Objective:To explore the clinical effect of a non-invasive ventilator combined with conventional therapy in the treatment of patients with chronic obstructive pulmonary disease(COPD)combined with respiratory failure.M...Objective:To explore the clinical effect of a non-invasive ventilator combined with conventional therapy in the treatment of patients with chronic obstructive pulmonary disease(COPD)combined with respiratory failure.Methods:68 patients with COPD combined with respiratory failure treated in our hospital from September 2021 to October 2023 were selected as the research subjects.Using the random number table method,they were divided into a control group and an experimental group of 34 cases each.The control group received conventional symptomatic treatment,and the experimental group received non-invasive ventilator treatment based on the control group.The clinical effects,blood gas indicators(partial pressure of carbon dioxide(PaCO_(2)),partial pressure of oxygen(PaO_(2)),arterial oxygen saturation(SaO_(2))),lung function(forced expiratory volume in 1 second(FEV1),forced vital capacity(FVC),6 min walking distance),complications,and inflammatory factor levels(c-reactive protein(CRP),interleukin-6(IL-6),neutrophil-to-lymphocyte ratio(NLR))of the two groups of patients were observed.Results:(1)The clinical efficacy of the patients in the experimental group(33/97.06%)was more significant as compared with the control group(25/73.53%)(P<0.05);(2)After treatment,the clinical efficacy of the two groups of patients in terms of FEV1,FEV1/FVC,6-minute walking distance,PaO_(2)and SaO_(2)all increased in the experimental group as compared to that of the control group(P<0.05);(3)After treatment,the PaCO_(2),CRP,IL-6,and NLR of the two groups of patients decreased,and the decrease in the experimental group was higher than that of the control group(P<0.05);(4)The patients’complication rate in the experimental group(2/5.88%)was lower as compared to that of the control group(9/26.46%)(P<0.05).Conclusion:Non-invasive ventilators combined with conventional therapy achieved good clinical results in treating patients with COPD and respiratory failure.展开更多
Objective:To explore the preventive effect of comprehensive nursing intervention on arteriovenous fistula failure in patients in the hemodialysis room.Methods:82 patients with arteriovenous fistula in the hemodialysis...Objective:To explore the preventive effect of comprehensive nursing intervention on arteriovenous fistula failure in patients in the hemodialysis room.Methods:82 patients with arteriovenous fistula in the hemodialysis chamber treated in our hospital from July 2022 to October 2023 were selected as the research subjects.The random number table method divided them into a control group and an experimental group of 41 cases each.The control group received general nursing intervention measures,while the experimental group underwent comprehensive nursing intervention.The incidence of complications(internal fistula failure,bleeding,thrombosis,infection),psychological emotions(SAS scale,SDS scale),quality of life(physical pain,physiological function,emotional function,social function),and nursing satisfaction(very satisfied,basically satisfied,satisfied,dissatisfied)were compared between the two groups of patients.Results:The incidence of complications in the experimental group(4,9.76%)was significantly lower than that in the control group(29,70.73%);the SAS scores and SDS scores of the patients in the experimental group after intervention were both lower than those in the control group;the quality of life score(physical pain,physiological function,emotional function,and social function)was all higher than those of the control group;the post-intervention nursing satisfaction of the experimental group(40,97.57%)was also significantly higher than that of the control group(29,73.17%);and the listed differences were statistically significant(P<0.01).Conclusion:For patients with hemodialysis ventricular arteriovenous fistula failure,comprehensive nursing intervention can reduce the incidence of complications,relieve anxiety and stress,improve quality of life and nursing satisfaction,and achieve better preventive effects.展开更多

We developed a putative rat model of schizophrenia by transfecting lentiviral particles carrying the Egr3 gene into bilateral hippocampal dentate gyrus. We assessed spatial working memory using the Morris water maze test, and neuronal metabolite levels in bilateral hippocampus and thalamus were determined by 3.0 T proton magnetic resonance spectroscopy. Choline content was significantly greater in the hippocampus after transfection, while N-acetylaspartate and the ratio of N-acetylaspartate to creatine/phosphocreatine in the thalamus were lower than in controls. This study is the first to report evaluation of brain metabolites using 3,0 T proton magnetic resonance spectroscopy in rats transfected with Egr3, and reveals metabolic abnormalities in the hippocampus and thalamus in this putative model of schizophrenia.展开更多
Objective:To observe the clinical effects of medical cupping for metabolic syndrome(MetS)with abdominal obesity.Methods:In total,75 patients with MetS with abdominal obesity were randomly divided into three groups:med...Objective:To observe the clinical effects of medical cupping for metabolic syndrome(MetS)with abdominal obesity.Methods:In total,75 patients with MetS with abdominal obesity were randomly divided into three groups:medical cupping,acupuncture,and waiting.Patients in the medical cupping group received smearing of Chinese medicine and cupping twice a week for 8 weeks.Patients in the acupuncture group received acupuncture on regulating the Dai meridian three times a week for 8 weeks.The waiting group was observed without any intervention.Changes in metabolic indices,including waist circumference(WC),blood pressure,fasting triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),fasting blood glucose(FBG),2-h blood glucose(2hBG),and subcutaneous fat thickness were observed and compared before and after treatment among the three groups.Results:After the treatment,the WC,TG,FBG,and 2hBG in the medical cupping and acupuncture groups were lower than those in the waiting group.No difference was observed between the medical cupping and acupuncture groups.The subcutaneous fat thickness at the upper umbilicus,right side of the umbilicus,and waist in the medical cupping and acupuncture groups were lower than those in the waiting group.The subcutaneous fat thickness at the upper umbilicus and waist in the medical cupping group was lower than that in the acupuncture group.The MetS prevalence in the medical cupping and acupuncture groups was lower than that in the waiting group.Conclusion:medical cupping treatment can effectively alleviate metabolic indices and subcutaneous fat thickness at the abdomen in patients with MetS and abdominal obesity and decrease the MetS prevalence.Its efficacy was better than that of waiting and similar to that of acupuncture.The frequency of medical cupping is lower than that of the acupuncture.Meanwhile,it circumvents some patients’fear of acupuncture.medical cupping should be clinically promoted.展开更多
