hypoxia for 6 weeks,and hypobaric hypoxia for 6 weeks followed by a normal environment for 6 weeks.Multiple pathophysiological parameters were evaluated at the histological,endocrine,and molecular levels.Hypobaric hypoxia exposure for 6 weeks during the prepubertal phase significantly altered physiological parameters,body functions,blood indices,and reproductive potential.Six weeks after returning to a normal environment,the damaged reproductive functions partially recovered due to compensatory mechanisms.However,several changes were not reversed after returning to a normal environment for 6 weeks,including disorders of body development and metabolism,increased red blood cells,increased fasting blood glucose,abnormal blood lipid metabolism,decreased testicular and epididymis weights,abnormal reproductive hormone levels,excessive apoptosis of reproductive cells,and decreased sperm concentration.In summary,a hypobaric hypoxic environment significantly impaired the reproductive function of prepubertal male rats,and a return to normal conditions during the postpubertal phase did not fully recover these impairments.展开更多
The awareness and acceptance of the human papillomavirus(HPV)vaccines among Chinese primary and ju-nior high school students is limited.A meta-analysis was conducted to evaluate the awareness of HPV and HPV vaccines,a...The awareness and acceptance of the human papillomavirus(HPV)vaccines among Chinese primary and ju-nior high school students is limited.A meta-analysis was conducted to evaluate the awareness of HPV and HPV vaccines,as well as the acceptance of HPV vaccines,providing evidence-based strategies to promote HPV vacci-nation.Based on the databases of CNKI,Wanfang,VIP,PubMed,Embase,and Cochrane library,the literatures about the awareness of HPV and HPV vaccines,as well as the acceptance of HPV vaccines among parents of primary and junior high school students were collected from the inception to June 2023.Subgroup analysis was used to find the source of heterogeneity.Publication bias was evaluated using funnel plots and Egger’s test.Fifteen literatures with 21,853 participants were included.The pooled HPV awareness,HPV vaccine awareness and acceptance rates among parents of primary and junior high school students in China were 42.90%(95%CI:33.34%-52.47%),28.11%(95%CI:18.20%-43.41%),and 55.29%(95%CI:45.85%-64.36%),respectively.The survey period and the proportion of female parents were the heterogeneity in awareness of HPV and HPV vac-cines,as well as acceptance of HPV vaccines by subgroup analysis.Additionally,regional distribution emerged as another significant source of heterogeneity in HPV vaccine acceptance.The primary cause for parents’reluctance to vaccinate their children was theirs worries about the safety of the vaccines(66.21%).Though the awareness of HPV and its vaccines was low among parents of primary and junior high school students in China,the accep-tance of HPV vaccines was relatively high.Strengthening health education and publicity was crucial to enhance awareness and acceptance,promoting HPV vaccination for effective cervical cancer prevention.展开更多
Developing a cotton fabric sensing layer with good waterproofness and breathability via a low-cost and eco-friendly method is increasingly important for the construction of comfortable and wearable electronic devices....Developing a cotton fabric sensing layer with good waterproofness and breathability via a low-cost and eco-friendly method is increasingly important for the construction of comfortable and wearable electronic devices.Herein,a waterproof and breathable cotton fabric composite decorated by reduced graphene oxide(rGO)and carbon nanotube(CNT),Cotton/rGO/CNT,is reported by a facile solution infiltration method,and we adopt such Cotton/rGO/CNT composite to develop a layer-by-layer structured multifunctional flexible sensor,enabling the high-sensitivity detection of pressure and temperature stimulus.Particularly,the multifunctional flexible sensor exhibits a high response toward tiny pressure,demonstrating salient superiority in the continuous and reliable monitoring of human physiological information.Concerning temperature sensing,a good linear response for the temperatures ranging from 28 to 40℃ is achieved by the multifunctional flexible sensor and gives rise to be successfully applied to the non-contact real-time monitoring of human respiration signal.Finally,an array consisting of multifunctional flexible sensors further demonstrates its feasibility in perceiving and mapping the pressure and temperature information of contact objects.This work provides a feasible strategy for designing cotton-based sensing layers that can effectively resist liquid water penetration and allow water vapor transmission,and offers reasonable insight for constructing comfort and multifunctional wearable electronics.展开更多
Objectives We aimed to evaluate the risk factors for moderate-to-severe bronchopulmonary dysplasia(BPD)and focus on discussing its relationship with the duration of initial invasive mechanical ventilation(IMV)in very ...Objectives We aimed to evaluate the risk factors for moderate-to-severe bronchopulmonary dysplasia(BPD)and focus on discussing its relationship with the duration of initial invasive mechanical ventilation(IMV)in very preterm neonates less than 32 weeks of gestational age(GA).Methods We performed a prospective cohort study involving infants born at 23–31 weeks of GA who were admitted to 47 different neonatal intensive care unit(NICU)hospitals in China from January 2018 to December 2021.Patient data were obtained from the Sina-northern Neonatal Network(SNN)Database.Results We identified 6538 very preterm infants,of whom 49.5%(3236/6538)received initial IMV support,and 12.6%(823/6538)were diagnosed with moderate-to-severe BPD symptoms.The median duration of initial IMV in the moderateto-severe BPD group was 26(17–41)days,while in the no or mild BPD group,it was 6(3–10)days.The incidence rate of moderate-to-severe BPD and the median duration of initial IMV were quite different across different GAs.Multivariable logistic regression analysis showed that the onset of moderate-to-severe BPD was significantly associated with the duration of initial IMV[adjusted odds ratio(AOR):1.97;95%confidence interval(CI):1.10–2.67],late-onset neonatal sepsis(LONS),and patent ductus arteriosus(PDA).Conclusion In this multicenter cohort study,the duration of initial IMV was still relatively long in very premature infants,and the longer duration of initial IMV accounts for the increased risk of moderate-to-severe BPD.展开更多
