This systematic review and meta-analysis summarized the available evidence of EGFR mutation epidemiology in Indian patients with adenocarcinoma (ADC) Non-Small Cell Lung Cancer (NSCLC). We conducted a structured literature search in PubMed, and EMBASE databases from January 2004 through October 2019. The primary outcome of interest was prevalence of EGFR mutation by gender, smoking status, and mutation subtype. The review included 34 studies. EGFR mutation prevalence was 39.5% in patients with ADC, and significantly higher in females, non-smokers, and patients with exon 19 deletions. The EGFR mutation frequency in Indian patients with ADC was higher than reported in Caucasians but at a lower range of that reported in East Asians. These findings support the use of EGFR mutation testing to guide choice of treatment.展开更多

Conventionalmethods struggle to manuallyextract features and capture lesions spatial and temporal variations. This study introduces a deep learning-basedConvolutional and Recurrent Neural Network (CNN-RNN) model with a ResNet-50 architecture which usedas the feature extractor to enhance skin cancer classification. Leveraging synergistic spatial feature extractionand temporal sequence learning, the model demonstrates robust performance on a dataset of 9000 skin lesionphotos from nine cancer types. Using pre-trained ResNet-50 for spatial data extraction and Long Short-TermMemory (LSTM) for temporal dependencies, the model achieves a high average recognition accuracy, surpassingprevious methods. The comprehensive evaluation, including accuracy, precision, recall, and F1-score, underscoresthe model’s competence in categorizing skin cancer types. What is not obvious and not recorded in a consultation is the clinical reasoning process that practitioners use. The research filmed three practitioners in the UK while they conducted a consultation and treatment on new patients. The practitioners and researchers viewed the films and used them as aide-memoirs while the reasoning process throughout was discussed. In order to determine the pattern, practitioners used the four examinations to gather information from the patient in an iterative process;their aesthetic reasoning was highly developed. Through triangulation they checked the information they received against a detailed understanding of the qi-dynamic. They used highly analytical strategies of forward(inductive) and backward(deductive) reasoning against the prototypes of the signs and symptoms that indicate a specific Zheng. This was achieved through an abductive process that linked description with explanation and causal factors with pathological mechanisms. The feedback loop with the patient continued through the consultation and into the treatment. A process of translation and interpretation was needed to turn the patient’s story into the practitioner’s story of qi-dynamics that then directed the treatment. Awareness of our clinical reasoning process will mitigate against biases, improve our diagnoses and treatment choices and support the training of students.展开更多

文摘Thoracic epidural anesthesia(TEA)has been the gold standard of perioperative analgesia in various abdominal and thoracic surgeries. However, misplaced or displaced catheters, along with other factors such as technical challenges, equipment failure, and anatomic variation, lead to a high incidence of unsatisfactory analgesia. This article aims to assess the different sources of TEA failure and strategies to validate the location of thoracic epidural catheters.A literature search of PubMed,Medline,Science Direct, and Google Scholar was done. The search results were limited to randomized controlled trials.Literature suggests techniques such as electrophysiological stimulation, epidural waveform monitoring, and x-ray epidurography for identifying thoracic epidural placement, but there is no one particular superior confirmation method;clinicians are advised to select techniques that are practical and suitable for their patients and practice environment to maximize success.
文摘Studies reporting the Indian prevalence of Epidermal Growth Factor Receptor (EGFR) mutation are mostly single centers with small sample sizes. These findings support the use of EGFR mutation testing to guide choice of treatment.