BACKGROUND:Hyperglycemia has been detected in many critically ill patients in the department of emergency medicine.But its mechanism and prognosis have not been well elucidated.In this study,we measured the serum leve...BACKGROUND:Hyperglycemia has been detected in many critically ill patients in the department of emergency medicine.But its mechanism and prognosis have not been well elucidated.In this study,we measured the serum level of glycated hemoglobin A1C(HbA1c) in critically ill patients to evaluate the effects of hyperglycemia on the prognosis of the patients.METHODS:A total of 826 critically ill patients,who had been treated at the Department of Emergency Medicine of Chaoyang Hospital during October 2006 and November 2007,were divided into a diabetes mellitus group(n=184) and a non-diabetes mellitus group(642) according to whether they had diabetes mellitus.Fasting glucose and HbA1 c were measured in all patients.Those in the diabetes mellitus group were further assigned to a drug therapy subgroup and a non-drug therapy subgroup;the serum level of HbA1 c and its relationship with short-term outcome were evaluated.RESULTS:Fasting glucose increased in 78.8% of the patients(88.6%in the diabetes mellitus group,and 75.9%in the non-diabetes mellitus group,P<0.05),and HbA1 c was elevated in 45.5% of the patients(78.3% in the diabetes mellitus group,and 36.1%in the non-diabetes mellitus group,P<0.01).Fasting glucose,HbA1 c and 28-day mortality were improved more significantly(P<0.01) in the drug therapy subgroup than in the non-drug therapy subgroup.The 28-day mortality was more significantly different in patients with fasting blood glucose >8.33 mmol/L than in those with fasting blood glucose <8.33 mmol/L.CONCLUSIONS:Hyperglycemia of critically ill patients could not totally attribute to stress response,especially in those who have no history of diabetes mellitus.Prognosis of hyperglycemia may vary among critically ill patients.展开更多

[Methods]86 cases of acute pancreatitis were randomly divided into the control group( 43 cases) and the treatment group( 43 cases). The control group was given saline enema and routine treatment of western medicine,while the treatment group was treated with Dahuang( Rhubarb) decoction retention enema and routine treatment,2 times/d. After 7 days of treatment,two groups of clinical curative effects were compared. And changes of IL-6,hs-CRP,endotoxin and amylase levels before and after treatment in two groups were compared. [Results] Abdominal pain,abdominal distension,exhaust defecation,bowel sounds in treatment group patients were obviously improved. After treatment,the decline levels of serum IL-6,hsCRP,endotoxin and amylase in the treatment group were significantly better than those in the control group. [Conclusions] Dahuang( Rhubarb) retention enema can significantly improve the clinical curative effect of severe acute pancreatitis,and its mechanism may be related to the inhibition of inflammatory reaction.展开更多
Spinal cord injury dramatically blocks information exchange between the central nervous system and the peripheral nervous system.The resulting fate of synapses in the motor cortex has not been well studied.To explore ...Spinal cord injury dramatically blocks information exchange between the central nervous system and the peripheral nervous system.The resulting fate of synapses in the motor cortex has not been well studied.To explore synaptic reorganization in the motor cortex after spinal cord injury,we established mouse models of T12 spinal cord hemi-section and then monitored the postsynaptic dendritic spines and presynaptic axonal boutons of pyramidal neurons in the hindlimb area of the motor cortex in vivo.Our results showed that spinal cord hemisection led to the remodeling of dendritic spines bilaterally in the motor cortex and the main remodeling regions changed over time.It made previously stable spines unstable and eliminated spines more unlikely to be re-emerged.There was a significant increase in new spines in the contralateral motor cortex.However,the low survival rate of the new spines demonstrated that new spines were still fragile.Observation of presynaptic axonal boutons found no significant change.These results suggest the existence of synapse remodeling in motor cortex after spinal cord hemi-section and that spinal cord hemi-section affected postsynaptic dendritic spines rather than presynaptic axonal boutons.This study was approved by the Ethics Committee of Chinese PLA General Hospital,China(approval No.201504168S)on April 16,2015.展开更多
Objective:To investigate the effect of saponin from the seed of Litchi chinensis Sonn(SLS)on the growth and apoptosis of human kidney epithelial cells(HKC)cultured in high glucose.Methods:HKC were cultured in DMEM/F12...Objective:To investigate the effect of saponin from the seed of Litchi chinensis Sonn(SLS)on the growth and apoptosis of human kidney epithelial cells(HKC)cultured in high glucose.Methods:HKC were cultured in DMEM/F12 medium supplemented with 30 mmol/L glucose and treated with or without SLS.In the normal group,isometric DMEM/F12 medium with 5.5mmol/L glucose was added.The secretion of TGF-β1 and fibronectin(FN)were detected by ELISA.Cell apoptosis was detected by the method of Annexin V-FITC/PI double staining.Western blot was used to detect the level of suppressor of cytokine signaling-1(SOCS-1).Results:The result of ELISA showed that the secretion of TGF-β1 and FN was decreased in SLS groups compared with those in 30 mmol/L glucose treated group(P<0.05).There were more cells apoptosis in 30 mmol/L glucose treated group than that in the normal group(P<0.01).Compared with the 30 mmol/L glucose treated group,the apoptosis of HKC were significantly decreased in SLS groups(P<0.01).Western blot showed that the level of SOCS-1 in high glucose+SLS group was decreased(P<0.01),compared with the high glucose group.Conclusion:SLS can reduce the secretion of TGF-β1 and FN in HKC by reducing the deposition of extracellular matrix.SLS also significantly reduced the apoptosis of HKC by inhibiting the level of SOCS-1.These results suggest the roles of SLS in preventing the progress of glomerular sclerosis.展开更多

This study was conducted to evaluate the comparative effectiveness of the external-application herbs and physical therapy in upper limb spasticity after stroke. Methods: A total of 100 patients with upper limb spasm after stroke (stroke time less than 1 month), have been recruited in Shun-yi Hospital of Beijing Traditional Chinese Medicine Hospital. Patients will be randomly allotted to one of two groups (N = 50, respectively): the TCM external application group, and the usual rehab (conventional physical therapy and medication treatment) group. The two groups will be administered once a month for three months. The primary outcome will be Modified Athworth Scale (MAS) and the simplified scale (Fugl-Meyer assessment, FMA) for upper limb. Results: Muscle tension and motor function were evaluated before and 4 weeks after treatments. There was a significant improvement in muscle tension and motor function in patients with stroke after the external-application herbs and physical therapy (P < 0.05). Conclusion: There was a significant improvement in kinematics in patients with stroke.展开更多