Living cells and active factors are the two core elements of tissue repair,directly affecting the healing efficiency of damaged tissue.Nanofat(NF)can release living cells,such as adipose-derived stem cells(ADSCs),as w...Living cells and active factors are the two core elements of tissue repair,directly affecting the healing efficiency of damaged tissue.Nanofat(NF)can release living cells,such as adipose-derived stem cells(ADSCs),as well as active growth factors to promote angiogenesis,thus realizing cell-based wound healing.Herein,a novel living electrospun short fibrous sponge is constructed by modifying three-dimensional(3D)bionic short fibers with engineered NF.The uniform distribution of the polydopamine(PDA)modification endows the living sponges with stable mechanical properties,reversible water absorption and excellent adhesion even after repeated compression by an external force and long-term aqueous immersion.Meanwhile,the living electrospun short fibrous sponges with uniform NF modification contain living cells such as ADSCs and active growth factors such as vascular endothelial growth factor(VEGF),which can effectively promote the tube formation of human umbilical vein endothelial cells(HUVECs).In vivo,the living sponges can effectively and continuously act on wounds and act as a bionic living skin to prevent the loss of internal nutrients,creating a comfortable and favorable microenvironment for tissue regeneration and promoting the healing of diabetic wounds.Therefore,living electrospun short fibrous sponges via engineered NF are expected to achieve continuous wound healing with in situ living cells and active factors in injured tissues.展开更多
Cellular respiration can provide energy for wound healing.However,some of retarded healing processes in local hyperglycemic environment suffer from a decrease in cellular adaptation to oxygen,thus reducing in situ oxi...Cellular respiration can provide energy for wound healing.However,some of retarded healing processes in local hyperglycemic environment suffer from a decrease in cellular adaptation to oxygen,thus reducing in situ oxidative metabolism.Herein,a three-dimensional(3D)extracellular matrix(ECM)bionic short fibrous sponge was prepared for chronic diabetic wound healing and effectively regulated cellular respiration by enhancing cellular adaptation to oxygen and remolding the local tissue microenvironment.The 3D bionic sponge scaffold exhibited good cell adhesion,biocompatibility,bioactivity,and,most importantly,aggregated oxygen atoms on the graphene oxide(GO)surface.In an in vitro assay,the oxygen atom-concentrating short fibrous sponge activated monocyte chemoattractant protein-1(MCP-1),induced the expression of vascular endothelial growth factor(VEGF),and effectively promoted angiogenesis in a hyperglycemic environment.The sponge was also applied to diabetic wounds in vivo to verify its roles in the promotion of angiogenesis and collagen deposition.These experiments confirmed the synergistic effect of GO with adipose-derived stem cells(ADSCs),which could further promote diabetic wound healing.Therefore,oxygen atom-concentrating short fibrous sponges that regulate cellular respiration provide a new idea for the repair of poorly healing wounds by improving oxidative metabolism and have importantclinical significance.展开更多

文摘Living cells and active factors are the two core elements of tissue repair,directly affecting the healing efficiency of damaged tissue.Nanofat(NF)can release living cells,such as adipose-derived stem cells(ADSCs),as well as active growth factors to promote angiogenesis,thus realizing cell-based wound healing.Herein,a novel living electrospun short fibrous sponge is constructed by modifying three-dimensional(3D)bionic short fibers with engineered NF.The uniform distribution of the polydopamine(PDA)modification endows the living sponges with stable mechanical properties,reversible water absorption and excellent adhesion even after repeated compression by an external force and long-term aqueous immersion.Meanwhile,the living electrospun short fibrous sponges with uniform NF modification contain living cells such as ADSCs and active growth factors such as vascular endothelial growth factor(VEGF),which can effectively promote the tube formation of human umbilical vein endothelial cells(HUVECs).In vivo,the living sponges can effectively and continuously act on wounds and act as a bionic living skin to prevent the loss of internal nutrients,creating a comfortable and favorable microenvironment for tissue regeneration and promoting the healing of diabetic wounds.Therefore,living electrospun short fibrous sponges via engineered NF are expected to achieve continuous wound healing with in situ living cells and active factors in injured tissues.

文摘Cellular respiration can provide energy for wound healing.However,some of retarded healing processes in local hyperglycemic environment suffer from a decrease in cellular adaptation to oxygen,thus reducing in situ oxidative metabolism.Herein,a three-dimensional(3D)extracellular matrix(ECM)bionic short fibrous sponge was prepared for chronic diabetic wound healing and effectively regulated cellular respiration by enhancing cellular adaptation to oxygen and remolding the local tissue microenvironment.The 3D bionic sponge scaffold exhibited good cell adhesion,biocompatibility,bioactivity,and,most importantly,aggregated oxygen atoms on the graphene oxide(GO)surface.In an in vitro assay,the oxygen atom-concentrating short fibrous sponge activated monocyte chemoattractant protein-1(MCP-1),induced the expression of vascular endothelial growth factor(VEGF),and effectively promoted angiogenesis in a hyperglycemic environment.The sponge was also applied to diabetic wounds in vivo to verify its roles in the promotion of angiogenesis and collagen deposition.These experiments confirmed the synergistic effect of GO with adipose-derived stem cells(ADSCs),which could further promote diabetic wound healing.Therefore,oxygen atom-concentrating short fibrous sponges that regulate cellular respiration provide a new idea for the repair of poorly healing wounds by improving oxidative metabolism and have importantclinical significance.
