Cybercriminals often use fraudulent emails and fictitious email accounts to deceive individuals into disclosing confidential information,a practice known as phishing.This study utilizes three distinct methodologies,Term Frequency-Inverse Document Frequency,Word2Vec,and Bidirectional Encoder Representations from Transform-ers,to evaluate the effectiveness of various machine learning algorithms in detecting phishing attacks.The study uses feature extraction methods to assess the performance of Logistic Regression,Decision Tree,Random Forest,and Multilayer Perceptron algorithms.The best results for each classifier using Term Frequency-Inverse Document Frequency were Multilayer Perceptron(Precision:0.98,Recall:0.98,F1-score:0.98,Accuracy:0.98).Word2Vec’s best results were Multilayer Perceptron(Precision:0.98,Recall:0.98,F1-score:0.98,Accuracy:0.98).The highest performance was achieved using the Bidirectional Encoder Representations from the Transformers model,with Precision,Recall,F1-score,and Accuracy all reaching 0.99.This study highlights how advanced pre-trained models,such as Bidirectional Encoder Representations from Transformers,can significantly enhance the accuracy and reliability of fraud detection systems. 展开更多

Change in Functional Status from Preoperative to One Year Postoperative in Patients Who Have Undergone Elective Open-Heart Surgery: A Repeated-Measures Study

Purpose: Although patient-related factors affect surgical outcomes, preoperative functional status is not measured by any cardiac risk score. Functional status can, however, be objectively measured using validated outcome tools such as the Late-Life Function and Disability Instrument (LLFDI). The purpose of this study was to determine 1) if there was a change over time in functional status, as measured by the LLFDI, in patients who underwent elective cardiac surgery, and if so, 2) what specific aspect(s) of functional status changed. Methods: A prospective longitudinal study of one year was conducted on elective cardiac surgery patients (n = 43) using the self-reported LLFDI, which measures Disability Frequency (frequency of participation in social tasks), Disability Limitation (ability to participate in social tasks) and Function Total (ease in performing routine activities). Higher scores indicate increased function and decreased disability. LLFDI scores were compared at three times (preoperative, six-week and one-year postoperative) using repeated measures ANOVA. Post hoc pairwise comparison was conducted for specific interactions. Results: Both Function Total and Disability Frequency significantly changed over time (p = 0.047 and p = 0.013, respectively). Specifically, patients’ function level was significantly higher one-year postoperative compared to preoperative (M difference = +3.48, SE = 1.48, p = 0.026). Likewise, Disability Frequency scores were significantly higher (i.e. more active) at one-year postoperative versus preoperative (M difference= +5.98, SE = 2.19, p = 0.033). Disability Limitation scores were not significantly different between any time points (p > 0.05). Conclusion: By one-year postoperative, patients demonstrated increased ease in their routine physical activities and were more participatory in social life tasks. Individuals who underwent elective cardiac surgery took more than six weeks to detect notable improvement in functional status, which was expected with a sternotomy approach. This study provides support for the use of the LLFDI as an effective tool to capture functional status in the cardiac population. These findings may assist cardiac patients in recovery timeline expectations. 展开更多

The model performance in simulating soil water content(SWC) is crucial for successfully modeling earth’s system,especially in high mountainous areas.In this study,the performance of Community Land Model 5.0(CLM5.0) in simulating liquid SWC was evaluated against observations from nine in-situ sites in the upper reach of the Heihe River Watershed(HRW),Northwest China.The CLM5.0 shows reliable performance in the study area with correlation coefficients(R) ranging between 0.79–0.93,root mean standard errors(RMSE)ranging between 0.044–0.097 m^(3)/m^(3),and the mean bias(BIAS) ranging between-0.084–0.061 m^(3)/m^(3).The slightly worse performance of CLM5.0 than CLM4.5 on alpine meadow and grassland is mainly caused by the revised canopy interception parameterization.The CLM5.0 overestimates interception and underestimates evapotranspiration(ET) on both alpine meadow and grassland during the growth period.The systematical overestimations at all the grassland sites indicate that the underestimation of ET is much larger than the overestimation of interception on grassland during growth period,while the errors of simulated interception and ET are partially canceled out on alpine meadow.Moreover,the underestimation of ET is more responsible for the overestimation of SWC than the overestimation of interception in the high mountainous area.It is necessary to estimate reasonable empirical parameter α(proportion of leaf water collection area) in interception parameterization scheme and further improve the dry surface layerbased soil evaporation resistance parameterization introduced in CLM5.0 in future researches.The performance of CLM5.0 is better under completely frozen stage than thawing stage and freezing stage,because of low variations of liquid SWC caused by extremely low hydraulic conductivity of soils.The underestimation of liquid SWC under frozen state is caused by underestimation of soil temperature,which leads to more ice mass and less liquid water in total water content. 展开更多

