BACKGROUND Gastric cancer(GC)is a deadly tumor with the fifth highest occurrence and highest global mortality rates.Owing to its heterogeneity,the underlying mechanism of GC remains unclear,and chemotherapy offers lit...BACKGROUND Gastric cancer(GC)is a deadly tumor with the fifth highest occurrence and highest global mortality rates.Owing to its heterogeneity,the underlying mechanism of GC remains unclear,and chemotherapy offers little benefit to individuals.AIM To investigate the clinical outcomes of TP53 and CDH1 mutations in GC.METHODS In this study,202 gastric adenocarcinoma tumor tissues and their corresponding normal tissues were collected.A total of 490 genes were identified using target capture.Through t-test and Wilcoxon rank-sum test,somatic mutations,microsatellite instability,and clinical statistics,including overall survival,were detected,compared,and calculated.RESULTS The mutation rates of 32 genes,including TP53,SPEN,FAT1,and CDH1 exceeded 10%.TP53 mutations had a slightly lower overall occurrence rate(33%).The TP53 mutation rate was significantly higher in advanced stages(stage Ⅲ/Ⅳ)than that in early stages(stage Ⅰ/Ⅱ)(P<0.05).In contrast,CDH1 mutations were significantly associated with diffuse GC.TP53 is related to poor prognosis of advanced-stage tumors;nevertheless,CDH1 corresponds to a diffuse type of cancer.TP53 is exclusively mutated in CDH1 and is primarily affected by two distinct GC mechanisms.CONCLUSION Different somatic mutation patterns in TP53 and CDH1 indicate two major mechanisms of GC.展开更多
Objective:To use Cite Space and VOSviewer to investigate the scientific production in the field of symptom clusters in cancer research.Methods:The search was performed using the terms“symptom clusters,”“cancer,”an...Objective:To use Cite Space and VOSviewer to investigate the scientific production in the field of symptom clusters in cancer research.Methods:The search was performed using the terms“symptom clusters,”“cancer,”and“oncology”on the Web of Science Core Collection database.The retrieval time was from 2001 to 2021,which covers the last 2 decades.Based on the production theory of scientific knowledge and the data mining of citations,data pertaining to the annual publications,journals,countries,organizations,authors,and keywords that produce symptom clusters in cancer research,as well as their cooperation(collaboration network),were extracted,and then both were clarified by the software tools VOSviewer(version 1.6.16)and Cite Space(version 6.1.R2).Results:A total of 1796 publications were retrieved between 2001 and 2021,and 473 relevant publications were included after screening.The results showed an increasing trend in published articles.The United States had the largest number of publications(261/473,55.18%),followed by China and Canada.The University of California,San Francisco,was the most productive institution.Current research hotspots included the analysis of symptom clusters and symptom management in patients with breast cancer and lung cancer,as well as any advanced cancer and cancer cachexia;fatigue-related symptom clusters and depression-anxiety symptom cluster;and the impacts of symptom clusters on quality of life.The research frontiers included analysis between health-related quality of life and symptom clusters,data mining in symptom clusters,research on the mental health status of cancer patients,and study of the mechanism and biological pathways of symptom clusters.Conclusions:The study provides insight into the global research perspective for the scientific progress on cancer symptom clusters,which suggests a growing scientific interest in this field,and more studies are warranted to guide symptom management.展开更多

BACKGROUND The management of severe extravasation injuries is still controversial.Extravasation injuries can be treated in many ways.AIM To present a series of patients with severe extravasation injuries due to infusi...BACKGROUND The management of severe extravasation injuries is still controversial.Extravasation injuries can be treated in many ways.AIM To present a series of patients with severe extravasation injuries due to infusion who were managed with ethacridine lactate dressing combined with localized closure and phototherapy.METHODS In this study,we evaluated the data of eight patients,including six from the Department of Burn,one(with colorectal carcinoma)from the Veteran Cadre Department,and one(with leukemia)from the Hematology Department.Of these,three patients were male and five were female.Age of the patients ranged from 10 mo to 72 years,including two children(10 and 19 mo of age).In this study,the infusion was stopped immediately when the extravasation was identified.The extravasation event was managed routinely using a blocking solution.A ring-shaped localized closure was performed using the blocking agents.Moreover,ethacridine lactate dressing and phototherapy were applied for 3-5 d.RESULTS In this study,the drugs contained in the infusates were iodixanol,norepinephrine,alprostadil,amino acids,fat emulsion,cefoselis,cefoxitin,and potassium chloride+concentrated sodium chloride.All of the patients achieved complete healing after treatment and no obvious adverse reactions were observed.CONCLUSION The treatment of severe extravasation injuries using a combination of localized closure,ethacridine lactate dressing,and phototherapy resulted in satisfactory outcomes in patients.展开更多
Objective:To explore the nurses’behaviors regarding clinical alarms,analyze the related influencing factors,and provide rationales for alarm management.Methods:A cross-sectional survey was conducted in China.The self...Objective:To explore the nurses’behaviors regarding clinical alarms,analyze the related influencing factors,and provide rationales for alarm management.Methods:A cross-sectional survey was conducted in China.The self-made questionnaire of nurses’clinical alarm-related knowledge,attitude,and behavior(NCAKAB)was used.Results:The valid response rate was 98.66%(n=2368).The average nurses’clinical alarm-related behaviors(NCAB)score was 65.14±7.95(out of 85).The dimension scores of NCAB from high to low were alarm learning(4.02±0.85,out of 5),alarm response(27.99±3.64,out of 35),alarm setting(19.24±3.88,out of 25),alarm recognition(7.63±1.68,out of 10)and alarm notification(6.25±1.84,out of 10).There were significant differences in alarm behavior scores between nurses of different ages(F=4.619,P=0.000),nursing stints(F=9.564,P=0.000),professional titles(F=4.425,P=0.004),departments(F=9.166,P=0.000),and hospital levels(t=2.705,P=0.007).The study showed that nurses’total alarm behavior scores were positively correlated with the total alarm knowledge score(r=0.267;P<0.001)and the total alarm attitude score(r=438;P<0.001).Conclusions:Nurses scored highest in alarm learning,followed by alarm response,alarm setting,alarm recognition,and alarm notification behavior.The factors that influenced alarm behavior included age,title,department,nursing stint,hospital level,professional title,alarm-related training,willingness to participate in alarm-related training,whether or not departments have improved alarm management over the last 3 years,and whether or not departments have formulated norms for alarm management.Nurses with higher scores for clinical alarm knowledge had higher correlating scores for alarm behavior;similarly,nurses with higher scores for clinical alarm attitude had higher scores for alarm-related behavior.展开更多