文摘The rapid growth of mobile applications, the popularity of the Android system and its openness have attracted many hackers and even criminals, who are creating lots of Android malware. However, the current methods of Android malware detection need a lot of time in the feature engineering phase.Furthermore, these models have the defects of low detection rate, high complexity, and poor practicability, etc. We analyze the Android malware samples, and the distribution of malware and benign software in application programming interface(API)calls, permissions, and other attributes. We classify the software’s threat levels based on the correlation of features. Then,we propose deep neural networks and convolutional neural networks with ensemble learning(DCEL),a new classifier fusion model for Android malware detection.First,DCEL preprocesses the malware data to remove redundant data, and converts the one-dimensional data into a two-dimensional gray image. Then, the ensemble learning approach is used to combine the deep neural network with the convolutional neural network, and the final classification results are obtained by voting on the prediction of each single classifier.Experiments based on the Drebin and Malgenome datasets show that compared with current state-of-art models, the proposed DCEL has a higher detection rate, higher recall rate, and lower computational cost.
文摘Skin cancer diagnosis is difficult due to lesion presentation variability. This research contributes a sophisticated model andvaluable guidance for deep learning-based diagnostics, also this model excels in overcoming spatial and temporalcomplexities, offering a sophisticated solution for dermatological diagnostics research.
文摘Cumulative assessment is a tool for the project developer to try and take into consideration not only their contribution to cumulative impacts but also other projects and external factors that may place their developments at risk. This study assessed the cumulative impacts of air emissions from 22 major power plants in southeast Bangladesh planned to generate 21,550 MW of electricity. It also includes anticipated growth in small to medium size industries, brickfields, highway traffic, inland water transport, transhippers, jetty, and vessel transports used for transporting fuel resources for these power plants.A 50 km by 50 km airshed is considered for air quality modeling.Cumulative analysis indicates that predicted MGLCs(Maximum Ground Level Concentrations)of NO2 and CO are complying with both Bangladesh NAAQS(National Ambient Air Quality Standards)and WBG(World Bank Group)Guidelines. The daily average MGLC of PM_(2.5)(62.45µg/m^(3))from all sources complies with NAAQS,however, exceeds the WBG Guidelines.Annual PM_(2.5) concentration(15.45µg/m^(3))exceeds NAAQS and WBG Guidelines. The PM10 concentration complies with the NAAQS for both 24-hour and annual averaging times.Annual average concentration(23.12µg/m^(3))exceeds WBG Guidelines.Daily average SO2 concentration(102.49µg/m^(3))complies with the NAAQS however,it exceeds the WBG guideline values.High concentrations of PM_(2.5) and SO2 are due to the contribution of transboundary emissions and secondary pollutants in the atmosphere. This dispersion modeling outcome can be used by the policymakers for the pollution reduction strategy.

文摘Proppant transport within fractures is one of the most critical tasks in oil,gas and geothermal reservoir stimulation,as it largely determines the ultimate performance of the operating well.Proppant transport in rough fracture networks is still a relatively new area of research and the associated transport mechanisms are still unclear. In this study, representative parameters of rough fracture surfaces formed by supercritical CO_(2) fracturing were used to generate a rough fracture network model based on a spectral synthesis method.Computational fluid dynamics(CFD)coupled with the discrete element method(DEM)was used to study proppant transport in this rough fracture network. To reveal the turning transport mechanism of proppants into branching fractures at the intersections of rough fracture networks,a comparison was made with the behavior within smooth fracture networks, and the effect of key pumping parameters on the proppant placement in a secondary fracture was analyzed. The results show that the transport behavior of proppant in rough fracture networks is very different from that of the one in the smooth fracture networks. The turning transport mechanisms of proppant into secondary fractures in rough fracture networks are gravity-driven sliding, high velocity fluid suspension, and fracture structure induction.Under the same injection conditions, supercritical CO_(2)with high flow Reynolds number still has a weaker ability to transport proppant into secondary fractures than water.Thickening of the supercritical CO_(2)needs to be increased beyond a certain value to have a significant effect on proppant carrying, and under the temperature and pressure conditions of this paper,it needs to be increased more than 20 times(about 0.94 m Pa s).Increasing the injection velocity and decreasing the proppant concentration facilitates the entry of proppant into the branching fractures, which in turn results in a larger stimulated reservoir volume. The results help to understand the proppant transport and placement process in rough fracture networks formed by reservoir stimulation, and provide a theoretical reference for the optimization of proppant pumping parameters in hydraulic fracturing.