The highway capacity manual(HCM)provides a formula to calculate the heavy vehicle adjustment factor(fHV)as a function of passenger car equivalent factors for the heavy vehicle(ET).However,a significant drawback is that the methodology was established solely based on human-driven passenger cars(HDPC)and human-driven heavy vehicles(HDHV).Due to automated passenger cars(APCs),a new adjustment factor(fAV)might be expected.This study simulated traffic flows at different percentages of HDHVs and APCs to investigate the impacts of HDHVs and APCs on freeway capacity by analyzing their influence on fHV and fAV values.The simulation determined observed adjustment factors at different percentages of HDHVs and APCs(fobserved).The HCM formula was used to calculate(fHCM).Modifications to the HCM formula are proposed,and vehicle adjustment factors due to HDHVs and APCs were calculated(fproposed).Results showed that,in the presence of APCs,while fobserved and fHCM were statistically significantly different,fobserved and fproposed were statistically equal.Hence,this study recommends using the proposed formula when determining vehicle adjustment factors(fproposed)due to HDHVs and APCs in the traffic stream. 展开更多

目的:从学习兴趣培养角度验证实战情境课程模式的作用,为深化体育教学改革提供理论依据,同时,为培养学生学习兴趣提供实践指导。方法:选取某普通高校二年级114名学生为实验对象,其中实验组男女生共53人,对照组男女生共61人。通过12周教... 目的:从学习兴趣培养角度验证实战情境课程模式的作用,为深化体育教学改革提供理论依据,同时,为培养学生学习兴趣提供实践指导。方法:选取某普通高校二年级114名学生为实验对象,其中实验组男女生共53人,对照组男女生共61人。通过12周教学实验干预,并运用多元协方差分析两种教学模式对大学生体育学习情境兴趣的影响。结果:在排除学生性别、体育锻炼行为阶段、排球学习经历,以及体质健康状况等因素后,组别对学习积极兴趣、消极兴趣与自主学习的主效应,具有统计学意义(F=15.410、8.764,P=0.001、0.004<0.01;F=6.197,P=0.014<0.05),而对关注与锻炼程度的主效应,具有边缘性统计学意义(F=3.415,P=0.067>0.05)。结论:实战情境课程模式有利于促进学生学习积极兴趣、自主学习与缓解学生学习消极兴趣;对关注与锻炼程度的作用不是非常显著,但存利好趋势。 展开更多

Peripheral nerve injuries commonly occur due to trauma,like a traffic accident.Peripheral nerves get severed,causing motor neuron death and potential muscle atrophy.The current golden standard to treat peripheral nerve lesions,especially lesions with large(≥3 cm)nerve gaps,is the use of a nerve autograft or reimplantation in cases where nerve root avulsions occur.If not tended early,degeneration of motor neurons and loss of axon regeneration can occur,leading to loss of function.Although surgical procedures exist,patients often do not fully recover,and quality of life deteriorates.Peripheral nerves have limited regeneration,and it is usually mediated by Schwann cells and neurotrophic factors,like glial cell line-derived neurotrophic factor,as seen in Wallerian degeneration.Glial cell line-derived neurotrophic factor is a neurotrophic factor known to promote motor neuron survival and neurite outgrowth.Glial cell line-derived neurotrophic factor is upregulated in different forms of nerve injuries like axotomy,sciatic nerve crush,and compression,thus creating great interest to explore this protein as a potential treatment for peripheral nerve injuries.Exogenous glial cell line-derived neurotrophic factor has shown positive effects in regeneration and functional recovery when applied in experimental models of peripheral nerve injuries.In this review,we discuss the mechanism of repair provided by Schwann cells and upregulation of glial cell line-derived neurotrophic factor,the latest findings on the effects of glial cell line-derived neurotrophic factor in different types of peripheral nerve injuries,delivery systems,and complementary treatments(electrical muscle stimulation and exercise).Understanding and overcoming the challenges of proper timing and glial cell line-derived neurotrophic factor delivery is paramount to creating novel treatments to tend to peripheral nerve injuries to improve patients'quality of life. 展开更多

The management of a combination of fracture and multiligament knee injury(MKI) in traumatic knee injury remains controversial, and there are evolving treatment recommendations. Currently, there are no studies focusing on older adult patients with MKI's in combination with tibia fractures. As a result, there is no well-established treatment algorithm for older adult patients with these complex injuries. We report two cases of MKI's with concomitant fractures in patients fifty years of age or older. Both patients were treated surgically for their associated tibial plateau fractures, but were managed with conservative treatment of the multiligamentous knee injuries. We also provide a review of the literature and guidelines for older adult patients with these types of complex traumatic injuries. Early to mid term acceptable outcomes were achieved for both patients through surgical fixation of the tibial plateau fracture and conservative treatment of the ligament injuries. We propose a comprehensive treatment algorithm for management of these complex injuries. 展开更多