Method: A total of 90 patients with...Objective: To explore the effect of Bifidobacterium triple live bacteria on inflammatory factors, oxidative stress and T lymphocyte subsets in patients with ulcerative colitis (UC). Method: A total of 90 patients with UC treated in our hospital were selected and randomly divided into the observation group and the control group each with 45 cases. The control group was given Mesalazin Enteric-coated Tablets 1 g/times, 4 times/d, continuous treatment for 1 months:when the clinical symptoms were stable, switched to 500 mg/times, 3 times/d, continuous treatment for 3 months. The observation group on the basis of conventional treatment to give Bifidobacterium triple live bacteria scattered adjuvant therapy (Bifico capsule), 0.84 g/time, 2 times/d, continuous treatment for 3 months. In the two groups, the levels of inflammatory factors (IL-6, IL-10, CRP, TNF-α), oxidative stress markers (MDA, SOD) and T lymphocyte subsets (CD3+, CD4+, CD8+, CD4+/CD8+) were compared pre and post treatment. Results: The levels of IL-6, CRP, MDA, TNF-α, CD8+ in both two groups were significantly decreased compared with treatment before;the levels of IL-10, SOD, CD3+, CD4+ and CD4+/CD8+were significantly increased compared with treatment before. After treatment, the levels of serum IL-6, CRP, TNF-α, MDA and CD8+ in observation group with the data (72.17±15.18) pg/mL, (21.52±10.21) mg/mL, (15.98±4.12) pg/mL respectively were decreased more significantly than those data in control group which were (86.55±17.26) pg/mL, (43.02±12.27) mg/L, (22.35±3.67) pg/mL, MDA level (5.89±0.56) nmol/mL in observation group was decreased more significantly than the level in the control group (6.75±0.68) nmol/mL;CD8+level (17.24±3.06)% in observation group was decreased more significantly than the level (19.01±2.62)% in the control group. After treatment, IL-10 level (70.21±6.03) pg/mL in observation group was increased more significantly than the level in the control group (56.48+ 8.67) pg/mL;SOD level (1.84±0.06) U/mL in observation group was increased more significantly than the level in the control group (1.32±0.05) U/mL;the levels of CD3+, CD4+, CD4+/CD8+ in the observation group (57.84±6.07)%;(36.78±4.32)%;(1.92±0.29) were increased more significantly than the level in the control group (54.93±6.87)%;(35.42±5.27)%;(1.89±0.12). Conclusion: Bifidobacterium triple live bacteria scattered adjuvant treatment of ulcerative colitis helps regulate oxidative stress and inflammatory cytokines inhibiting inflammatory reaction and improving enhance function, and can effectively improve the clinical symptoms of patients with UC.展开更多

Histology, histochemistry, electron microscope methods and healing rate measurement were used to show the FGF expression and wound healing process. Results:Endogenous FGF were expressed in both of the groups, in which of the experimental group was higher than that of the control group, the wound surface healing rate of experimental group was also higher and paralleled with FGF expression. Conclusion:Arnebia Root oil has effects to promote FGF expression and enhances wound surface repair. The wound healing mechanism between the action of Arnebia Root oil and function of FGF need further investigating.展开更多
Objective:Computer-aided diagnosis using deep learning algorithms has been initially applied in the field of mammography,but there is no large-scale clinical application.Methods:This study proposed to develop and veri...Objective:Computer-aided diagnosis using deep learning algorithms has been initially applied in the field of mammography,but there is no large-scale clinical application.Methods:This study proposed to develop and verify an artificial intelligence model based on mammography.Firstly,mammograms retrospectively collected from six centers were randomized to a training dataset and a validation dataset for establishing the model.Secondly,the model was tested by comparing 12 radiologists’performance with and without it.Finally,prospectively enrolled women with mammograms from six centers were diagnosed by radiologists with the model.The detection and diagnostic capabilities were evaluated using the freeresponse receiver operating characteristic(FROC)curve and ROC curve.Results:The sensitivity of model for detecting lesions after matching was 0.908 for false positive rate of 0.25 in unilateral images.The area under ROC curve(AUC)to distinguish the benign lesions from malignant lesions was0.855[95%confidence interval(95%CI):0.830,0.880].The performance of 12 radiologists with the model was higher than that of radiologists alone(AUC:0.852 vs.0.805,P=0.005).The mean reading time of with the model was shorter than that of reading alone(80.18 s vs.62.28 s,P=0.032).In prospective application,the sensitivity of detection reached 0.887 at false positive rate of 0.25;the AUC of radiologists with the model was 0.983(95%CI:0.978,0.988),with sensitivity,specificity,positive predictive value(PPV),and negative predictive value(NPV)of94.36%,98.07%,87.76%,and 99.09%,respectively.Conclusions:The artificial intelligence model exhibits high accuracy for detecting and diagnosing breast lesions,improves diagnostic accuracy and saves time.展开更多

Methods: Using Chinese words of “impaired glucose tolerance, abnormalglucose tolerance, IGT” and “TG” as search terms, China National Knowledge Infrastructure, VIP, Wanfang Dataand Pubmed were searched from database inception until October 2017. All the controlled clinical researches on thetreatment of IGT by TG meeting the inclusion and exclusion criteria were retrieved and analyzed by ReviewManager 5.3 software. Results: A total of 7 articles including 1082 participants were enrolled. Meta-analysisshowed that the OR value of inversion rate was 2.17, 95% CI (1.65, 2.84). Weighted mean difference (WMD) valueof fasting plasma glucose was -0.25, 95% CI (-0.39, -0.11). After 2 h, the WMD value of serum glucose was -0.73,95% CI (-0.96, -0.51), all of which were better than these of control group. The OR value of progression rate (type2 diabetes mellitus) was 0.44, 95% CI (0.32, 0.59), less than the control group. All of the differences werestatistically significant. No hepatic and renal toxicity case was reported. Only 1 article reported adverse reactions inthe course of treatment. Conclusion: TG could treat IGT effectively, delay and even invert the progress of IGT, butits security still needed further discussion.展开更多