文摘Distinct brain remodeling has been found after different nerve reconstruction strategies, including motor representation of the affected limb. However, differences among reconstruction strategies at the brain network level have not been elucidated. This study aimed to explore intranetwork changes related to altered peripheral neural pathways after different nerve reconstruction surgeries, including nerve repair,endto-end nerve transfer, and end-to-side nerve transfer.Sprague–Dawley rats underwent complete left brachial plexus transection and were divided into four equal groups of eight:no nerve repair, grafted nerve repair, phrenic nerve end-to-end transfer, and end-to-side transfer with a graft sutured to the anterior upper trunk.Resting-state brain functional magnetic resonance imaging was obtained 7 months after surgery. The independent component analysis algorithm was utilized to identify group-level network components of interest and extract resting-state functional connectivity values of each voxel within the component.Alterations in intra-network resting-state functional connectivity were compared among the groups.Target muscle reinnervation was assessed by behavioral observation(elbow flexion)and electromyography. The results showed that alterations in the sensorimotor and interoception networks were mostly related to changes in the peripheral neural pathway.Nerve repair was related to enhanced connectivity within the sensorimotor network, while end-to-side nerve transfer might be more beneficial for restoring control over the affected limb by the original motor representation. The thalamic-cortical pathway was enhanced within the interoception network after nerve repair and end-to-end nerve transfer.Brain areas related to cognition and emotion were enhanced after end-to-side nerve transfer. Our study revealed important brain networks related to different nerve reconstructions. These networks may be potential targets for enhancing motor recovery.

文摘Skeletal muscle has a robust regeneration ability that is impaired by severe injury, disease, and aging.resulting in a decline in skeletal muscle function. Therefore, improving skeletal muscle regeneration is a key challenge in treating skeletal muscle-related disorders.Owing to their significant role in tissue regeneration, implantation of M2 macrophages(M2MФ)has great potential for improving skeletal muscle regeneration. Here,we present a short-wave infrared(SWIR)fluorescence imaging technique to obtain more in vivo information for an in-depth evaluation of the skeletal muscle regeneration effect after M2MФtransplantation.SWIR fluorescence imaging was employed to track implanted M2MФin the injured skeletal muscle of mouse models. It is found that the implanted M2MФaccumulated at the injury site for two weeks. Then,SWIR fluorescence imaging of blood vessels showed that M2MФimplantation could improve the relative perfusion ratio on day 5(1.09±0.09 vs 0.85±0.05;p=0.01)and day 9(1.38±0.16 vs 0.95±0.03;p=0.01)post-injury,as well as augment the degree of skeletal muscle regencration on day 13 post-injury.Finally,multiple linear regression analyses determined that post-injury time and relative perfusion ratio could be used as predictive indicators to evaluate skeletal muscle regeneration. These results provide more in vivo details about M2MФin skeletal muscle regeneration and confirm that M2MФcould promote angiogenesis and improve the degree of skeletal muscle repair, which will guide the research and development of M2MФimplantation to improve skeletal muscle regeneration.

文摘For patients with chronic spinal cord injury, the co nventional treatment is rehabilitation and treatment of spinal cord injury complications such as urinary tract infection, pressure sores, osteoporosis, and deep vein thrombosis.Surgery is rarely perfo rmed on spinal co rd injury in the chronic phase, and few treatments have been proven effective in chronic spinal cord injury patients.Development of effective therapies fo r chronic spinal co rd injury patients is needed. We conducted a randomized controlled clinical trial in patients with chronic complete thoracic spinal co rd injury to compare intensive rehabilitation(weight-bearing walking training)alone with surgical intervention plus intensive rehabilitation. This clinical trial was registered at ClinicalTrials.gov(NCT02663310).The goal of surgical intervention was spinal cord detethering, restoration of cerebrospinal fluid flow, and elimination of residual spinal cord compression. We found that surgical intervention plus weight-bearing walking training was associated with a higher incidence of American Spinal Injury Association Impairment Scale improvement, reduced spasticity, and more rapid bowel and bladder functional recovery than weight-bearing walking training alone.Overall, the surgical procedures and intensive rehabilitation were safe.American Spinal Injury Association Impairment Scale improvement was more common in T7-T11 injuries than in T2-T6 injuries.Surgery combined with rehabilitation appears to have a role in treatment of chronic spinal cord injury patients.