Endoscopic submucosal dissection(ESD) is a wellestablished, minimally invasive treatment for superficial neoplasms of the gastrointestinal tract. The universal adoption of ESD has been limited by its slow learning curve, long procedure times, and high risk of complications. One technical challenge is the lack of a second hand that can provide traction, as in conventional surgery. Reliable tissue retraction that exposes the submucosal plane of dissection would allow for safer and more efficient dissection. Magnetic anchor guided endoscopic submucosal dissection(MAGESD) has potential benefits compared to other current traction methods. MAG-ESD offers dynamic tissue retraction independent of the endoscope mimicking a surgeon's "second hand". Two types of magnets can be used: electromagnets and permanent magnets. In this article we review the MAG-ESD technology, published work and studies of magnets in ESD. We also review the use of magnetic anchor guidance systems in natural orifice transluminal endoscopic surgery and the idea of magnetic non-contact retraction using surface ferromagentization. We discuss the current limitations, the future potential of MAG-ESD and the developments needed for adoption of this technology. 展开更多

BACKGROUND Spontaneous peritonitis is an infection of ascitic fluid without a known intraabdominal source of infection. spontaneous fungal peritonitis (SFP) is a potentially fatal complication of decompensated cirrhosis, defined as fungal infection of ascitic fluid in the presence of ascitic neutrophil count of greater than 250 cells/mL. AIM To determine the prevalence of fungal pathogens, management and outcomes (mortality) of SFP in critically ill cirrhotic patients. METHODS Studies were identified using PubMed, EMBASE, Cochrane Central Register of Controlled Trials and Scopus databases until February 2019. Inclusion criteria included intervention trials and observation studies describing the association between SFP and cirrhosis. The primary outcome was in-hospital, 1-mo, and 6- mo mortality rates of SFP in cirrhotic patients. Secondary outcomes were fungal microorganisms identified and in hospital management by anti-fungal medications. The National Heart, Lung and Blood Institute quality assessment tools were used to assess internal validity and risk of bias for each included study. RESULTS Six observational studies were included in this systematic review. The overall quality of included studies was good. A meta-analysis of results could not be performed because of differences in reporting of outcomes and heterogeneity of the included studies. There were 82 patients with SFP described across all the included studies. Candida species, predominantly Candida albicans was the fungal pathogen in majority of the cases (48%-81.8%) followed by Candida krusei (15%- 25%) and Candida glabrata (6.66%-20%). Cryptococcus neoformans (53.3%) was the other major fungal pathogen. Antifungal therapy in SFP patients was utilized in 33.3% to 81.8% cases. The prevalence of in hospital mortality ranged from 33.3% to 100%, whereas 1-mo mortality ranged between 50% to 73.3%. CONCLUSION This systematic review suggests that SFP in end stage liver disease patient is associated with high mortality both in the hospital and at 1-mo, and that antifungal therapy is currently underutilized. 展开更多

A Modified Groundwater Module in SWAT for Improved Streamflow Simulation in a Large, Arid Endorheic River Watershed in Northwest China 被引量:6

Interactions between surface water and groundwater are dynamic and complex in large endorheic river watersheds in Northwest China due to the influence of both irrigation practices and the local terrain. These interactions interchange numerous times throughout the middle reaches, making streamflow simulation a challenge in endorheic river watersheds. In this study, we modified the linear-reservoir groundwater module in SWAT(Soil and Water Assessment Tools, a widely used hydrological model) with a new nonlinear relationship to better represent groundwater processes; we then applied the original SWAT and modified SWAT to the Heihe River Watershed, the second largest endorheic river watershed in Northwest China, to simulate streamflow. After calibrating both the original SWAT model and the modified SWAT model, we analyzed model performance during two periods: an irrigation period and a non-irrigation period. Our results show that the modified SWAT model with the nonlinear groundwater module performed significantly better during both the irrigation and non-irrigation periods. Moreover, after comparing different runoff components simulated by the two models, the results show that, after the implementation of the new nonlinear groundwater module in SWAT, proportions of runoff components changed-and the groundwater flow had significantly increased, dominating the discharge season. Therefore, SWAT coupled with the non-linear groundwater module represents the complex hydrological process in the study area more realistically. Moreover, the results for various runoff components simulated by the modified SWAT models can be used to describe the hydrological characteristics of lowland areas. This indicates that the modified SWAT model is applicable to simulate complex hydrological process of arid endorheic rivers. 展开更多