Results: After treatment, the levels of APTT and PT in the two groups were significantly higher than those before treatment, while the levels of DD were significantly lower than those before treatment, and the levels of APTT and PT [(38.19±7.05) s,(17.60±3.68) s] in the observation group were significantly higher than those in the control group, while the levels of DD [(0.51±0.10) mg/L] in the observation group were significantly lower than those in the control group;the levels of CRP, PCT and TNF-α in the two groups were significantly lower than those before treatment, and the levels of CRP, PCT and TNF-α [(29.10±4.76) mg/L, (0.59±0.17) μg/L, (57.28±10.61) ng/L] in the observation group were significantly lower than those in the control group;the levels of SOD in the two groups were significantly higher than those before treatment, but MDA was significantly lower than that before treatment, and the level of SOD [(179.10±4.76) U/mL] in the observation group was significantly higher than that in the control group after treatment, while the level of MDA [(1.97±0.55) mmol/mL] in the observation group was significantly lower than that in the control group after treatment;the levels of NO in the two groups were significantly higher than those before treatment, while the levels of ET and vWF were significantly lower than those before treatment, and the levels of NO [(10.56±1.17) U/mL] in the observation group were significantly higher than those in the control group after treatment, while the levels of ET and vWF [(105.83±5.20) pg/mL, (102.47±16.91)%] in the observation group were significantly lower than those in the control group after treatment. Conclusions:Complex salvia miltiorrhiza injection can improve the blood coagulation function, alleviate inflammation, enhance antioxidant function and vascular endothelial function in patients with acute pancreatitis, which has clinical value of promotion.展开更多
Objective: To study the effects of Dengzhan Shengmai Capsule combined with antiplatelet drug and statin therapy on platelet function, inflammatory response and lipid metabolism in patients with unstable angina pectori...Objective: To study the effects of Dengzhan Shengmai Capsule combined with antiplatelet drug and statin therapy on platelet function, inflammatory response and lipid metabolism in patients with unstable angina pectoris. Methods: A total of 96 patients who were diagnosed with unstable angina pectoris for the first time in Beijing Shunyi District Hospital of Traditional Chinese Medicine between February 2015 and January 2018 were chosen and randomly divided into two groups, the observation group received Dengzhan Shengmai Capsule combined with antiplatelet drug and statin therapy, and the control group only accepted antiplatelet drug and statin therapy. Platelet function parameters, inflammatory response parameters and lipid metabolism parameters were determined before treatment and after 2 months of treatment. Results: Compared with those of same group before treatment, PAF, P-selectin, PDGF, E-selectin, Gal-3, TNF-α, IFN-γ, LDL-C, PCSK9, Chemerin and Visfatin contents in serum as well as C3aR and C5aR contents in peripheral blood platelets of both groups of patients were decreasing whereas sFGL2, TGF-β1, APN and Vaspin contents in serum were increasing after treatment, and PAF, P-selectin, PDGF, E-selectin, Gal-3, TNF-α, IFN-γ, LDL-C, PCSK9, Chemerin and Visfatin contents in serum as well as C3aR and C5aR contents in peripheral blood platelets of observation group after treatment were lower than those of control group whereas sFGL2, TGF-β1, APN and Vaspin contents in serum were higher than those of control group. Conclusion: Dengzhan Shengmai Capsule combined with antiplatelet drug and statin therapy for unstable angina pectoris can more effectively inhibit the platelet aggregation and improve the inflammatory response and lipid metabolism.展开更多

Methods:A total of 92 cases of patients with coronary heart disease with mild to moderate hypertension were selected as the object of observation, according to the random data table, they were divided into the control group (n=46) and observation group (n=46), and patients in the control group were treated with Nifedipine Controlled Release Table therapy, on this basis, the observation group patients were given telmisartan treatment, two groups were treated for 6 months. The levels of the blood pressure, inflammatory factors, vascular endothelial function and left ventricular function compared between the two groups before and after treatment. Results: There were no significant differences in the levels of SBP, DBP, hs-CRP, TNF-α, NO, ET-1, LVEF, LVEDD and LVESD in the two groups before treatment. After treatment, two groups of SBP, DBP, hs-CRP, TNF-α, ET-1, LVEDD and LVESD levels were significantly lower than those in the same group before treatment, and after treatment, the levels of SBP, DBP, hs-CRP, TNF-α, ET-1 and LVESD in the observation group were significantly lower than those in the control group, while there were no significant difference in the level of LVEDD between the two groups after treatment;Compared with level in the group before treatment, the levels of NO and LVEF in the two groups were significantly increased, and the observation group [(82.13±19.01) μmol/L, (52.83±7.45)%] was significantly higher than the control group ((67.37±13.08) μmol/L, (49.47±6.96)%)Conclusion: Telmisartan combined with Nifedipine Controlled Release Table in treating coronary heart disease with mild to moderate hypertension, can effectively control blood pressure, reduce the inflammatory stress, improve vascular endothelial function and left ventricular function of patients, has an important clinical value.展开更多
文摘Objective:To explore the clinical effect of a non-invasive ventilator combined with conventional therapy in the treatment of patients with chronic obstructive pulmonary disease(COPD)combined with respiratory failure.Methods:68 patients with COPD combined with respiratory failure treated in our hospital from September 2021 to October 2023 were selected as the research subjects.Using the random number table method,they were divided into a control group and an experimental group of 34 cases each.The control group received conventional symptomatic treatment,and the experimental group received non-invasive ventilator treatment based on the control group.The clinical effects,blood gas indicators(partial pressure of carbon dioxide(PaCO_(2)),partial pressure of oxygen(PaO_(2)),arterial oxygen saturation(SaO_(2))),lung function(forced expiratory volume in 1 second(FEV1),forced vital capacity(FVC),6 min walking distance),complications,and inflammatory factor levels(c-reactive protein(CRP),interleukin-6(IL-6),neutrophil-to-lymphocyte ratio(NLR))of the two groups of patients were observed.Results:(1)The clinical efficacy of the patients in the experimental group(33/97.06%)was more significant as compared with the control group(25/73.53%)(P<0.05);(2)After treatment,the clinical efficacy of the two groups of patients in terms of FEV1,FEV1/FVC,6-minute walking distance,PaO_(2)and SaO_(2)all increased in the experimental group as compared to that of the control group(P<0.05);(3)After treatment,the PaCO_(2),CRP,IL-6,and NLR of the two groups of patients decreased,and the decrease in the experimental group was higher than that of the control group(P<0.05);(4)The patients’complication rate in the experimental group(2/5.88%)was lower as compared to that of the control group(9/26.46%)(P<0.05).Conclusion:Non-invasive ventilators combined with conventional therapy achieved good clinical results in treating patients with COPD and respiratory failure.