文摘A new genus and species,Cretopleciofungivora simpsoni gen.et sp.nov.,from the extinct family Pleciofungivoridae(Diptera:Bibionomorpha),is discovered in mid-Cretaceous Kachin amber.Previously, this family was known only from imprints in sedimentary rocks of the Jurassic and the Lower Cretaceous.Discovery of a representative of Pleciofungivoridae in Kachin amber confirms the presence of the family in the Upper Cretaceous. The new species has a unique structure of fore tarsus, with lobed and extended tarsal segments Ⅱ to Ⅳ,a feature hitherto known only in a few species of extant Sciaroidea.Although not particularly rare, the new species is currently known only from female specimens.Possible reasons for this phenomenon, very unusual in Sciaroidea, are briefly discussed, including parthenogenesis as a potentially plausible hypothesis.

文摘Federated Learning(FL),a burgeoning technology, has received increasing attention due to its privacy protection capability. However, the base algorithm FedAvg is vulnerable when it suffers from so-called backdoor attacks.Former researchers proposed several robust aggregation methods.Unfortunately, due to the hidden characteristic of backdoor attacks, many of these aggregation methods are unable to defend against backdoor attacks. What's more, the attackers recently have proposed some hiding methods that further improve backdoor attacks'stealthiness, making all the existing robust aggregation methods fail.To tackle the threat of backdoor attacks,we propose a new aggregation method,X-raying Models with A Matrix(XMAM),to reveal the malicious local model updates submitted by the backdoor attackers.Since we observe that the output of the Softmax layer exhibits distinguishable patterns between malicious and benign updates, unlike the existing aggregation algorithms,we focus on the Softmax layer's output in which the backdoor attackers are difficult to hide their malicious behavior.Specifically, like medical X-ray examinations,we investigate the collected local model updates by using a matrix as an input to get their Softmax layer's outputs. Then,we preclude updates whose outputs are abnormal by clustering.Without any training dataset in the server, the extensive evaluations show that our XMAM can effectively distinguish malicious local model updates from benign ones. For instance, when other methods fail to defend against the backdoor attacks at no more than 20%malicious clients, our method can tolerate 45%malicious clients in the black-box mode and about 30%in Projected Gradient Descent(PGD)mode.Besides, under adaptive attacks, the results demonstrate that XMAM can still complete the global model training task even when there are 40%malicious clients. Finally,we analyze our method's screening complexity and compare the real screening time with other methods. The results show that XMAM is about 10–10000 times faster than the existing methods.