文摘Objective:To explore the preventive effect of comprehensive nursing intervention on arteriovenous fistula failure in patients in the hemodialysis room.Methods:82 patients with arteriovenous fistula in the hemodialysis chamber treated in our hospital from July 2022 to October 2023 were selected as the research subjects.The random number table method divided them into a control group and an experimental group of 41 cases each.The control group received general nursing intervention measures,while the experimental group underwent comprehensive nursing intervention.The incidence of complications(internal fistula failure,bleeding,thrombosis,infection),psychological emotions(SAS scale,SDS scale),quality of life(physical pain,physiological function,emotional function,social function),and nursing satisfaction(very satisfied,basically satisfied,satisfied,dissatisfied)were compared between the two groups of patients.Results:The incidence of complications in the experimental group(4,9.76%)was significantly lower than that in the control group(29,70.73%);the SAS scores and SDS scores of the patients in the experimental group after intervention were both lower than those in the control group;the quality of life score(physical pain,physiological function,emotional function,and social function)was all higher than those of the control group;the post-intervention nursing satisfaction of the experimental group(40,97.57%)was also significantly higher than that of the control group(29,73.17%);and the listed differences were statistically significant(P<0.01).Conclusion:For patients with hemodialysis ventricular arteriovenous fistula failure,comprehensive nursing intervention can reduce the incidence of complications,relieve anxiety and stress,improve quality of life and nursing satisfaction,and achieve better preventive effects.
文摘Objective:This study aimed to survey the characteristics and treatments of cancer pain in Beijing hospitals,China.Methods:At 20:00 on December 22,2009,there conducted a questionnaire survey in 2238 patients with malignant tumors of 26 hospitals in Beijing,and the survey results were statistically analyzed.Results:The 531 cases in 2238 patients had pained and 121 cases (22.79%) had outbreak pain with one week.At 20:00 on December 22,2009,199 cases (38%) in the above-mentioned 531 pain patients suffered the moderate to severe pain (pain scores ≥ 4).The number of pain (pain scores ≥ 4) patients in the consecutive three days from December 20 to December 22 were 150.Conclusion:In the 531 cancer pain patients of the surveyed hospitals,38% of the patients were not satisfied with the pain control.Doctors believed that the main reason for pain not controlled was the non-standard treatment,but patients considered to be afraid of addiction.
文摘The transcription factor early growth response protein 3 (EGR3) is involved in schizophrenia. We developed a putative rat model of schizophrenia by transfecting lentiviral particles carrying the Egr3 gene into bilateral hippocampal dentate gyrus. We assessed spatial working memory using the Morris water maze test, and neuronal metabolite levels in bilateral hippocampus and thalamus were determined by 3.0 T proton magnetic resonance spectroscopy. Choline content was significantly greater in the hippocampus after transfection, while N-acetylaspartate and the ratio of N-acetylaspartate to creatine/phosphocreatine in the thalamus were lower than in controls. This study is the first to report evaluation of brain metabolites using 3,0 T proton magnetic resonance spectroscopy in rats transfected with Egr3, and reveals metabolic abnormalities in the hippocampus and thalamus in this putative model of schizophrenia.
文摘Objective:To observe the clinical effects of medical cupping for metabolic syndrome(MetS)with abdominal obesity.Methods:In total,75 patients with MetS with abdominal obesity were randomly divided into three groups:medical cupping,acupuncture,and waiting.Patients in the medical cupping group received smearing of Chinese medicine and cupping twice a week for 8 weeks.Patients in the acupuncture group received acupuncture on regulating the Dai meridian three times a week for 8 weeks.The waiting group was observed without any intervention.Changes in metabolic indices,including waist circumference(WC),blood pressure,fasting triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),fasting blood glucose(FBG),2-h blood glucose(2hBG),and subcutaneous fat thickness were observed and compared before and after treatment among the three groups.Results:After the treatment,the WC,TG,FBG,and 2hBG in the medical cupping and acupuncture groups were lower than those in the waiting group.No difference was observed between the medical cupping and acupuncture groups.The subcutaneous fat thickness at the upper umbilicus,right side of the umbilicus,and waist in the medical cupping and acupuncture groups were lower than those in the waiting group.The subcutaneous fat thickness at the upper umbilicus and waist in the medical cupping group was lower than that in the acupuncture group.The MetS prevalence in the medical cupping and acupuncture groups was lower than that in the waiting group.Conclusion:medical cupping treatment can effectively alleviate metabolic indices and subcutaneous fat thickness at the abdomen in patients with MetS and abdominal obesity and decrease the MetS prevalence.Its efficacy was better than that of waiting and similar to that of acupuncture.The frequency of medical cupping is lower than that of the acupuncture.Meanwhile,it circumvents some patients’fear of acupuncture.medical cupping should be clinically promoted.
文摘BACKGROUND:Hyperglycemia has been detected in many critically ill patients in the department of emergency medicine.But its mechanism and prognosis have not been well elucidated.In this study,we measured the serum level of glycated hemoglobin A1C(HbA1c) in critically ill patients to evaluate the effects of hyperglycemia on the prognosis of the patients.METHODS:A total of 826 critically ill patients,who had been treated at the Department of Emergency Medicine of Chaoyang Hospital during October 2006 and November 2007,were divided into a diabetes mellitus group(n=184) and a non-diabetes mellitus group(642) according to whether they had diabetes mellitus.Fasting glucose and HbA1 c were measured in all patients.Those in the diabetes mellitus group were further assigned to a drug therapy subgroup and a non-drug therapy subgroup;the serum level of HbA1 c and its relationship with short-term outcome were evaluated.RESULTS:Fasting glucose increased in 78.8% of the patients(88.6%in the diabetes mellitus group,and 75.9%in the non-diabetes mellitus group,P<0.05),and HbA1 c was elevated in 45.5% of the patients(78.3% in the diabetes mellitus group,and 36.1%in the non-diabetes mellitus group,P<0.01).Fasting glucose,HbA1 c and 28-day mortality were improved more significantly(P<0.01) in the drug therapy subgroup than in the non-drug therapy subgroup.The 28-day mortality was more significantly different in patients with fasting blood glucose >8.33 mmol/L than in those with fasting blood glucose <8.33 mmol/L.CONCLUSIONS:Hyperglycemia of critically ill patients could not totally attribute to stress response,especially in those who have no history of diabetes mellitus.Prognosis of hyperglycemia may vary among critically ill patients.
文摘[Objectives] To observe the clinical effect of Dahuang( Rhubarb) retention enema on patients with severe acute pancreatitis and its influences on leukocyte interleukin 6( IL-6),high sentive C reactive protein( hs-CRP) and endotoxin. [Methods]86 cases of acute pancreatitis were randomly divided into the control group( 43 cases) and the treatment group( 43 cases). The control group was given saline enema and routine treatment of western medicine,while the treatment group was treated with Dahuang( Rhubarb) decoction retention enema and routine treatment,2 times/d. After 7 days of treatment,two groups of clinical curative effects were compared. And changes of IL-6,hs-CRP,endotoxin and amylase levels before and after treatment in two groups were compared. [Results] Abdominal pain,abdominal distension,exhaust defecation,bowel sounds in treatment group patients were obviously improved. After treatment,the decline levels of serum IL-6,hsCRP,endotoxin and amylase in the treatment group were significantly better than those in the control group. [Conclusions] Dahuang( Rhubarb) retention enema can significantly improve the clinical curative effect of severe acute pancreatitis,and its mechanism may be related to the inhibition of inflammatory reaction.

文摘Spinal cord injury dramatically blocks information exchange between the central nervous system and the peripheral nervous system.The resulting fate of synapses in the motor cortex has not been well studied.To explore synaptic reorganization in the motor cortex after spinal cord injury,we established mouse models of T12 spinal cord hemi-section and then monitored the postsynaptic dendritic spines and presynaptic axonal boutons of pyramidal neurons in the hindlimb area of the motor cortex in vivo.Our results showed that spinal cord hemisection led to the remodeling of dendritic spines bilaterally in the motor cortex and the main remodeling regions changed over time.It made previously stable spines unstable and eliminated spines more unlikely to be re-emerged.There was a significant increase in new spines in the contralateral motor cortex.However,the low survival rate of the new spines demonstrated that new spines were still fragile.Observation of presynaptic axonal boutons found no significant change.These results suggest the existence of synapse remodeling in motor cortex after spinal cord hemi-section and that spinal cord hemi-section affected postsynaptic dendritic spines rather than presynaptic axonal boutons.This study was approved by the Ethics Committee of Chinese PLA General Hospital,China(approval No.201504168S)on April 16,2015.

文摘Objective:To investigate the effect of saponin from the seed of Litchi chinensis Sonn(SLS)on the growth and apoptosis of human kidney epithelial cells(HKC)cultured in high glucose.Methods:HKC were cultured in DMEM/F12 medium supplemented with 30 mmol/L glucose and treated with or without SLS.In the normal group,isometric DMEM/F12 medium with 5.5mmol/L glucose was added.The secretion of TGF-β1 and fibronectin(FN)were detected by ELISA.Cell apoptosis was detected by the method of Annexin V-FITC/PI double staining.Western blot was used to detect the level of suppressor of cytokine signaling-1(SOCS-1).Results:The result of ELISA showed that the secretion of TGF-β1 and FN was decreased in SLS groups compared with those in 30 mmol/L glucose treated group(P<0.05).There were more cells apoptosis in 30 mmol/L glucose treated group than that in the normal group(P<0.01).Compared with the 30 mmol/L glucose treated group,the apoptosis of HKC were significantly decreased in SLS groups(P<0.01).Western blot showed that the level of SOCS-1 in high glucose+SLS group was decreased(P<0.01),compared with the high glucose group.Conclusion:SLS can reduce the secretion of TGF-β1 and FN in HKC by reducing the deposition of extracellular matrix.SLS also significantly reduced the apoptosis of HKC by inhibiting the level of SOCS-1.These results suggest the roles of SLS in preventing the progress of glomerular sclerosis.

文摘The treatment of urethral strictures remains a challenging field in urology even though there are a variety of procedures to treat it at present,as no one approach is superior over another.This paper reviewed the surgical options for the management of different sites and types of anterior urethral stricture,providing a brief discussion of the controversies regarding this issue and suggesting possible future advancements.Among the existing procedures,simple dilation and direct vision internal urethrotomy are more commonly used for short urethral strictures(<1 cm,soft and no previous intervention).Currently,urethroplasty using buccal mucosa or penile skin is the most widely adopted clinical techniques and have proved successful.Nonetheless,complications such as donor site morbidity remain problem.Tissue engineering techniques are considered as a promising solution for urethral reconstruction,but require further investigation,as does stem cell therapy.
文摘Background: Spasm is one of the most serious physical impairment after stroke. the external-application herbs have showed some effects on the spasm. This study was conducted to evaluate the comparative effectiveness of the external-application herbs and physical therapy in upper limb spasticity after stroke. Methods: A total of 100 patients with upper limb spasm after stroke (stroke time less than 1 month), have been recruited in Shun-yi Hospital of Beijing Traditional Chinese Medicine Hospital. Patients will be randomly allotted to one of two groups (N = 50, respectively): the TCM external application group, and the usual rehab (conventional physical therapy and medication treatment) group. The two groups will be administered once a month for three months. The primary outcome will be Modified Athworth Scale (MAS) and the simplified scale (Fugl-Meyer assessment, FMA) for upper limb. Results: Muscle tension and motor function were evaluated before and 4 weeks after treatments. There was a significant improvement in muscle tension and motor function in patients with stroke after the external-application herbs and physical therapy (P < 0.05). Conclusion: There was a significant improvement in kinematics in patients with stroke.