文摘This study investigates resilient platoon control for constrained intelligent and connected vehicles(ICVs)against F-local Byzantine attacks. We introduce a resilient distributed model-predictive platooning control framework for such ICVs.This framework seamlessly integrates the predesigned optimal control with distributed model predictive control(DMPC)optimization and introduces a unique distributed attack detector to ensure the reliability of the transmitted information among vehicles.Notably, our strategy uses previously broadcasted information and a specialized convex set, termed the“resilience set”,to identify unreliable data. This approach significantly eases graph robustness prerequisites, requiring only an(F+1)-robust graph,in contrast to the established mean sequence reduced algorithms, which require a minimum(2F+1)-robust graph.Additionally,we introduce a verification algorithm to restore trust in vehicles under minor attacks, further reducing communication network robustness. Our analysis demonstrates the recursive feasibility of the DMPC optimization.Furthermore, the proposed method achieves exceptional control performance by minimizing the discrepancies between the DMPC control inputs and predesigned platoon control inputs, while ensuring constraint compliance and cybersecurity.Simulation results verify the effectiveness of our theoretical findings.
文摘Background:There is mounting evidence that regular physical activity is an important prerequisite for healthy cognitive aging.Consequently, the finding that almost one-third of the adult population does not reach the recommended level of regular physical activity calls for further public health actions. In this context, digital and home-based physical training interventions might be a promising alternative to center-based intervention programs. Thus, this systematic review aimed to summarize the current state of the literature on the effects of digital and home-based physical training interventions on adult cognitive performance. Methods:In this pre-registered systematic review(PROSPERO;ID:CRD42022320031),5 electronic databases(PubMed,Web of Science,Psyclnfo,SPORTDiscus, and Cochrane Library)were searched by 2 independent researchers(FH and PT)to identify eligible studies investigating the effects of digital and home-based physical training interventions on cognitive performance in adults. The systematic literature search yielded 8258 records(extra17 records from other sources),of which 27 controlled trials were considered relevant. Two reviewers(FH and PT)independently extracted data and assessed the risk of bias using a modified version of the Tool for the assEssment of Study qualiTy and reporting in EXercise(TESTEX scale). Results:Of the 27 reviewed studies,15 reported positive effects on cognitive and motor-cognitive outcomes(i.e.,performance improvements in measures of executive functions, working memory, and choice stepping reaction test),and a considerable heterogeneity concerning study-related,population-related,and intervention-related characteristics was noticed.A more detailed analysis suggests that,in particular, interventions using online classes and technology-based exercise devices(i.e.,step-based exergames)can improve cognitive performance in healthy older adults.Approximately one-half of the reviewed studies were rated as having a high risk of bias with respect to completion adherence(≤85%)and monitoring of the level of regular physical activity in the control group. Conclusion:The current state of evidence concerning the effectiveness of digital and home-based physical training interventions is mixed overall, though there is limited evidence that specific types of digital and home-based physical training interventions(e.g.,online classes and step-based exergames)can be an effective strategy for improving cognitive performance in older adults. However, due to the limited number of available studies, future high-quality studies are needed to buttress this assumption empirically and to allow for more solid and nuanced conclusions.
文摘Background:Acute improvement in range of motion(ROM)is a widely reported effect of stretching and foam rolling, which is commonly explained by changes in pain threshold and/or musculotendinous stiffness.Interestingly, these effects were also reported in response to various other active and passive interventions that induce responses such as enhanced muscle temperature. Therefore,we hypothesized that acute ROM enhancements could be induced by a wide variety of interventions other than stretching or foam rolling that promote an increase in muscle temperature. Methods:After a systematic search in PubMed,Web of Science, and SPORTDiscus databases,38 studies comparing the effects of stretching and foam rolling with several other interventions on ROM and passive properties were included. These studies had 1134 participants in total, and the data analysis resulted in 140 effect sizes(ESs).ES calculations were performed using robust variance estimation model with R-package.Results:Study quality of the included studies was classified as fair(PEDro score=4.58)with low to moderate certainty of evidence. Results showed no significant differences in ROM(ES=0.01,p=0.88),stiffness(ES=0.09,p=0.67),or passive peak torque(ES=-0.30,p=0.14)between stretching or foam rolling and the other identified activities.Funnel plots revealed no publication bias. Conclusion:Based on current literature, our results challenge the established view on stretching and foam rolling as a recommended component of warm-up programs. The lack of significant difference between interventions suggests there is no need to emphasize stretching or foam rolling to induce acute ROM,passive peak torque increases,or stiffness reductions.

文摘In Chinese medicine, practitioners assess patients’ complaints, analyze their underlying problems, identify causes and come to a diagnosis, which then directs treatment. Awareness of our clinical reasoning process will mitigate against biases, improve our diagnoses and treatment choices and support the training of students.