文摘Objective: To explore the effect of Bifidobacterium triple live bacteria on inflammatory factors, oxidative stress and T lymphocyte subsets in patients with ulcerative colitis (UC). Method: A total of 90 patients with UC treated in our hospital were selected and randomly divided into the observation group and the control group each with 45 cases. The control group was given Mesalazin Enteric-coated Tablets 1 g/times, 4 times/d, continuous treatment for 1 months:when the clinical symptoms were stable, switched to 500 mg/times, 3 times/d, continuous treatment for 3 months. The observation group on the basis of conventional treatment to give Bifidobacterium triple live bacteria scattered adjuvant therapy (Bifico capsule), 0.84 g/time, 2 times/d, continuous treatment for 3 months. In the two groups, the levels of inflammatory factors (IL-6, IL-10, CRP, TNF-α), oxidative stress markers (MDA, SOD) and T lymphocyte subsets (CD3+, CD4+, CD8+, CD4+/CD8+) were compared pre and post treatment. Results: The levels of IL-6, CRP, MDA, TNF-α, CD8+ in both two groups were significantly decreased compared with treatment before;the levels of IL-10, SOD, CD3+, CD4+ and CD4+/CD8+were significantly increased compared with treatment before. After treatment, the levels of serum IL-6, CRP, TNF-α, MDA and CD8+ in observation group with the data (72.17±15.18) pg/mL, (21.52±10.21) mg/mL, (15.98±4.12) pg/mL respectively were decreased more significantly than those data in control group which were (86.55±17.26) pg/mL, (43.02±12.27) mg/L, (22.35±3.67) pg/mL, MDA level (5.89±0.56) nmol/mL in observation group was decreased more significantly than the level in the control group (6.75±0.68) nmol/mL;CD8+level (17.24±3.06)% in observation group was decreased more significantly than the level (19.01±2.62)% in the control group. After treatment, IL-10 level (70.21±6.03) pg/mL in observation group was increased more significantly than the level in the control group (56.48+ 8.67) pg/mL;SOD level (1.84±0.06) U/mL in observation group was increased more significantly than the level in the control group (1.32±0.05) U/mL;the levels of CD3+, CD4+, CD4+/CD8+ in the observation group (57.84±6.07)%;(36.78±4.32)%;(1.92±0.29) were increased more significantly than the level in the control group (54.93±6.87)%;(35.42±5.27)%;(1.89±0.12). Conclusion: Bifidobacterium triple live bacteria scattered adjuvant treatment of ulcerative colitis helps regulate oxidative stress and inflammatory cytokines inhibiting inflammatory reaction and improving enhance function, and can effectively improve the clinical symptoms of patients with UC.
文摘Objective:To investigate the effection of Arnebia Root oil on the FGF expression in wound surface and the ability to promote wound surface healing. Methods:24 wound surfaces of patients were divided into two groups. Experimental group was treated by Arnebia Root oil and the control was treated by petrolatum gauze. Histology, histochemistry, electron microscope methods and healing rate measurement were used to show the FGF expression and wound healing process. Results:Endogenous FGF were expressed in both of the groups, in which of the experimental group was higher than that of the control group, the wound surface healing rate of experimental group was also higher and paralleled with FGF expression. Conclusion:Arnebia Root oil has effects to promote FGF expression and enhances wound surface repair. The wound healing mechanism between the action of Arnebia Root oil and function of FGF need further investigating.

文摘Objective:Computer-aided diagnosis using deep learning algorithms has been initially applied in the field of mammography,but there is no large-scale clinical application.Methods:This study proposed to develop and verify an artificial intelligence model based on mammography.Firstly,mammograms retrospectively collected from six centers were randomized to a training dataset and a validation dataset for establishing the model.Secondly,the model was tested by comparing 12 radiologists’performance with and without it.Finally,prospectively enrolled women with mammograms from six centers were diagnosed by radiologists with the model.The detection and diagnostic capabilities were evaluated using the freeresponse receiver operating characteristic(FROC)curve and ROC curve.Results:The sensitivity of model for detecting lesions after matching was 0.908 for false positive rate of 0.25 in unilateral images.The area under ROC curve(AUC)to distinguish the benign lesions from malignant lesions was0.855[95%confidence interval(95%CI):0.830,0.880].The performance of 12 radiologists with the model was higher than that of radiologists alone(AUC:0.852 vs.0.805,P=0.005).The mean reading time of with the model was shorter than that of reading alone(80.18 s vs.62.28 s,P=0.032).In prospective application,the sensitivity of detection reached 0.887 at false positive rate of 0.25;the AUC of radiologists with the model was 0.983(95%CI:0.978,0.988),with sensitivity,specificity,positive predictive value(PPV),and negative predictive value(NPV)of94.36%,98.07%,87.76%,and 99.09%,respectively.Conclusions:The artificial intelligence model exhibits high accuracy for detecting and diagnosing breast lesions,improves diagnostic accuracy and saves time.
文摘Objective: To evaluate the clinical efficacy and safety of Tianqijiangtang Capsule (TG) during the treatment ofimpaired glucose tolerance (IGT). Methods: Using Chinese words of “impaired glucose tolerance, abnormalglucose tolerance, IGT” and “TG” as search terms, China National Knowledge Infrastructure, VIP, Wanfang Dataand Pubmed were searched from database inception until October 2017. All the controlled clinical researches on thetreatment of IGT by TG meeting the inclusion and exclusion criteria were retrieved and analyzed by ReviewManager 5.3 software. Results: A total of 7 articles including 1082 participants were enrolled. Meta-analysisshowed that the OR value of inversion rate was 2.17, 95% CI (1.65, 2.84). Weighted mean difference (WMD) valueof fasting plasma glucose was -0.25, 95% CI (-0.39, -0.11). After 2 h, the WMD value of serum glucose was -0.73,95% CI (-0.96, -0.51), all of which were better than these of control group. The OR value of progression rate (type2 diabetes mellitus) was 0.44, 95% CI (0.32, 0.59), less than the control group. All of the differences werestatistically significant. No hepatic and renal toxicity case was reported. Only 1 article reported adverse reactions inthe course of treatment. Conclusion: TG could treat IGT effectively, delay and even invert the progress of IGT, butits security still needed further discussion.
文摘objective: to explore the effect of detail nursing intervention on headache and negative emotion in patients with subarachnoid hemorrhage. Methods: the clinical data of 100 cases of subarachnoid hemorrhage from January 2015 to January 2017 of neurosurgery in our hospital were randomly divided into the control group and the experimental group, Detail nursing intervention in experimental group, Comparison of two groups of clinical nursing effects. Results the duration of hospitalization and headache in the experimental group were shorter than those in the control group. Fewer headaches than control group (P<0.05) Less headache than control group (p<0.05). Results: the results of SAS and SDS scores were significantly lower than those in the control group (p<0.05) after nursing in the experimental group. Conclusion: the patients with subarachnoid hemorrhage receive detailed nursing intervention;contribute to the alleviation of their headache symptoms, Relieves the patient of negative emotional problems.

文摘Objective: To investigate the effects of complex salvia miltiorrhiza on blood coagulation, inflammation, oxidative stress and endothelial index in patients with acute pancreatitis. Methods: A total of 122 patients with acute pancreatitis admitted to digestive system department in our hospital from April 2016 to November 2017 were randomly divided into control group (n=61) and observation group (n=61), the control group was treated with ulinastatin and the observation group was treated with complex salvia miltiorrhiza injection on the basis of the control group. The levels of coagulation, inflammation, oxidative stress and endothelial markers were compared between the two groups. Results: After treatment, the levels of APTT and PT in the two groups were significantly higher than those before treatment, while the levels of DD were significantly lower than those before treatment, and the levels of APTT and PT [(38.19±7.05) s,(17.60±3.68) s] in the observation group were significantly higher than those in the control group, while the levels of DD [(0.51±0.10) mg/L] in the observation group were significantly lower than those in the control group;the levels of CRP, PCT and TNF-α in the two groups were significantly lower than those before treatment, and the levels of CRP, PCT and TNF-α [(29.10±4.76) mg/L, (0.59±0.17) μg/L, (57.28±10.61) ng/L] in the observation group were significantly lower than those in the control group;the levels of SOD in the two groups were significantly higher than those before treatment, but MDA was significantly lower than that before treatment, and the level of SOD [(179.10±4.76) U/mL] in the observation group was significantly higher than that in the control group after treatment, while the level of MDA [(1.97±0.55) mmol/mL] in the observation group was significantly lower than that in the control group after treatment;the levels of NO in the two groups were significantly higher than those before treatment, while the levels of ET and vWF were significantly lower than those before treatment, and the levels of NO [(10.56±1.17) U/mL] in the observation group were significantly higher than those in the control group after treatment, while the levels of ET and vWF [(105.83±5.20) pg/mL, (102.47±16.91)%] in the observation group were significantly lower than those in the control group after treatment. Conclusions:Complex salvia miltiorrhiza injection can improve the blood coagulation function, alleviate inflammation, enhance antioxidant function and vascular endothelial function in patients with acute pancreatitis, which has clinical value of promotion.
文摘Objective: To study the effects of Dengzhan Shengmai Capsule combined with antiplatelet drug and statin therapy on platelet function, inflammatory response and lipid metabolism in patients with unstable angina pectoris. Methods: A total of 96 patients who were diagnosed with unstable angina pectoris for the first time in Beijing Shunyi District Hospital of Traditional Chinese Medicine between February 2015 and January 2018 were chosen and randomly divided into two groups, the observation group received Dengzhan Shengmai Capsule combined with antiplatelet drug and statin therapy, and the control group only accepted antiplatelet drug and statin therapy. Platelet function parameters, inflammatory response parameters and lipid metabolism parameters were determined before treatment and after 2 months of treatment. Results: Compared with those of same group before treatment, PAF, P-selectin, PDGF, E-selectin, Gal-3, TNF-α, IFN-γ, LDL-C, PCSK9, Chemerin and Visfatin contents in serum as well as C3aR and C5aR contents in peripheral blood platelets of both groups of patients were decreasing whereas sFGL2, TGF-β1, APN and Vaspin contents in serum were increasing after treatment, and PAF, P-selectin, PDGF, E-selectin, Gal-3, TNF-α, IFN-γ, LDL-C, PCSK9, Chemerin and Visfatin contents in serum as well as C3aR and C5aR contents in peripheral blood platelets of observation group after treatment were lower than those of control group whereas sFGL2, TGF-β1, APN and Vaspin contents in serum were higher than those of control group. Conclusion: Dengzhan Shengmai Capsule combined with antiplatelet drug and statin therapy for unstable angina pectoris can more effectively inhibit the platelet aggregation and improve the inflammatory response and lipid metabolism.
文摘Objective: To investigate the effect of telmisartan combined with Nifedipine Controlled Release Tablet on inflammatory factors, vascular endothelial function and left ventricular function in patients with coronary heart disease with mild to moderate hypertension. Methods:A total of 92 cases of patients with coronary heart disease with mild to moderate hypertension were selected as the object of observation, according to the random data table, they were divided into the control group (n=46) and observation group (n=46), and patients in the control group were treated with Nifedipine Controlled Release Table therapy, on this basis, the observation group patients were given telmisartan treatment, two groups were treated for 6 months. The levels of the blood pressure, inflammatory factors, vascular endothelial function and left ventricular function compared between the two groups before and after treatment. Results: There were no significant differences in the levels of SBP, DBP, hs-CRP, TNF-α, NO, ET-1, LVEF, LVEDD and LVESD in the two groups before treatment. After treatment, two groups of SBP, DBP, hs-CRP, TNF-α, ET-1, LVEDD and LVESD levels were significantly lower than those in the same group before treatment, and after treatment, the levels of SBP, DBP, hs-CRP, TNF-α, ET-1 and LVESD in the observation group were significantly lower than those in the control group, while there were no significant difference in the level of LVEDD between the two groups after treatment;Compared with level in the group before treatment, the levels of NO and LVEF in the two groups were significantly increased, and the observation group [(82.13±19.01) μmol/L, (52.83±7.45)%] was significantly higher than the control group ((67.37±13.08) μmol/L, (49.47±6.96)%)Conclusion: Telmisartan combined with Nifedipine Controlled Release Table in treating coronary heart disease with mild to moderate hypertension, can effectively control blood pressure, reduce the inflammatory stress, improve vascular endothelial function and left ventricular function of patients, has an important clinical value.
